The construct of mixed depression (MxD), namely the simultaneous presence of excitatory symptoms during a depressive episode, has gained increased interest in the recent years. The Koukopoulos Mixed Depression Rating Scale (KMDRS) has been recently validated for the diagnosis and assessment of MxD. The aim of this study was to validate the Italian version of the KMDRS.
Patients with a major depressive episode (MDE) (N=396, age, mean=44.73 years, SD=15.93) were recruited at Lucio Bini Center, Rome, Italy. All participants were administered the Italian version of the KMDRS, the Montgomery-Åsberg Depression Rating Scale (MADRS), the Young Mania Rating Scale (YMRS), the Clinical Global Impressions (CGI), and the Global Assessment of Functioning (GAF). We calculated predictive validity, and performed Receiver Operator Curve (ROC) analysis, Cronbach's alpha, and inter-rater reliability.
Predictive validity (C-statistic= 0.80; IC 95% 0.75, 0.85) and capacity [OR=1.25±.03 (SE); 95% CI 1.18, 1.32; p<.001] were good, matching those of the original version, whereas internal consistency (Cronbach's alpha = .70, IC 95% 0.65, 0.75) and interrater reliability (Cohen's kappa= 0.74) were adequate.

However, although lithium is universally accepted as a reference standard in the treatment of BD, its diffusion in clinical practice is still limited. In order to test awareness of the indications for the use of lithium, its methods of clinical use and the monitoring of the main side effects, a survey was carried out among young Italian psychiatrists (less than 40 years old). This survey was followed by an expert meeting in which the results of the survey were analyzed and discussed in order to identify the main critical issues regarding drug management and possible solutions. This work highlights how young Italian psychiatrists have presented a good awareness of the use of lithium in clinical practice. In particular, knowledge of the guidelines regarding safety and attention to strategies that can improve long-term adherence is adequate.This narrative review addresses the interconnections among stress, mental disorders and migraine with a specific focus on non-pharmacological interventions that may be effective in improving both migraine and the psychiatric comorbidity. Migraine is often comorbid with depression, anxiety, personality disorders, and sleep disorders. Subjective stress and stressors are common triggers for migraine attacks and are risk factors for chronification, whilst mental disorders and stress responses are closely linked in a bidirectional relation. Recent studies show that psychiatric comorbidity is associated with migraine severity, worse outcomes, increased disability and reduce quality of life. Numerous studies on non-pharmacological interventions for migraine were published and behavioural treatments included biofeedback, cognitive-behavioural therapy, relaxation training, stress management and brief psychodynamic psychotherapy. Taken together, psychological interventions proved to be effective in migraine treatment and a combination of pharmacological and psychological treatment appear to be more effective than either medication or psychotherapy alone. Non-pharmacological interventions effectiveness should be due to the improvement of migraine, stress-related vulnerability and mental disorders together and the combined treatment could prevent the chronification circuit of migraine. Well-designed long-term studies are needed to clarify comparative effectiveness of non-pharmacological techniques in the treatment and the prevention of migraine.
This paper belongs to a series of three manuscripts examining the clinically relevant evidence about the use of trazodone in major depressive disorder (MDD). One of the papers provides general clinical guidance for the use of trazodone MDD. Another paper evaluates the clinically relevant evidence pertaining to the use of trazodone in MDD with insomnia, and the present manuscript evaluates the properties of prolonged release trazodone (T-RP) and trazodone once-a-day (T-OAD), to identify the characteristics of patients that may benefit from the use of one of the formulations.
PubMed and Cochrane Library were searched for English language papers by using combinations of the following key words trazodone, once-a day, prolonged release, extended release, slow release. United States, European and Italian Medicine Agency prescribing information for trazodone were consulted as well.
Trazodone is a multifunctional drug, characterized by different affinities for specific receptors and transporters. Clinical efficacy on specific symptoms is influenced by pharmacokinetic aspects and variables such as the dose and the formulation that is prescribed and taken.
