There is increasing interest in the relation between screen use and sleep problems in early childhood. In a sample of 30-month-old children, this study used observational measures of screen use during the hour or so leading up to bedtime, parent reports of screen use during the child's bedtime routine, and actigraphic measures of toddler sleep to complement parent-reported sleep problems. Whether screen use was observed during the pre-bedtime period or was reported by the parents as part of the nightly bedtime routine, greater screen use in either context was associated with more parent-reported sleep problems. Additionally, more frequent parent-reported screen use during the bedtime routine was also associated with actigraphic measures of later sleep, shorter sleep, and more night-to-night variability in duration and timing of sleep. These associations suggest the negative consequences of screen use for children's sleep extend both to aspects of sleep reported by parents (e.g., bedtime resistance, signaled awakenings) and to aspects measured by actigraphy (e.g., shorter and more variable sleep).
Multidetector-row computed tomography is often used as a first-line test in the diagnostic evaluation of cardiovascular diseases including aortic dissection, coronary artery disease and pulmonary embolism. This study evaluated the impact of reducing the tube potential from 120 kVp to 100 kVp in a selected group of patients presenting to the Emergency Room with acute chest pain. The primary end point was how the reduction of radiation dose affected image quality.

The current study was performed over a period of 2 years between July, 2016 and July, 2018. This study included patients who presented to the Emergency Room or to an outpatient clinic and were suspected to have a coronary, a pulmonary (pulmonary embolism), or an aortic (aortic dissection) etiology. Suspicion was determined by the medical provider based on clinical picture, EKG, and lab results when available. All patients were referred for computed tomography angiography (CTA) testing as part of their diagnostic evaluation. A total of 84 patients were involved in the study. Seventy of the patients underwent the low acquisition Kvp technique (100 Kvp - Group I). In the remaining 14 patients, the standard acquisition technique (120-140 Kvp - Group II) was utilized.

This study showed the feasibility of using low energy CTA to significantly reduce the patient's radiation exposure without markedly affecting the image quality and diagnostic accuracy.

The use of low energy CTA protocols in cases of acute chest pain revealed no major difference regarding the image quality with marked reduction of the radiation dose received by the patient.With an aging population and significant overlap of risk factors, the cohort of patients with acute coronary syndrome (ACS) and concomitant atrial fibrillation (AF) is a sizable and growing one, with implications on cardiac reserve, anticoagulation and antiplatelet therapies, and related complications. The present study uses a large national database to analyze the impact of AF on patients admitted with an ACS. We queried the 2012 to 2014 National Readmissions Database to identify patients admitted with an ACS using International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9 CM) codes. These patients were then subcategorized based on the presence or absence of AF. Analysis of their initial hospitalization, 30-day readmissions and healthcare utilization and the economic burden was performed. Among 1,558,205 patients with ACS, 270,966 (17.4%) were noted to have concomitant AF. At baseline, these patients were older and more likely female, with a significantly higher burden of comorbidities. Patients with AF had longer and more complicated index hospitalizations with significantly higher mortality rates (8.6% vs 4.6%). Coronary artery bypass graft was the preferred method of revascularization in patients with AF as compared to percutaneous coronary intervention. The 30-day readmissions were higher in the AF group (15.6 vs 10.8%), largely driven by noncardiac causes. This study reviewed the literature on the incidence and mechanism of drug-induced myopathy in patients with type 2 diabetes mellitus who are taking SGLT2 inhibitors with and without the use of statins.The ability to mentally rotate objects in space is a fundamental cognitive capacity. Previous studies showed that the time to rotate the image of a figure to match another increases progressively with angular disparity. It remains unclear whether this increase in response time with angular disparity could reflect increased processing operations or more cognitive effort instead of a sustained use of a 'rotate' mechanism without a change in workload. We collected response times as well as pupillary responses that index cognitive workload and activity in the brainstem's locus coeruleus, from a sample of 38 young adults performing a chronometric mental rotations task. The results showed the expected increase in response times but no increase in pupil diameters between 60, 120, and 180 degrees of rotation, suggesting no significant changes in arousal levels when rotating figures near and far. This indicates that during mental rotation the load on cognitive resources remains constant irrespective of angular distance.
