The aim of this study was to determine hospital variation in quality of hip fracture care using a composite process indicator (textbook process) and to evaluate at patient level whether fulfilment of the textbook process indicator was associated with better outcomes during hospital stay.
Hip fracture patients aged 70 and older operated in five hospitals between 1 January 2018 and 31 December 2018 were included. Textbook process for hip fracture care was defined as follows (1) assessment of malnutrition (2) surgery within 24 h, (3) orthogeriatric management during admission and (4) operation by an orthopaedic trauma certified surgeon. Hospital variation analysis was done by computing an observed/expected ratio(OR 1.01, 95% CI 0.78-1.30).

At patient level, this quality indicator is associated with fewer complications during hospital stay.The meniscus is an integral part of the human knee, preventing joint degradation by distributing load from the femoral condyles to the tibial plateau. Recent qualitative studies suggested that the meniscus is constituted by an intricate net of collagen channels inside which the fluid flows during loading. The aim of this study is to describe in detail the structure in which this fluid flows by quantifying the orientation and morphology of the collagen channels of the meniscal tissue. A 7 mm cylindrical sample, extracted vertically from the central part of a lateral porcine meniscus was freeze-dried and scanned using the highest-to-date resolution Microscopic Computed Tomography. The orientation of the collagen channels, their size and distribution was calculated. Comparisons with confocal multi-photon microscopy imaging performed on portions of fresh tissue have shown that the freeze-dried procedure adopted here ensures that the native architecture of the tissue is maintained. Sections of the probe at different heights were examined to determine differences in composition and structure along the sample from the superficial to the internal layers. Results reveal a different arrangement of the collagen channels in the superficial layers with respect to the internal layers with the internal layers showing a more ordered structure of the channels oriented at 30[Formula see text] with respect to the vertical, a porosity of 66.28% and the mean size of the channels of 22.14 [Formula see text].
A point-of-care ultrasound education program in obstetrics was developed to train antenatal healthcare practitioners in rural Zanzibar. The study group consisted of 13 practitioners with different training backgrounds physicians, clinical officers, and nurse/midwives. Trainees received an intensive 2-week antenatal ultrasound course consisting of lectures and hands-on practice followed by 6months of direct supervision of hands-on scanning and bedside education in their clinical practice environments. Trainees were given a pre-course written exam, a final exam at course completion, and practical exams at 19 and 27weeks.
