INTRODUCTION Drug-resistant seizures due to hemimegalencephaly in neonates and infants are a unique surgical and anesthesia challenge. While early surgery in these patients may predict a better seizure control, a lower body weight, limited blood volume, and surgical blood loss may make hemispherectomy prohibitive. METHODS Two infants (weight, 8.7 kg and 3.7 kg) underwent interhemispheric vertical hemispherotomy with endoscope assistance. In the first case, during the lateral disconnection, excessive bleeding prompted the surgeon to coagulate the lenticulostriate arteries at the origin from the middle cerebral artery to reduce bleeding. In the second infant, the lenticulostriate arteries were coagulated before initiating the lateral disconnection. RESULTS In both infants, the blood loss from lateral dissection was reduced by coagulation of lenticulostriate arteries. CONCLUSION The authors suggest that early coagulation of the lenticulostriate arteries is a useful strategy to minimize blood loss in low-weight infants undergoing hemispherotomy.INTRODUCTION Awake brain surgery (ABS) represents a rare surgical procedure in children as age and psychological aspects, which are considered to interfere with its feasibility and psychological outcome and limit its application. Only few pediatric case series have been reported so far, indicating a more complex translation of this surgical approach to children. However, the advances in neuropsychological testing and monitoring may have a substantial impact on ameliorating the eligibility of children undergoing awake procedures. This study addresses the condition of ABS in a pediatric cohort, focusing on its practicability and diversified outcome aspects. METHODS We performed a retrospective review and prospective outcome analysis of pediatric patients with CNS lesions undergoing ABS between 2005 and 2018, completed at the University of Lyon, France. RESULTS Eighteen children were considered for ABS with respect to the eloquent location of their CNS lesions documented in their pre-operative MRI. Experiments with different implants both organic and non-organic had led to introduction of sometimes extremely peculiar materials coming from different species like ox bone or elephant's ivory. The aim of this article is to present not widely known concept of ivory use in bone surgery that set its foot in the history of orthopaedics and laid foundations for orthobiologic reconstructions. METHODS Retrospective analysis of articles and books published between 1846 and 2017 that describe various examples of ivory application in the treatment of fresh fractures, non-unions and reconstruction of joints. RESULTS Our research shows that ivory to the surgical world was introduced by Friedrich Dieffenbach, founder of the modern plastic surgery. It was also used with different rate of success by many of the famous surgeons of the nineteenth and twentieth century to include Trendelenburg, Billroth, Volkmann, Paget and Hey Groves. Ivory was immensely popular in bone surgery and became material of choice demonstrating amazing biological properties and very low rate of infections. CONCLUSION Ivory has served well in successful treatment of various orthopaedic conditions for over 100 years. In this article, we are using history as a stepping stone to examine material that is not rejected by the body and promotes bony healing without increased infection or other complications. It is worth considering further analysis of historically acquired specimens for further development of materials for further orthopaedic fracture and reconstructive techniques.PURPOSE The classic Putti-Platt technique was modified by suturing the lateral subscapularis tendon to the capsule instead of the anterior glenoid edge. We report the clinical results of a modified Putti-Platt procedure in patients with traumatic anterior shoulder instability. METHODS The study reports results in 434 patients (450 shoulders) with traumatic anterior shoulder instability. The patients comprised 322 men and 122 women with a mean age of 22 years. Clinical results were evaluated by the Rowe score, Japan Shoulder Society Shoulder Instability Score (JSS-SIS); recurrence of instability, instability severity index score (ISIS), restriction of external rotation, return to sports activities, and osteoarthritis on plain radiographs and MRI were examined. RESULTS The mean Rowe score improved from 26 to 90 points, and the mean JSS-SIS improved from 51 to 88 points. Altogether, 419 patients (97%) had no recurrence at a minimum of two years of follow-up, even though the study included 87 patients with an ISIS of ≥ 7 points.
