P7C3 purchase El objetivo principal de las terapias de estimulación cognitiva consiste en promover la plasticidad y la capacidad de aprendizaje que aún se poseen en la vejez y retrasar las manifestaciones clínicas de procesos neurodegenerativos como la demencia tipo Alzheimer. Variables como la reserva cognitiva pueden mediar en los beneficios de la intervención. Objetivo. Conocer si existe interacción entre el nivel de reserva y la estimulación cognitiva, y si influye en el rendimiento cognitivo de sujetos con demencia tipo Alzheimer. Pacientes y métodos. Participaron 20 sujetos (edad 66-89 años) con demencia tipo Alzheimer, usuarios de un centro de día. Se utilizó un diseño pretest-postest controlado. El grupo experimental participó en la intervención durante seis meses. Se clasificó a los pacientes en dos niveles de reserva cognitiva (alta y baja) y posteriormente se aplicó una amplia batería neuropsicológica para un análisis exhaustivo de la cognición. Se analizaron las diferencias pre y postintervención a través de un ANOVA de dos factores, uno de medidas repetidas (puntuaciones pre y postintervención) y otro de medidas independientes (nivel de reserva cognitiva). Resultados. En las puntuaciones de los subtests de historietas (WAIS-III), fracasos para mantener la categoría y porcentaje de errores del test de clasificación de tarjetas de Wisconsin se encontró interacción. Conclusiones. Se ha podido observar el efecto positivo de la interacción en la función ejecutiva, concretamente en las capacidades de planificación y secuenciación, organización perceptiva, inhibición de respuesta, razonamiento lógico y flexibilidad mental.The musculoskeletal system has an integral role throughout life, including structural support to the body, protection, and allowing a range of fine to complex movements for daily living to elite sporting events. At various times, injuries to the musculoskeletal system occur resulting in varying levels of impact to the person both acutely and chronically. Specifically, there is a spectrum of complexity in orthopedic injuries, with some such as common muscle strains, that while burdensome will have no impact on life-long functional ability, and others that can result in long lasting disability. Focusing on extremity injuries, this review highlights i) the current impact of orthopedicinjuries in sport and daily life; ii) the foundation of bone and skeletal muscle repair and regeneration; and iii) the disruptions in regenerative healing due to traumatic orthopedic injuries. Neben ovariellen sind außerhalb der Schwangerschaft auch primär extraovarielle Dysgerminome des Mediastinums und Retroperitoneums beschrieben. Nach adäquater chirurgischer und systemischer Therapie besteht eine gute Prognose. METHODIK Bericht eines primär extraovariell-intraabdominalen Dysgerminoms in der Schwangerschaft, systematische Literaturrecherche. FALLBERICHT Bei einer 35-jährigen II Gravida wird in der 20. Schwangerschaftswoche ein suspekter intraabdominaler Tumor festgestellt. Bei der folgenden Explorativlaparotomie wird bei unauffälligen Ovarien ein Tumorkonglomerat mit Darminfiltration reseziert und ein Dysgerminom diagnostiziert. In der 34. SSW wird eine primäre Sectio Caesarea durchgeführt, erneuter Tumorbefall komplett reseziert und folgend systemisch Bleomycin, Etoposid und Cisplatin appliziert. Nach 12 Monaten erfolgt bei zystischen Adnexen eine erneute Exploration mit Hysterektomie und Adnexektomie beidseits ohne Nachweis von Malignität. Nach nun 16 Monaten besteht weiter eine Komplettremission. ZUSAMMENFASSUNG Präsentiert wird die Erstbeschreibung eines primär extraovariell-intraabdominalen Dysgerminoms in der Schwangerschaft. Durch die Entscheidung, die zytoreduktive Operation während und die Systemtherapie erst nach Prolongation der Schwangerschaft durchzuführen, konnte größtmöglicher Benefit für das Kind mit der günstigen Prognose der Mutter vereint werden. Angesichts limitierter Datenlage müssen bei Dysgerminomen in der Schwangerschaft individuelle, interdisziplinäre Konzepte erstellt werden.Chronic liver injury due to viral hepatitis, alcohol abuse, and metabolic disorders is a worldwide health concern. Insufficient treatment of chronic liver injury leads to fibrosis, causing liver dysfunction and carcinogenesis. Most cases of hepatocellular carcinoma (HCC) develop in the fibrotic liver. Pathological features of liver fibrosis include extracellular matrix (ECM) accumulation, mesenchymal cell activation, immune deregulation, and angiogenesis, all of which contribute to the precancerous environment, supporting tumor development. Among liver cells, hepatic stellate cells (HSCs) and macrophages play critical roles in fibrosis and HCC. These two cell types interplay and remodel the ECM and immune microenvironment in the fibrotic liver. Once HCC develops, HCC-derived factors influence HSCs and macrophages to switch to protumorigenic cell populations, cancer-associated fibroblasts and tumor-associated macrophages, respectively. This review aims to summarize currently available data on the roles of HSCs and macrophages in liver fibrosis and HCC, with a focus on their interaction.
