To explore treatment strategies for patients with positive margins after cervical cold knife conization (CKC) by estimating the risk of residual or recurrent CIN2 or worse (CIN2+).

A retrospective study included 569 patients receiving CKC for CIN3 in Xiangya Hospital from January 2013 to December 2017. Demographic characteristics and test results were obtained before CKC, after CKC, at 6, 12, and 24months, then annually thereafter. The primary end point was residual/recurrent CIN2+ post-CKC.

Fourteen (2.46%) patients had residual/recurrent CIN2+ with a median time of occurrence at 12months post-CKC. Taking the average age and hrHPV viral load tested by Hybrid Capture 2 (HC2) as thresholds, the risk of residual/recurrent CIN2+ was higher in women aged over 40years or with a baseline HC2 of 300 or more for the ratio of relative light units to positive cut-off values. Patients with positive margins were at higher risk of residual/recurrent CIN2+ (hazard ratio 3.66, 95% confidence interval 1.25-10.71), especially when endocervix was involved. A total of 536 (94.20%) patients received HPV testing within 6months after CKC. Patients with both positive HPV testing results and positive margins were at the highest risk of residual/recurrent CIN2+.

From January 2008 to December 2009, 3333 specimens of vermiculate electric-ray were caught. Total length (LT ), sex, maturity stage, catch location and depth were recorded for each specimen. The LT of vermiculate electric-ray ranged from 6.7-24.6 cm. The authors estimate an irregular spatial structure, with a high-density patch ( x ¯ = 53 ind. ha-1 ) located on the east coast, which concentrates 65.2% of the specimens. The high-density patch consists mainly of large juveniles (13.3-19.5 cm LT ), sub-adults (14.0-19.8 cm LT ) and young adults (14.7-21.3 cm LT ). Data indicate that adults migrate to the high-density patch to reproduce. Males reached maturity at 14.5 cm LT , whereas females reached maturity at 19.3 cm LT . Vitellogenesis in female vermiculate electric-ray begins in June; ovulation, mating, fertilization and gestation in October and birth begins in February. This indicates an annual cycle with vitellogenesis and consecutive gestation, in females synchronized in reproduction. Fecundity was 1-8 ( x ¯ = 4), and the sex ratio of embryos was 11. The birth occurred between February and April, with an average size at parturition of 6.3 cm LT . Incidental capture of sub-adults and adults of N. vermiculatus by bottom trawls threatens the survival of this species.
Krebs von den Lungen-6 (KL-6) is a high-molecular-weight (200kDa) glycoprotein proposed as a diagnostic biomarker for differentiating interstitial lung disease (ILD). Systemic sclerosis (SSc) is a rare immune-mediated disorder, and ILD is the leading cause of morbidity and mortality. Pleuroparenchymal fibroelastosis (PPFE) has been described to have a poor prognosis in SSc-ILD patients. This study undertook to compare serial changes in KL-6 in SSc-ILD patients with and without PPFE, to verify its prognostic value as a disease biomarker.

Twenty-five SSc-ILD patients (median IQR, 62 (56-58); 20% males) were retrospectively enrolled. 12 SSc-ILD patients (48%) had also a radiological diagnosis of PPFE. Serum KL-6 concentrations were measured by KL-6 reagent assay (Fujirebio Europe, Ghent, Belgium).

Serum KL-6 measurements were increased in SSc-ILD patients with and without PPFE compared with healthy controls (P<.0001). Comparative analysis of the rate of variation of KL-6 over the 6years of follow-up was performed by serial two-yearly KL-6 measurements Δ1(t1-t0), Δ2(t2-t1) and Δ3(t3-t2). In SSc-ILD patients with PPFE pattern, Δ3 was significantly different than those without PPFE pattern (P=.0020). Serum KL-6 levels were significantly different (P=.0455) either at Δ2 and Δ3 in the PPFE group. In SSc-ILD patients with PPFE, at t3 serum KL-6 concentrations were inversely correlated with FEV1 (r=-.76; P=.037) and FVC percentages (r=-.79; P=.028).

These results suggest that serial measurements of KL-6 in the follow-up of these patients may help to monitor disease progression. In real life, in SSc-ILD patients PPFE should be always evaluated at CT and when present should suggest a tight follow-up to monitor its evolution.
