Acute generalized exanthematous pustulosis (AGEP) is a rare cutaneous drug reaction that presents with the rapid onset of disseminated pustules, erythema, and edema. It is commonly associated with pristinamycin and aminopenicillins, but a few cases of vancomycin induced AGEP have been reported. Here, we describe a patient who presented with AGEP 12 hours following vancomycin administration, and had an atypical clinical course, with the AGEP persisting for weeks. Vancomycin was confirmed to be the etiologic agent with patch testing, a modality with growing evidence for utility in drug-induced reaction cases.The development of calcium salt deposits in the skin can occur in the presence or absence of membranous ossification and are categorized into osteoma cutis (i.e., cutaneous osteoma) and calcinosis cutis. For the former, distinction into primary or secondary osteoma cutis is mainly based on clinical and histopathological parameters, as primary osteoma cutis originates without any underlying intradermal inflammatory or neoplastic process, as opposed to a far greater number of secondary osteoma cutis that occur on the grounds of inflammation or tumors. Genetic disorders might predispose a person to the formation of these overall rare tumors. However, some patients develop primary osteoma cutis in the absence of any genetic background. In pre-menopausal women with fair skin, the condition of multiple miliary osteoma cutis is a relevant differential diagnosis for solid subcutaneous facial nodules. While pathogenesis remains unclear, most affected individuals have suffered from acne vulgaris at some point. Excision might be a viable option for disturbing lesions, as are ablative lasers. Here, we discuss and review relevant causes of calcium salt deposits in the skin based on a notable case of multiple primary osteoma cutis of the face in an otherwise healthy woman.BACKGROUND The study of the use of growth factors in conjunction with microneedling is limited. It is hypothesized that the use of human growth factors will decrease recovery time by downregulating inflammation and will continue stimulating collagen synthesis initiated by microneedling. OBJECTIVE We sought to evaluate the safety and efficacy of microneedling in combination with a novel human recombinant growth factor regenerative complex (PolyGF) for skin improvement. METHODS This study was a randomized controlled trial evaluating changes in multiple skin parameters via provider, imaging, and patient self-assessments after four microneedling treatments, each spaced one month apart, with or without a novel growth factor. Twenty female patients aged 35 to 60 years were included. Evaluation parameters included fine lines and wrinkles, evenness of skin tone, skin clarity, age spots, skin smoothness, skin firmness, and hydration of skin via dermatologic Canfield VISIA (Fairfield, New Jersey) imaging, and patient self-assessment. RESULTS Significant improvement in skin texture was reported to higher degrees for the PolyGF group by VISIA imaging assessments than in the control group. Improvements in firmness and texture were seen in both the PolyGF and control groups by subjective assessment. CONCLUSION While microneedling improved skin in several parameters, the addition of the novel growth factor to microneedling helped improve skin texture and hydration to a higher degree. Further studies are needed to characterize the effects of growth factor serum in conjunction with microneedling.Hailey-Hailey (HHD), or benign familial chronic pemphigus disease, is a rare autosomal dominant blistering disorder characterized by recurrent vesicles that erode and macerate into weeping and crusting plaques. HHD has been shown to be resistant to several treatment options. Although not yet approved as a treatment for HHD, recent reports have suggested the use of low-dose naltrexone (LDN) as a successful treatment option for controlling recalcitrant HHD. We present a case of a 50-year-old woman with a 20-year history of biopsy-confirmed HHD with recurrent painful and pruritic vesicles and plaques. The patient developed significant clinical improvement of the cutaneous lesions with LDN treatment after only 26 days of treatment. It is important for dermatologists to consider LDN as a viable treatment option for HHD, especially in recalcitrant patients. We suggest this novel treatment as a rapidly effective option to resistant HHD.BACKGROUND The neck is one of the most common areas affected by the aging process. A novel two product combination system composed of a serum and cream with hyaluronic acid and multiple strong antioxidants were investigated to determine their efficacy and safety in neck rejuvenation. CA77.1 purchase OBJECTIVE The objective of this prospective, randomized, double-blind, placebo-controlled clinical trial was to assess the efficacy and safety of a novel serum containing fractionated hyaluronic acid, peptides, and antioxidants for photodamage of the neck. METHODS This was an institutional review board (IRB)-approved, randomized, double blind, placebo-controlled clinical trial involving 31 healthy subjects with moderate-to-severe neck wrinkling corresponding to at least a Grade 2 in wrinkles and score of 4 in elastosis on the Fitzpatrick-Goldman Wrinkle Scale. Twenty subjects were randomized to receive the active cream and serum system, while 11 subjects were randomized to receive the vehicles alone in serum and cream format for a course of two months. RESULTS Both active and placebo cream and serum showed improvement of wrinkles, laxity, pigmentation, erythema, dryness, and texture of the skin, and high patient satisfaction scores. Histology of one of the active serums and cream samples revealed improvement in the quality of papillary dermal collagen and increase in the number of elastic fibers in the upper dermis after treatment. CONCLUSION Our prospective, randomized controlled trial showed that the novel serum and cream showed improvement in skin aging on the neck, was well-tolerated by patients, and had a high degree of patient satisfaction.
