This study evaluated the effects of inclusion of purple prairie clover (PPC, Dalea purpurea Vent.), a native legume forage, with native cool-season grasses on the in vitro fermentation and in situ digestibility of mixed forages. Methods Whole plant PPC and mixtures of cool-season grasses were harvested when the PPC reached the vegetative (VEG), full flower (FL) and seedpod (SP) stages, and were combined in ratios (DM basis) of 0100, 2575, 5050, 7525 and 1000 at each maturity. In vitro ruminal incubations using these mixtures were conducted for 48 h to determine gas production (GP), in vitro DM disappearance (IVDMD), total volatile fatty acids (VFA) and ammonia-N production. BMS202 Mixtures of forages harvested when the PPC reached the FL stage and 5050 mixture of forages harvested at VEG, FL and SP stages were incubated in the rumen of three heifers for 0, 2, 6, 12, 24, 48, 72 and 96 h to determine in situ degradabilities of DM, neutral detergent fibre (aNDF) and crude protein (CP). Results Contents of aNDF and ADF increased (P  SP (P  less then  0.001), whereas the effective degradability of aNDF was similar between VEG and FL and both were greater (P  less then  0.01) than SP. Conclusions Inclusion of vegetative PPC in a mixed forage diet resulted in the greatest digestibility and incorporation of PPC before seedpod stage with native grasses had a positive effect on ruminal fermentation. Effects of PPC on ruminal digestion depend on both the stage of maturity and its proportion in mixed legume-grass pastures. Pastures containing 50% of PPC in full flower stage would likely provide the greatest quality diet to grazing ruminants subject to potential animal selectivity. © The Author(s). 2020.Background Determination of nutrient requirements in the late gestating and lactating sows is essential to optimize sow productivity. The objectives of the present study were to quantify amino acid (AA) fluxes and heat production across portal-drained viscera (PDV) and liver in multiparous sows during transition and lactation. Methods Eight second parity sows were fitted with indwelling catheters in the femoral artery and in the mesenteric, portal and hepatic veins. Eight hourly sets of blood samples were taken starting 0.5 h before feeding at - 10, - 3, + 3, and + 17 d in milk (DIM). Blood gases, plasma metabolites and apparent total tract digestibility (ATTD) of nutrients were measured. Results Feed intake, the ATTD of DM, energy, nitrogen, fat and crude fiber changed with DIM (P  less then  0.001). Except for Glu, O2, and urea, all net portal fluxes were positive, and all were affected by DIM (P  less then  0.05) and by sampling time (P  less then  0.01). Compared with pre partum levels, net portal uptake of AA was 3-63% lower at + 3 DIM but 40-100% higher at + 17 DIM. Net portal fluxes of AA peaked at 1.5 to 2.5 h after feeding except for Glu, and they were positively correlated with changes in sow feed intake across DIM. The net portal recovery was low for Met (49%), Thr (54%), and His (54%) and high for the remaining essential AA (63-69%) and none of them differed across DIM. Net hepatic uptake (i.e. hepatic oxidation) of Lys, Thr, Ile, Leu and Phe peaked at 0.5 to 2.5 h after feeding, whereas uptake of Trp, Val, and His was constant, while that of Met was close to zero. Conclusion The net portal recovery was substantially lower for Met, Thr, and His than the remaining essential AA. Hepatic AA oxidation peaks 0.5 to 2.5 h after feeding. The heat production in PDV and liver was approximately two-fold higher at peak lactation compared to other stages. The study suggests that lysine was the limiting AA in peak lactation but not in early lactation. © The Author(s). 2020.Background To reduce overutilization of laboratory testing many interventions have been tried, but selecting the most effective intervention for a given setting is challenging. To be sustainable, interventions need to align with healthcare providers' needs and daily practices. This study aimed to assess the extent of overutilization and the perspectives of healthcare providers, which may be used to guide the choice of intervention. Methods The extent of inappropriate laboratory testing in internal medicine inpatients was evaluated using a database. Surveys and focus groups were used to investigate healthcare providers' perceptions on its causes and solutions. Results On average, patients had 5.7 laboratory orders done during the first week of admission, whereas guidelines advise performing laboratory testing no more than twice per week. Repeat testing of normal test results occurred in up to 85% of patients. The frequency of laboratory testing was underestimated by survey responders, even though the majority of responders (78%) thought that laboratory tests are ordered too frequently. Residents were considered to be most responsible for laboratory test ordering.The primary causes of overutilization discussed were personal factors, such as a lack of awareness and knowledge, as well as feelings of insecurity. Regarding possible solutions, residents generally recommended educational interventions, whereas specialists tended to favour technical solutions such as lockouts. Conclusion Inappropriate laboratory testing is common in internal medicine. The most important causes are a lack of awareness and knowledge, especially in residents. The intervention most favoured by residents is education, suggesting educational interventions may be most applicable. © 2020 The Authors.
