Dural venous sinus injury is a rare complication of otological surgery that can lead to life-threatening sequelae, the management of which is complex and poorly described.
This paper describes the case of a 40-year-old female who underwent routine right myringoplasty complicated by sigmoid sinus laceration. The patient subsequently developed right-sided lateral sinus thrombosis leading to fulminant intracranial hypertension. The patient underwent successful emergency management by surgical reconstruction of the sigmoid sinus, followed by endovascular thrombolysis, catheter balloon angioplasty and endovascular stenting.
Torrential haemorrhage following otological procedures is uncommon and rarely requires packing of a bleeding venous sinus. This case highlights that injury to a highly dominant venous sinus can lead to venous outflow obstruction and life-threatening intracranial hypertension. To our knowledge, the development of this complication following otological surgery and its management has not been reported previously.

Acute epistaxis can be a life-threatening airway emergency, requiring in-patient admission. The coronavirus disease 2019 pandemic placed significant strain on hospital resources, and management has shifted towards an out-patient-centred approach.
A five-month single-centre retrospective study was undertaken of all epistaxis patients managed by the ENT department. A pre-coronavirus disease 2019 pandemic group was managed with pre-existing guidelines, compared to new guidelines for the coronavirus disease 2019 pandemic group. A telephone survey was performed on out-patients with non-dissolvable packs to assess patient comfort and satisfaction.
A total of 142 patients were seen. The coronavirus disease 2019 pandemic group had significantly more patients aged over 65 years (p = 0.004), an increased use of absorbable dressings and local haemostatic agents (Nasopore and Surgiflo), and fewer admissions (all p < 0.0005). Rates of re-presentation and morbidity, and length of hospital stay were similar. The telephone survey revealed out-patient management to be efficacious and feasible.
The coronavirus disease 2019 pandemic has shifted epistaxis management towards local haemostatic agents and out-patient management; this approach is as safe and effective as previously well-established regimens.

The author of these descriptions, Francesco Roncalli Parolino (1692-1769), was a renowned European physician. Roncalli Parolino focused his study on the treatment of scurvy and he promoted the inclusion of the Greenlandic and Faroese therapy into the broader European context. He was influenced to do this due to the already integrated European perspective of medicine which his book follows.Like now, medicine in 18th-century Europe was multicentric and characterised by rich intellectual activity, which contributed to the enhancement of clinical practice during this period. At the time, Greenland and Faroe Islands were also integrated into this European context because they contributed for medical-scientific development that would lay the foundations for modern medicine. Francesco Roncalli Parolino obtained just recognition for these regions through the advancement and defence of their valuable medical contributions.Petrol contains mixture of mutagens and carcinogens which have potential health risk after prolonged occupational exposure. We have compared genotoxicity and its amelioration in blood samples from 70 petrol pump attendants, working in congested area of the Ahmedabad city, India and similar number (n = 70) of Control samples from office workers dwelling in less polluted areas of the city. The cytokinesis-block micronucleus assay showed highly significant frequencies of micronucleus in Exposed than in the Controls. The sister chromatid exchanges were also significantly increased while the cell cycle proliferative index was significantly decreased in the Exposed individuals than the Controls. Addition of standardized doses of vitamin C and vitamin E in the lymphocyte cultures (in vitro) significantly improved all the biomarkers. The long-term occupational petrol exposure causes genotoxic effects and use of vitamins C and E for protection should be further explored in randomized controlled studies. The brick-kiln (BK) sector in Nepal is largely an informal sector. MK-4827 order This study investigated the influence of working and living conditions (WLCs) in BKs on productivity at two levels - BK level and workers' level - using primary data collected from 781 workers and 80 BK entrepreneurs in 12 districts of Nepal. WLCs were assessed based on the provision of nine amenities to workers at BKs. Correlation and regression analyses revealed that WLCs have a positive influence on both BK level as well as workers' level productivity. Moreover, large BKs with better investment in zig-zag technology and mechanization are more likely to spend on improving WLCs than small BKs, who are reluctant to invest in WLCs due mainly to a lack of financial resources. The study suggests an integrated approach emphasizing equally improved WLCs and cleaner technology in the BK sector to transform it into a healthier and socio-environmentally responsible industry. To maintain homoeostasis, cells must degrade damaged or misfolded proteins and synthesize functional replacements. Maintaining a balance between these processes, known as protein turnover, is necessary for stress response and cellular adaptation to a changing environment. Damaged mitochondria must also be removed and replaced. Changes in protein and mitochondrial turnover are associated with aging and neurodegenerative disease, making it important to understand how these processes occur and are regulated in cells. To achieve this, reliable assays of turnover must be developed. Several methods exist, including pulse-labelling with radioactive or stable isotopes and strategies making use of fluorescent proteins, each with their own advantages and limitations. Both cell culture and live animals have been used for these studies, in systems ranging from yeast to mammals. In vivo assays are especially useful for connecting turnover to aging and disease. With its short life cycle, suitability for fluorescent imaging, and availability of genetic tools, Drosophila melanogaster is particularly well suited for this kind of analysis.
