Adenocarcinoma constitutes around 0.5 to 2% of all neoplastic lesions of the urinary bladder, out of which, the mucinous variant is seen on rare occasions. Intestinal metaplasia resulting from nephrolithiasis, chronic inflammation and hydronephrosis could be the precursor lesion. Bladder exstrophy and remnants of the urachus are considered as other possible risk factors. The tumour prefers patients who are over 50 years. Hematuria, difficult voiding and suprapubic pain are the most common presenting symptoms. The tumour has a very bad prognosis and is poorly responsive to chemo and radiotherapy and hence surgery is the treatment of choice. Hence early diagnosis is of paramount importance. While managing adenocarcinoma of the bladder, it is important to rule out any possibility of a metastatic lesion that could have come from other organs where mucinous adenocarcinoma is more common.

The unexpected turn of events the world over since the onset of the coronavirus pandemic may serve as stressors which can trigger or worsen mental disorders particularly psychosis. There is little or no reports about acute psychosis on the background of this current global pandemic particularly in the African context. The aim of this article describes a case series of acute schizophrenia-like disorder with COVID-19 related psychotic themes in the context of the current pandemic.
We present a case series of 2 patients with first episode psychotic illnesses during the COVID-19 pandemic. Both patients presented at the mental health outpatient clinic of the Obafemi Awolowo University Teaching Hospitals Complex, Ile-Ife, Nigeria.
Two patients with no previous history of mental illness presented with COVID-19 delusional themes and odd behaviours. Mental state examination revealed abnormalities in behaviour, thought system, perception and cognition. ICD10 diagnosis of Acute and Transient Psychotic disorder was made in both patients. There were no abnormal physical health findings and symptoms resolved with the use of antipsychotics.
Information on the COVID-19 pandemic may influence the content of psychotic experiences among the general population. A high index of suspicion and acute services are important to ensure prompt intervention and promotion of community mental health. Clinicians need to be well informed to be able to identify what deviates from the norm, and to be able to provide prompt intervention.

Real-time continuous glucose monitoring (CGM) has changed the way people with type 1 diabetes mellitus (T1DM) and health care providers perceived diabetes management and glucose control.
The purpose was to compare emotional well-being between adult people with type 1 diabetes who used CGM or conventional self-monitoring of blood glucose (SMBG).
The MEDLINE/PubMed, the Cochrane Library/Embase, CINAHL, Scopus, Web of Science, ProQuest databases were searched. Primary outcomes were health-related quality of life, glycemic control and fear of hypoglycemia. The inclusion criteria were adults, RCTs, CGM, SMBG, survey studies on quality of life, fear of hypoglycemia.
The meta-analysis was based on 11 studies with a total of 1228 T1DM. HFS worry domain analysis indicated reduction of hypoglycemia fear in CGM user compared with SMBG users Cohen's d equaled -0.24 (95% CI -0.41 to -0.07), mean difference -3.15 (95% Cl -5.48 to - 0.82). The outcome analysis for the DTSQ brought about Cohen's d at 0.23 (95% CI -0.18; 0.63). Osteoporosis is still under-diagnosed in Poland, among others due to limited accessibility to gold standard diagnostic technique, dual X-ray absorptiometry (DXA) of the proximal femur and lumbar spine. The aim of this study was to assess the agreement between a novel quantitative technique REMS, and DXA in bone mineral density evaluation and the diagnosis of osteoporosis.
All recruited patients (116) underwent DXA and REMS of the proximal femur and lumbar spine. The diagnostic agreement of REMS was assessed through a direct comparison with DXA results, with separate analysis for the proximal femur and lumbar spine scans. Additional sub-analysis of the impact of gender, age, and BMI was performed.
After the exclusion of patients due to significant skeletal impairments, missing results, and erroneous reports, 66 results of the femur and 58 reports of the lumbar spine were analyzed. The diagnostic agreement between DXA and REMS results was 82.8% in the lumbar spine group and 84.8% in the femur group. Strong correlations between REMS and DXA results were found, both in the lumbar spine and femur groups. The correlations between REMS and DXA scores remained strong regardless of gender, age, and BMI.
REMS showed a significant diagnostic agreement with the corresponding DXA measurements. The study confirms further the usefulness of REMS in the assessment of osteoporosis.
