De Garengeot hernia, wherein the appendix is present within a femoral hernia, is a rare disease; therefore, the clinicopathological features remain to be clarified. This study aimed to reveal the clinicopathological characteristics of De Garengeot hernia.
Six patients who underwent appendectomy and herniorrhaphy between 1999 and 2018 were included. The incidence of De Garengeot hernia was 3.2% among the 182 femoral hernias that required surgery during the study period. The median age of the patients was 78years, and five patients were women. The median body mass index was 20.1. Patients frequently had fever or elevated CRP level. Preoperative diagnoses based on computed tomography were femoral (n = 3), inguinal (n = 2), and De Garengeot (n = 1) hernias. Emergency and elective surgeries were performed in four and two patients, respectively. Histopathological examination of the resected appendix showed gangrenous appendicitis (n = 3), perforated appendicitis (n = 2), and appendiceal ischemia (n = 1) in the patients. Postoperatively, one patient developed sepsis.

Low back pain (LBP) is a major public health problem worldwide. Significant practice variation exists despite guidelines, including strong interventionist focus by some practitioners. Translation of guidelines into pathways as integrated treatment plans is a next step to improve implementation. The goal of the present study was to analyze international examples of LBP pathways in order to identify key interventions as building elements for care pathway for LBP and radicular pain.
International examples of LBP pathways were searched in literature and grey literature. Authors of pathways were invited to fill a questionnaire and to participate in an in-depth telephone interview. Pathways were quantitatively and qualitatively analyzed, to enable the identification of key interventions to serve as pathway building elements.
Eleven international LBP care pathways were identified. Regional pathways were strongly organized and included significant training efforts for primary care providers and an intermediate level of caregivers in between general practitioners and hospital specialists. Hospital pathways had a focus on multidisciplinary collaboration and stepwise approach trajectories. Key elements common to all pathways included the consecutive screening for red flags, radicular pain and psychosocial risk factors, the emphasis on patient empowerment and self-management, the development of evidence-based consultable protocols, the focus on a multidisciplinary work mode and the monitoring of patient-reported outcome measures.

One of his most important insights deals with the method of interviewing in psychopathology. Specifically, how an interviewer (i.e. psychopathologist) approaches and explores mental phenomena that present themselves in the person being interviewed. By cultivating a phenomenological attitude, clinicians also acquire and renew self-criticism (Selbskritik), an essential aspect of a true psychopathologist. A fundamental part of the critical attitude in descriptive psychopathology is the re-examination of prejudices present in psychopathological interviewing. This review outlines and discusses six types of prejudices, as abstracted by Karl Jaspers, and illustrates them with clinical examples. (i) Philosophical prejudice deals with deductive and inductive reasoning; (ii) theoretical prejudice addresses the modeling of understanding of mental phenomena by analogy with natural sciences; (iii) somatic prejudice addresses biological reductionism in psychopathology; (iv) psychological prejudice focuses on approaches that consist of psychological interpretations that transgress the boundaries of psychological intelligibility; (v) pictorial prejudice deals with conceptualizations of psychic life as an analogy with certain visual depictions; (vi) medical prejudice is connected with strivings towards quantity, objectivity and diagnostic precision in psychopathology. To have the ability to represent the inner life of a person, we must dispense with prejudices in the clinical encounter with our patients. With the demands of modern-day life, and requests for quick clinical assessments, the idea of examining our prejudices and performing phenomenological interviews is more important than ever.
