Endoscopic Retrograde Cholangio Pancreaticography (ERCP) is an invasive procedure which can be used for therapeutic purpose. But it has versatile complications. It depends upon patient factors and gastroenterologist's expertise. This study was done to determine factors affecting morbidity and mortality after ERCP for obstructive jaundice. The prospective observational study was conducted among 30 patients with obstructive jaundice in the department of Gastroenterology and Surgery of Combined Military Hospital (CMH), Dhaka during the period of July 2016 to December 2016. After obtaining a detailed history, general physical and systemic examinations were done for all enrolled patients and they were subjected to do relevant investigations. The data were collected by the active participation of the patient and patient was interviewed by the preformed case record form. ERCPs were carried out in a standard fashion by using a side viewing duodenoscope. Every variable like number of pancreatic duct cannulation, sphinof acute pancreatitis are the commonest significant post ERCP morbidity and mortality predictors.Both of heart failure and chronic obstructive pulmonary diseases are in the elderly. They have an important impact on quality of life and functional status, show high morbidity and mortality rates and lead to extensive health-care costs. Although both diseases have been studied extensively, information about the prevalence of heart failure in stable chronic obstructive pulmonary disease (COPD) patients is lacking. For people who have both the diseases, identifying the cause of breathing symptoms can be challenging. It seems therefore plausible that a considerable proportion of patients with a diagnosis of COPD have concomitant heart failure, which remains unrecognized by primary care physicians or pulmonologists. The main aim of this study was to assess the prevalence of heart failure in patients with a diagnosis of chronic obstructive pulmonary disease. The prevalence of previously unknown heart failure was assessed in 100 patients ≥40 years with a GP (General physician) diagnosis of COPD, in a stable phase of their disease. This descriptive cross sectional study was carried out in the Department of Medicine, Shaheed Ziaur Rahman Medical College hospital, Bogra, Bangladesh from November 2011 to April 2012. A total of 100 cases of chronic breathlessness coming into out patient department and admitted into Department of Medicine of Shaheed Ziaur Rahman Medical College Hospital, Bogra were included based on inclusion & exclusion criteria. After taking informed written consent data was collected by a questionnaire and relevant investigations were done. Then collected data was analyzed and were expressed as mean ±SE and statistical significance of difference among the group was calculated by unpaired students 't' test. In this study we founded among 100 participating patients with a diagnosis of COPD by their physician, in 24 (24%) patients had previously unrecognized heart failure. Therefore, by this study we recommend that evaluation and assessment of cardiac status is very important in elderly patients with COPD.Smoking has extensive effects on respiratory function. The rate of cigarette smoking among young people has continued to increase steadily. Exposure to cigarette smoking is usually measured in terms of 'Pack-Year'. The aim of the study was to observe the effect of pack years on pulmonary functions in apparently healthy asymptomatic smokers by measuring FVC, FEV₁, FEV₁/FVC ratio and PEFR value by digital spirometer. This cross-sectional observational study was conducted in the department of Physiology, Sylhet MAG Osmani Medical College in collaboration with department of Medicine, Sylhet MAG Osmani Medical College Hospital, Sylhet, Bangladesh from July 2016 to June 2017. A total number of 100 male smokers, age ranging from 18-60 years and BMI within normal limit (18.5-24.9kg/m²) were included in this study. Their pulmonary functions were studied by measuring FVC, FEV₁, FEV₁/FVC ratio and PEFR. Relationship between pack-year and pulmonary function test parameters was established by one way ANOVA test. In this study we found that lung function parameters shared an inverse relationship with pack year of smoking. The percentage of predicted values of FEV₁, FEV₁/FVC ratio and PEFR were significantly lower (p less then 0.001) in heavy smokers who smoked more than 30 pack years. Therefore, by this study we inferred that those who smoked more than 10 pack years are associated with accelerated decline in lung function.COVID-19 is a declared pandemic by World Health Organization. The diagnostic tests are not of high specificity and sensitivity, so far. Pro-inflammatory biomarkers and High Resolution Computed Tomography (HRCT) of chest are the common investigations performed to evaluate the diagnosis and prognosis of COVID-19. The objective of this study was to estimate the sensitivity, specificity and accuracy of HRCT and to find out the relationship between the biomarkers and HRCT findings. This was a cross-sectional descriptive study carried out in the COVID-19 suspected isolation unit of a tertiary hospital from July 2020 to November of 2020. Data were collected from electronic medical record (EMR). Relationship test were done by t-test and one-way-ANOVA test. Total 123 cases were enrolled after matching with selection criterion. EPZ011989 Mean age of male was 62.5 years and female 57.7 years. Highest frequency of participants was observed in the 60-69 year age group. According to HRCT% findings 4 groups were made. They are below 25%, 25%-50%, 51%-75% and 76%-100%. The distribution of case among these groups was 15%, 44%, 35% and 6% respectively. The relationship between biomarkers (NLR, D-dimer, Ferritin and CRP) and HRCT% was found significant (p less then 0.05). In HRCT 92.5% lesion were peripherally placed and 99.2% cases were affected by both lungs. The sensitivity and specificity of HRCT were found 46% and 72.6% respectively with 62% accuracy. Raised biomarkers are significantly related to the more lung involvement in case of COVID-19 suspected pneumonia patients. These biomarkers will be helpful as diagnostic and prognostic markers for this disease. HRCT percentage can play an important role as diagnostic and prognostic tool in case of COVID-19 suspected cases.
