Control of the coronavirus disease 2019 (COVID-19) heavily relies on universal access to testing to identify who is infected; tracking them to make sure they do not spread the disease further; and tracing those with whom they have been in contact. The recent surge in COVID-19 cases in Zimbabwe is an urgent national public health concern and requires coordinated efforts to scale up testing using capacity already in existence in country. There is need for substantial decentralization of testing, investment in better working conditions for frontline health workers and implementation of measures to curb corruption within government structures.
De-escalation of empirical antimicrobial therapy, a key component of antibiotic stewardship, is considered difficult in ICUs with high rates of antimicrobial resistance.
To assess the feasibility and the impact of antimicrobial de-escalation in ICUs with high rates of antimicrobial resistance.
Multicentre, prospective, observational study in septic patients with documented infections. Patients in whom de-escalation was applied were compared with patients without de-escalation by the use of a propensity score matching by SOFA score on the day of de-escalation initiation.
A total of 262 patients (mean age 62.2 ± 15.1 years) were included. Antibiotic-resistant pathogens comprised 62.9%, classified as MDR (12.5%), extensively drug-resistant (49%) and pandrug-resistant (1.2%). In 97 (37%) patients de-escalation was judged not feasible in view of the antibiotic susceptibility results. Of the remaining 165 patients, judged as patients with de-escalation possibility, de-escalation was applied in 60 (22.9%). When associated with decaying corpses, beetles can be of great value in estimating the postmortem interval. In order to consolidate a useful database for the forensic field, it is necessary to study the entomological fauna associated with the carcasses in different geographical regions since the diversity of insects varies according to the biogeoclimate zone. Thus, this study aimed to assess the influence of environmental and ecological factors on the composition and succession of beetles associated with pig carcasses exposed in southern Brazil. Collections were carried out during the hot/dry and cold/wet seasons. A total of 415 specimens belonging to 18 Coleoptera families were sampled. The highest total abundance (n = 329) and diversity (n = 44 taxa) were recorded in the cold/wet season, corresponding to approximately 80% of the total sampled from the two seasons. Dermestidae (26.7%) was the family most abundantly sampled. Regarding eating habits, in an increasing order of importance were necrophagous (43.3%), predator (31.6%), and omnivorous (0.05%). In the hot/dry season, there was no faunal succession. In the cold/wet season, the succession was more associated with differences in abundance than to the presence or absence of a specific taxon by decomposition stage. Considering all the factors analyzed in the current study, three species of beetles, Dermestes maculatus DeGeer (Dermestidae), Euspilotus azureus (Sahlberg, 1823) (Histeridae), and Oxelytrum discicolle Brullé, 1840 (Silphidae), could be identified as being of the greatest forensic relevance in this biogeoclimatic zone.
New approaches are needed to address the evolution of the primary aldosteronism syndrome and to increase its recognition. Herein, we review evidence indicating that primary aldosteronism is a prevalent syndrome that is mostly unrecognized, and present a pragmatic and pathophysiology-based approach to improve diagnosis and treatment.
Evidence was gathered from published guidelines and studies identified from PubMed by searching for primary aldosteronism, aldosterone, renin, and hypertension. This evidence was supplemented by the authors' personal knowledge, research experience, and clinical encounters in primary aldosteronism.
Renin-independent aldosterone production is a prevalent phenotype that is diagnosed as primary aldosteronism when severe in magnitude, but is largely unrecognized when milder in severity. Renin-independent aldosterone production can be detected in normotensive and hypertensive individuals, and the magnitude of this biochemical phenotype parallels the magnitude of blood pressure elee pathogenesis of hypertension and cardiovascular disease. Expanding screening indications and simplifying the diagnostic approach will enable implementation of targeted treatment for primary aldosteronism.
Girls with premature adrenarche (PA) may have a higher risk of developing polycystic ovary syndrome (PCOS) and metabolic syndrome. The biological purpose of adrenarche is unknown and the role of novel biosynthetic pathways remains unclear.
To compare the urinary steroid metabolome and enzyme activities of girls with PA to age-matched control girls and to published steroid values of girls with normal adrenarche and of women with PCOS and their newborn daughters.
Prospective observational study from 2009 to 2014.

Twenty-three girls with PA and 22 healthy, age-matched girls.
Steroid metabolites in 24-hour urine samples, including 4 progesterones, 5 corticosterones, aldosterone, 13 androgens, 2 estrogens, 14 glucocorticoids, and enzyme activities represented by metabolite ratios.
Girls with PA had a higher body mass index (mean standard deviation scores 0.9 vs -0.3, P = 0.013). Androgen excretion was higher in PA girls than in control girls (median 3257 nmol/24 hours vs 1627 nmol/24 hours, P < 0.001), in particular metabolites from alternate androgen pathways. The amount of progesterone, corticosterone, aldosterone, estrogen, and cortisol metabolites were similar between groups. Activities of 17β-hydroxysteroid-dehydrogenase and of 17,20-lyase were higher in girls with PA. Activities of 3β-hydroxysteroid-dehydrogenase, 21-hydroxylase, and 5α-reductase activity were not different between groups, in contrast to published results on girls with normal adrenarche or PCOS females.
