Studies on frailty frequently only include older persons. The mapping of factors related to this syndrome and negative outcomes associated with it, also in middle age, may assist in health strategies to each age group.

To investigate social and health factors related to the frailty phenotype and to analyze the probabilistic relationships between frailty, falls and hospitalization in middle-aged and older persons.

This is a cross-sectional study using data for 4442 middle-aged (50-59 years) and older participants (60 years or older) from the Brazilian Longitudinal Study of Aging (ELSI-Brazil) 2015 and 2016. Bayesian network models were estimated with the score-based hill-climbing algorithm to identify factors associated with frailty, falls and hospitalization.

Mean age was 63.7 years, and prevalence of frailty was 8.5 % and 11.9 % among middle-aged and older participants, respectively. In the former, the probability of frailty increased when "poor" self-rated memory was considered in the model; and in the latter, the probability of frailty was greatest among individuals who did not participate socially and had the lowest level of education. In both age groups, frailty was an important factor that influenced the probability of negative health outcomes such as falls and hospitalization. However, this result depended on combinations of health factors in each sample.

Epirubicin is a first-line chemotherapeutic drug for the clinical treatment of diffuse large B cell lymphoma (DLBCL), but the overexpression of multidrug resistance (MDR) transporter proteins, especially P-glycoprotein (P-gp), renders epirubicin ineffective. Some studies reveal the potential role of melatonin in chemotherapeutic synergy and MDR.

The cell viability and apoptosis were determined by CCK-8 assay and acridine orange/ethidium bromide (AO/EB) fluorescence staining assay. Immunofluorescence and immunohistochemical staining were used to detect the expression of P-gp in DLBCL cells and tissues. Rhodamine-123 accumulation assay was used to evaluate the pump function of P-gp. The possible mechanisms of melatonin sensitize DLBCL cells to epirubicin were explored by western blotting, cytochrome C release, and pulldown assay.

Melatonin significantly enhanced the epirubicin-induced cell proliferation suppression, epirubicin-induced apoptosis, and reduced the IC50 value of epirubicin. Further, melatonin synergized with epirubicin to promote the activation of the mitochondria-mediated apoptosis pathway and increased the accumulation of epirubicin in DLBCL cells by inhibiting the expression and function of P-gp. Immunohistochemical staining studies revealed that P-gp expression was positively correlated with P65 expression. Epirubicin was subsequently discovered to upregulate the expression of P-gp by activating the NF-κB pathway in the DLBCL cells. Melatonin reduced the amount of P65 protein in the nucleus and abrogated the ability of P65 to bind to the ABCB1 promoter, decisively suppressing P-gp expression.

However, limited research has examined these factors in conjunction with one another or explored potential mechanisms underlying these associations. In a sample of 114 preschool-aged children and their parents, we examined how parents' beliefs about math and math anxiety together relate to children's math achievement and how parents' practices to support math might explain these associations. We used a range of measures of parental math input, including survey measures of the home numeracy environment as well as observations of number talk. Parents with stronger beliefs about the importance of math tended to have children with more advanced math skills, and parents with math anxiety tended to exacerbate the effects of these beliefs such that children whose math-anxious parents held strong beliefs about math's importance performed best. Furthermore, we found some evidence that parents' math practices may relate to this interaction or to children's math skills, but no single measure of math input mediated the effect of the interaction between parental math anxiety and parental math beliefs on children's math outcomes.
