Aqueous, ethanol, 50% ethanol extracts were tested using agar-gel overlay, nucleation, and artificial urine methods in comparison to marketed products. The aqueous extract of Sundernagar and Rampur seeds showed highest inhibition potential (9.0 ± 0.81 mm) than alcohol and hydroalcohol. Sundernagar seeds (aqueous extract) also showed maximum inhibition (45 ± 1.2 and 24 ± 0.88%) in nucleation and artificial urine assay, respectively. The Tris-buffer (pH. 8.0) extract of dry seeds showed more inhibition with low protein than high protein germinated seeds. The findings revealed that horse gram protein is not alone responsible for CaOx-crystals inhibition but its secondary metabolites may also contribute. Among all samples, buffer and aqueous extracts showed maximum activity, followed by hydroalcohol and alcohol. Further, TLC and UPLC method were used to monitor the presence of amino acids in aqueous and Cystone extracts. Sundernagar seeds contain 11 amino acids with additional arginine, aspargine, and methionine. Moreover, higher antioxidant potential was observed in seeds of high altitude, which relates the factors of altitudinal variation in secondary metabolism of plants. Here, we present a 85-year old caucasian lady presenting with GB syndrome and Syndrome of Inappropriate Antidiuretic Hormone (SIADH) who eventually got diagnosed with SMZL.Lemierre's syndrome (LS) is a complex medical condition that is characterized by an acute oropharyngeal infection leading to thrombophlebitis of the internal jugular vein and an eventual metastatic spread to distant vital organs. This metastatic spread is from septic emboli and is most frequently seen in the lungs, kidneys, and large joints. Central nervous system (CNS) involvement in LS is extremely rare, and only a few cases of CNS involvement have been reported in the literature. For all cases of LS, early diagnosis and treatment are crucial, yet due to the rarity of CNS complications in LS, diagnostic patterns and treatment algorithms are not fully understood for this subset of presentations. In this report, we present a case of 19-year-old immune-competent female who presented with a Fusobacterium oropharyngeal infection that was complicated by suppurative sinusitis, cavernous sinus thrombosis, meningitis, and an abducens nerve palsy. Our patient was treated with broad-spectrum antibiotics, anticoagulation, and multiple surgical interventions. Demographic presentation of AE is usually in the pediatric age group. However, there is recorded documentation of its occurrence even in adults. AE can present with unexplained, relentless, and recurrent gastrointestinal symptoms such as paroxysmal pain, nausea, bloating, and diarrhoea that improve with antiepileptic therapy. It is commonly linked with electroencephalography (EEG) changes in the temporal lobes along with symptoms that reflect the involvement of the central nervous system (CNS) such as altered consciousness, confusion, and lethargy. Due to the vague nature of these symptoms, there is a high chance of misdiagnosing a patient. We present the case of a 20-year-old man with AE who was misdiagnosed with psychogenic abdominal pain after undergoing multiple investigations with various hospital departments.The radio-immunization effects of radiotherapy with abscopal tumor regressions have been documented in several experimental and clinical studies. Here, we present a patient with bladder cancer and relapsed metastatic disease to the left supraclavicular/axillary area and left lung. Concurrent weekly hypofractionated radiotherapy of both areas (8Gy/fraction/week, four fractions in total) and bi-weekly immunotherapy with anti-programmed cell death protein 1 (anti-PD-1) monoclonal antibodies resulted in complete regression of the axillary metastatic masses and of the lung metastasis, three months after the onset of therapy. In CT scans, however, a sternum infiltrating mass growing proximal to the margins of the radiotherapy fields was strikingly evident, while multiple hepatic metastases also appeared. Lymphopenia during radio-immunotherapy was recorded. The current report does not confirm the abscopal effects of radio-immunotherapy and furthermore, suggests that progressive disease or even hyper-progression may occur in a subgroup of patients.
