The convergent procedure (CP) may offer improved efficacy by combining endocardial and epicardial ablation. METHODS We reviewed 113 consecutive patients undergoing the CP at our institution. The cohort was divided into two groups based on the presence (n = 92) or absence (n = 21) of continuous rhythm monitoring (CM) following the CP. Outcomes were reported in two ways. First, using a conventional definition of any atrial fibrillation/atrial tachycardia (AF/AT) recurrence lasting >30 seconds, after a 90 day blanking period. Second, by determining AF/AT burden at relevant time points in the group with CM. RESULTS Across the entire cohort, 88% had either persistent or long-standing persistent AF, mean duration of AF diagnosis before the CP was 5.1 ± 4.6 years, 45% had undergone at least one prior AF ablation, 31% had impaired left ventricle ejection fraction and 62% met criteria for moderate or severe left atrial efrom transdiaphragmatic to sub-xiphoid surgical access. Future trials will be necessary to determine which patients are most likely to benefit from the convergent approach. © 2020 Wiley Periodicals, Inc.AIM With the intent of providing insights for healthcare managers, this study explored pediatric intensive care unit nurses' lived experience of professional identity in the context of organizational change. BACKGROUND While professional identity improves retention of nurses and the provision of quality care, outcomes of importance for managers, organizational change perturbs this identity. METHOD The study used a hermeneutic-phenomenological design. Data was collected via individual interviews, photographs, participant observation, and document review. A purposive sampling strategy was used to recruit pediatric intensive care unit nurses (n=15) in a large Canadian pediatric hospital. RESULTS Nurses' critical care identity eroded in this organisation due to the interplay between hospital re-design and new eligibility criteria for patient admissions. CONCLUSION Interactions between multiple projects and the unit context, as well as nursing professional identity need to be considered early on during project planning. This study fills an important gap in research concerning the management challenges brought about by the intersection of multiple changes. IMPLICATIONS FOR NURSING MANAGEMENT The results from this study bring to light three important lessons for nurse managers 1) the specific unit context should be evaluated before a project is initiated, 2) the physical environment needs to be considered when determining staffing requirements, and 3) identity transitions need to be managed. Activation of the lectin and alternative pathways (LP and AP, respectively) contribute to the deterioration of conditions in certain diseases such as ischemia-reperfusion injuries and age-related macular degeneration (AMD). In the current study, we generated dual complement inhibitors of the pathways MAp44-FH and sMAP-FH by fusing full-length MAp44 or small mannose-binding lectin-associated protein (sMAP), LP regulators, with the N-terminal five short consensus repeat (SCR) domains of complement factor H (SCR1/5-FH), an AP regulator. The murine forms of both fusion proteins formed a complex with endogenous mannose-binding lectin (MBL) or ficolin A in the circulation when administered in mice intraperitoneally. Multiple complement activation assays revealed that sMAP-FH had significantly higher inhibitory effects on activation of the LP and AP in vivo as well as in vitro compared to MAp44-FH. Our results indicate that the novel fusion protein sMAP-FH inhibits both the LP and AP activation in mice and in human sera, and could be an effective therapeutic agent for diseases in which both the LP and AP activation are significantly involved. © 2020 Federation of American Societies for Experimental Biology.The present study aimed to evaluate the effect of a single intramuscular injection of melatonin on response of estrus, rate of conception, ovarian and uterine blood supply during the non-breeding season. Melatonin powder was dissolved in dimethyl sulfoxide (DMSO) and olive oil and injected in two dose of 25 mg (MEL-1; n = 10) and 50 mg (MEL-2; n = 10) in May. Ten ewes were served as the control (CON). Results showed that oestrus response was higher in the MEL- 2 group than in other groups (p 0.05). In conclusion, the definite changes in the studied parameters showed two novel findings; (1) the increase in melatonin dose results in increasing the blood flow expressed by increased Doppler velocities in fatty tailed ewes, (2)as well as, melatonin can initiate the breeding season regardless of the season earlier by 1-2 months in ewes. This cluster was characterized in solution by multinuclear NMR spectroscopies, as well as in the solid-state by single crystal X-ray diffraction analysis. This species is stereochemically nonrigid in solution and possesses a folded cyclo -Sn 4 core that undergoes rapid ring-flipping at higher temperatures. The solid-state molecular structure is remarkably unsymmetrical and possesses a nearly planar cyclo -Sn 4 core. The DMP substituents are arranged such that three are approximately coplanar, while one is nearly perpendicular to the cyclo -Sn 4 core. Density functional theory calculations for a [PhSn] 4 model system show that this distorted geometry about the cyclo -Sn 4 core maximizes σ-bonding between the Sn centers in a manner reminiscent of trans -bent bonding in the heavy group 14 analogues of alkenes and alkynes. © 2020 WILEY-VCH Verlag GmbH & Co.
