This case serves to highlight the severity of eye trauma caused by less-lethal weapons; severe morbidity or even mortality can occur. Clinical description A civilian presented after a projectile weapon reportedly fired by Denver law enforcement struck his face during one of the recent protests in May 2020 in Denver. Upon ophthalmic examination, we observed no light perception (NLP) vision, periorbital ecchymosis, and devastating globe trauma. Primary diagnosis, interventions, and outcomes Further examination revealed a severely ruptured globe with extensive hemorrhage and extrusion of intraocular contents. Attempts to repair the ruptured globe failed because of the lack of remaining scleral tissue and expulsion of intraocular contents. Postoperatively, the visual acuity remained NLP and the eye was unable to hold pressure. Subsequently, the patient elected for enucleation. Conclusion Despite the purported safety advancements of less-lethal weapons, we continue to see high levels of morbidity and mortality. Permanent vision loss, loss of the eye, and death caused by these weapons have been reported. We hope that this information will serve as an example to help promote judicious use of these weapons by the proper authorities. Additionally, protesters and bystanders should be aware of these dangers and utilize high-quality eye protection.Introduction The coronavirus disease 2019 (COVID-19) pandemic has led to rapid adoption of teleophthalmology to deliver eyecare remotely. The purpose of our study was to assess the implementation and patient acceptability of video consultation for outpatient ophthalmic care at our institution. Methods We conducted a retrospective, cross-sectional analysis and patient survey of adult patients who completed a virtual video visit at our institution from 18 March 18 through to 27 April 2020. All video visit encounters were assessed for patient characteristics, diagnoses, management, and follow-up outcomes. Patients were surveyed for their feedback on acceptability and utility of their virtual video consultation. Results A total of 219 patients (mean age 55 years; range 21-89 years) completed 231 video visit encounters at our department over a 6-week period, of whom 118 were women (54%). About half of these encounters were acute visits (102 visits, 47%). However, the roles of circulating PCBs on stroke risk have not been elucidated. The purpose of this study was to examine whether serum PCBs could increase the risk for stroke among Chinese participants. A total of 241 patients with stroke and age- (5-year interval) and gender-matched controls were recruited in Tianjin, China. Concentrations of 17 PCB congeners were measured by using high-resolution gas chromatography with mass spectrometric detection. Conditional logistic regression models were applied to assess the odds ratio (OR) and 95% confidential interval (CI). Significant positive associations were observed between serum total PCBs and dioxin-like PCBs and the risk for stroke (all p trends less then 0.05), and the adjusted ORs for the highest tertile (T3) of serum concentration compared with the lowest tertile (T1) were 1.704 (1.073, 2.506) and 1.846 (1.156, 2.949). However, no significant association was found for non-dioxin-like PCBs (OR for T3 vs. T1 1.388, 95%CIs 0.850, 2.266; p trend 0.118). Stratified analysis by stroke subtypes (ischemic or hemorrhagic strokes) showed no significant heterogeneity (OR for T3 vs. T1 1.644 vs. 1.824; p for interaction 0.458). The results suggest detrimental roles of PCBs, mainly dioxin-like PCBs, in stroke risk, irrespective of stroke subtypes. Further well-designed prospective studies with larger sample size are invited to confirm these associations.Over 60% of rheumatoid arthritis (RA) patients achieve a good response after 12 months of treatment when following the European league against rheumatism (EULAR) guidelines for treatment. However, almost half of patients still suffer from moderate to severe disease activity despite this. In addition, mental health problems may remain despite reduced measures of inflammation systemically and within joints. Depression is two times more common in RA patients than in the general population, and intriguingly a bi-directional relationship with RA has been shown in cross-sectional studies. Chronic inflammation impairs the physiological responses to stress including effective coping behaviours, resulting in depression, which leads to a worse long-term outcome in RA. In RA patients, the pain score is not always solely related to inflammatory arthritis and immunological disease activity by Bąk et al. (Patient Prefer Adherence 13223-231, [1]). Non-inflammatory pain secondary to anxiety, depression, sleep disturbance and the psychosocial situation needs to be considered whilst fibromyalgia, mechanical pain and neuropathic pain can also contribute to overall pain scores by Chancay et al.
