The Proteus mirabilis PmirS clinical isolate, which was susceptible to imipenem (0.5 μg/mL) and amikacin (1 μg/mL), was recovered from a bronchial aspirate of a patient who recently underwent lung transplantation. The P. mirabilis PmirR clinical isolate, which exhibited resistance to imipenem (16 μg/mL) and amikacin (24 μg/mL), was isolated 3 weeks later from the same patient and the same specimen type. Using short-read sequencing technology, these isolates appeared to be genetically identical except the cpxA gene of the PmirR isolate that was mutated leading to the His-208-Pro substitution. The structural alteration was localized in the histidine kinase, adenylate cyclase, methyl accepting protein, phosphatase (HAMP) domain, which is involved in the signal transduction between the sensor kinase and the regulator response of the CpxA/CpxR two-component system (TCS). No significant defect in the growth rate was found between the PmirS and PmirR isolates. This study suggests that alteration in CpxA might confer imipenem and amikacin resistance in P. mirabilis. This study brings new evidence that the TCS alteration could provide an adaptive capacity in a clinical context by conferring antibiotic resistance without fitness cost.Background It is difficult for novice surgeons to manipulate the oblique laparoscope in single-incision laparoscopic percutaneous extraperitoneal closure (SILPEC) for inguinal hernia because of collisions between the instruments. To standardize manipulation of the laparoscope, we studied the viewing direction of the oblique laparoscope, and assessed the optimal manipulation of the laparoscope to avoid collisions. Methods A retrospective chart review was performed on patients who underwent SILPEC between April 2016 and April 2017. The viewing direction of the 30° oblique laparoscope was measured according to the location of the field stop pointer. Patients were divided into three groups according to the viewing direction at the beginning of the procedure the inside viewing direction was from -90° to -11°, upward viewing direction was from -10° to 10°, and outside viewing direction was from 11° to 90°. The length of the procedure, viewing direction at the end, and the percentage of cases in which there was a change in viewing direction during the procedure were compared. Results Ninety-eight cases of SILPEC were performed during the study period. The percentage of patients with a change in category of viewing direction in the inside, upward, and outside groups was 35%, 21%, and 11%, respectively, showing a significant difference among the three groups. Conclusions Setting the initial viewing direction to the outside can reduce correction of the viewing direction during SILPEC. Because the intersection angle between the outside-viewing laparoscope and forceps is close to a right angle, this might reduce collisions.Edible oils and oil-containing foods have been regarded as the main source of 3-monochloropropane-1, 2-diol (3-MCPD) esters. A total of 3,847 individual food samples were collected in China to carry out 3-MCPD fatty acid esters analysis. The samples comprising edible oils, fried foods and bakery foods from 31 Chinese provinces, municipalities and autonomous regions were obtained from the national food contaminant information system during 2015-2017. The dietary exposure and potential risks associated with the consumption of 3-MPCD esters from edible oils and oil-containing foods were estimated using a semi-probabilistic assessment model. The findings indicate that the potential health risks caused by dietary 3-MCPD esters from edible oils and oil-containing foods were of low concern for most of the Chinese population. However, the exposure risk of the consumers with excessive edible oil consumption calls for attention.Background Remnant gastric cancer (RGC) is increasing due to past use of subtotal gastrectomy to treat benign diseases, improvements in the detection of gastric cancer, and increased survival rates after gastrectomy for gastric cancer. Laparoscopic access provides the advantages and benefits of minimally invasive surgery. However, laparoscopic completion total gastrectomy (LCTG) for RGC is technically demanding, even for experienced surgeons. Because of its rarity and heterogeneity, no standard surgical strategy has been established and few surgeons will develop technical expertise to carry out this procedure. Aim To describe our standard technique, giving surgeons a head start in LCTG and report the early experience with this stepwise approach.
