Regular narrow QRS tachycardia was induced with critical AH delay on programmed atrial stimulation. An atrial overdrive pacing (AOD) was performed during the tachycardia (Fig 1 & 2). Cushing's disease is a rare endocrine disorder characterised by cortisol overproduction with severe complications. Therapies for cortisol reduction are often necessary. Here we report the outcomes from the pivotal phase III study of osilodrostat (a potent oral inhibitor of cytochrome P450 11B1, mitochondrial [11β-hydroxylase]; Novartis Pharma AG, Basel, Switzerland) in patients with Cushing's disease.
LINC 3 was a prospective, multicentre, open-label, phase III study with a double-blind randomised withdrawal period, that comprised four periods. Patients aged 18-75 years, with confirmed persistent or recurrent Cushing's disease (defined as mean 24-h urinary free cortisol [UFC] concentration >1·5 times the upper limit of normal [ULN] and morning plasma adrenocorticotropic hormone above the lower limit of normal) who had previously had pituitary surgery or irradiation, or were newly diagnosed and who refused surgery or were not surgical candidates, were recruited from 66 hospital sites and private clinicady drug, after the core study phase.
Twice-daily osilodrostat rapidly reduced mean 24-h UFC and sustained this reduction alongside improvements in clinical signs of hypercortisolism; it was also generally well tolerated. Osilodrostat is an effective new treatment option that is approved in Europe for the treatment of endogenous Cushing's syndrome and in the USA for Cushing's disease.

Exercise-based fall prevention programs mainly refer to multimodal and challenging balance exercises. Individual load monitoring and interpretations are crucial to enable adequate adaptation responses on the individual level. Thus, assessing internal responses to external stimuli throughout an intervention period need to be adequately addressed. The aim of this secondary analysis of a 3-armed randomized controlled trial was to analyze internal and external loads of unstable vs. stable balance Exergame training in healthy seniors. We intended to elucidate whether differences of external and internal load criteria occur over the intervention period.
A total of 51 healthy seniors (females n=34; males n=17; age 69±6years; BMI 27±5) were allocated to either volitional stepping (VOL), volitional stepping under unstable conditions (VOL+US) or an inactive control group (CON). VOL and VOL+US completed 8weeks of Exergame based step training (three weekly sessions, 45min each) using the Dividat Senso device. Our results indicate that between-group differences in perceived efforts a) can funnel over time, b) depend on game content and c) do not necessarily affect overall scoring. Recently, an amendment to the Narcotics Law regarding the "not inconsiderable quantities" was introduced. The new limits for methamphetamine are derived from the varying potency of the respective enantiomers. Switzerland, however, does not practice this distinction and there is only one limit quantity, without considering the isomeric structure. To examine whether this single value is still contemporary, 26 Thai pill samples from the years 2000, 2001, 2007, 2009 and 2017 were analyzed by HPLC-MS/MS and GC-MS. Both methods resulted in similar stereoisomeric distributions the pills mainly consist of the more potent S-(+)-methamphetamine, some even being enantiopure. Others show enantiomeric mixtures of R-(-)-/S-(+)-methamphetamine, but rarely in an equimolar ratio. There even was one sample, where mainly the less potent R-(-)-methamphetamine was detected. In conclusion, the analyses revealed that the single value for a "not inconsiderable quantity" in Switzerland seems outdated. Most of the sized pills showed a much higher concentration of the more potent S-(+)-methamphetamine. The risks related to taking such a pill are much higher and therefore the limit quantity should be adapted to the potency of the respective enantiomers.
The simplified criteria for the diagnosis of autoimmune hepatitis (AIH) include immunofluorescence testing (IFT) of antinuclear and smooth muscle autoantibodies (ANA and SMA) on rodent tissue sections. We aimed to establish scoring criteria for the implementation of ANA IFT on human epithelioma-2 (HEp-2) cells and ELISA-based testing.
ANA and SMA reactivity of 61 AIH sera and 72 non-alcoholic fatty liver disease controls were separately assessed on tissue sections and HEp-2 cells to compare the diagnostic value at increasing titers. A total of 113 patients with AIH at diagnosis and 202 controls from 3 European centers were assessed by IFT as well as 3 different commercially available ANA ELISA and 1 anti-F-actin ELISA.
ANA assessment by IFT on liver sections had 83.6% sensitivity and 69.4% specificity for AIH at a titer of 140. On HEp-2 cells, sensitivity and specificity were 75.4% and 73.6%, respectively, at an adjusted titer of 1160. Area under the curve (AUC) values of ANA ELISA ranged from 0.70-0.87, with ELISA coated with HEp-2 extracts in addition to selected antigens performing significantly better.
