We aimed to compare the technical difficulties, complications, long-term efficacy, and risks between right- and left-sided approach transhepatic tunneled catheterization.
We retrospectively evaluated transhepatic tunneled catheter placement cases in our institution between May 2012 and November 2019. Demographic and procedural parameters were recorded. Statistical tests were used to compare the complication rates of right- and left-sided approach. Furthermore, Cox regression analyses were used to investigate the relationship between functional catheter days and included parameters.
A total of 83 procedures were performed in 46 patients, with a female to male ratio of 1.88 and a mean age of 55.5 ± 18.2years. Indication for catheter placement was chronic renal insufficiency and loss of central venous access through traditional routes in all cases. Median functional catheter durations were 28days (1-382) and 55.5days (1-780) for right-sided and left-sided access, respectively. Complication rates were similar for both sides. There was no difference between primary and revision procedures in terms of safety and efficacy outcomes. In univariate Cox regression analysis, gender was the only variable which was found to be statistically significant (HR = 2.014 (1.004-4.038)) for functional catheter days. selleck chemicals llc In multivariate Cox regression model, gender and access side were included which failed to reach statistical significance.

It may arise from liver infection or as a consequence of resection and thermal ablation of cancer. Currently, there is no consensus about the treatment strategy. Surgery is considered the main therapy by most authors. However, this systematic literature review shows that the success rate of percutaneous treatments may reach 75%. Adding to such evidence, we also report the case of a woman affected by iatrogenic bronchobiliary fistula secondary to liver thermal ablation, successfully treated with percutaneous drainage plus embolization. Summarizing these results, we encourage the percutaneous management of bronchobiliary fistula by providing a 3-step decision-making algorithm, aimed at reducing the need for major surgery.Complex oncological treatment can be associated with lymphatic vascular injury that is burdened by considerable morbidity. We report the case of a 59-year-old woman with an unusual lympho-veno-cutaneous fistula, diagnosed by magnetic resonance lymphangiography and treated by minimally invasive embolization therapy and venous recanalization.Two novel fluorescent peptide-based chemosensors, including A (2-amino-benzoyl-Ser-Glu-Glu-NH2) and B (2-amino-benzoyl-Ala-Glu-Pro-Glu-Ala-Glu-Pro-NH2) were synthesized and characterized by nuclear magnetic resonance (NMR) spectra. These fluorescent probes exhibited excellent selective and sensitive responses to Al3+ ions over other metal ions in aqueous buffered solutions. The limits of detection for both chemosensors towards the Al3+ ions were in the order of ∼10-7 M (A 155 nM and B 195 nM), which clearly indicates that these probes have significant potential for biological applications. They also displayed high binding affinity (1.3029 × 104 M-1 and 1.7586 × 104 M-1 relevant to A and B respectively). These two chemosensors are great analytical probes that produce turn-on responses upon binding to Al3+ ions through an intramolecular charge transfer (ICT) mechanism. In addition, the application of both chemosensors was examined over a wide range of pH. The fluorescent peptide-based probes and Al3+ form a 11 coordination complex according to the ESI-MS and Job's plot analysis. Notably, upon addition of Al3+ to these chemosensors, a fluorescence enhancement of approximately 8-fold was observed and the binding mode was determined using NMR titration and fluorescence emission data.Molecularly imprinted polymers (MIPs) are artificially synthesized materials to mimic the molecular recognition process of biological macromolecules such as substrate-enzyme or antigen-antibody. The combination of these biomimetic materials with electrochemical techniques has allowed the development of advanced sensing devices, which significantly improve the performance of bare or catalyst-modified sensors, being able to unleash new applications. However, despite the high selectivity that MIPs exhibit, those can still show some cross-response towards other compounds, especially with chemically analogous (bio)molecules. Thus, the combination of MIPs with chemometric methods opens the room for the development of what could be considered a new type of electronic tongues, i.e. sensor array systems, based on its usage. In this direction, this review provides an overview of the more common synthetic approaches, as well as the strategies that can be used to achieve the integration of MIPs and electrochemical sensors, followed by some recent examples over different areas in order to illustrate the potential of such combination in very diverse applications.Osteoporosis is a disease of low bone mass which leads to an increased risk of fracture. The prevalence of osteoporosis is greatest amongst postmenopausal women who also form a large portion of patients with osteoarthritis presenting for total knee arthroplasty. We present a case of a 77-year-old female with knee osteoarthritis and osteoporosis on denosumab who underwent robotic-assisted total knee arthroplasty. Postoperatively, she suffered an atraumatic midshaft femur fracture at the pin site associated with the use of intraoperative robotic technology. This is a unique complication related to the use of robotics which requires additional skeletal fixation versus standard arthroplasty surgical technique. We recommend caution and further study regarding the use of robotics in total knee arthroplasty in the setting of osteoporosis and denosumab use.
