Testicular torsion is a surgical emergency, requiring time-critical surgery to avoid potential organ loss. This study collates national data on paediatric testicular torsion, offering a comprehensive insight into the current standard of care in England for paediatric patients with suspected testicular torsion.
National data collection using Freedom of Information requests. Of 134 acute secondary care NHS trusts in England that treat paediatric patients, responses were received from 121 trusts - representing 140 hospitals - providing a picture of the current standard of care in England.
Less than two thirds of hospitals have a paediatric A&E department. One third of hospitals use inter-hospital transfer for paediatric patients with suspected torsion, with variable age restrictions. Mean transfer distance was 19·4 miles (range 2·2 to 61·6 miles). No statistically significant relationship between transfer arrangements and the number of non-viable testicle incidents was found (P=0·15, ns). There was no between symptom onset and seeking medical attention is crucial, and raising awareness in the community for young boys and parents may be imperative to improve rates of testicular salvage.
There appear to be various patterns of sidedness with relation to the common urogenital malformations observed in pediatric urology. The objective of this statistical review was to synthesize this data and to assess if these patterns are significant.
Eighteen urogenital conditions were investigated and for each condition the five largest studies that noted laterality were included. The sidedness of each condition was then analysed for statistical significance.
Three conditions had a statistically significant higher proportion on the right side palpable undescended testis (63%, p=0.0002), inguinal hernia (59%, p=0.0001) and hydrocele (60%, p=0.003). Three conditions were significantly more common on the left side impalpable undescended testis (59%, p=0.0008), renal agenesis (54%, p=0.02) and vesico-ureteric junction obstruction (71%, p<0.0001) while both pelvi-ureteric junction obstruction (62%, p=0.09) and absent vas deferens (61%, p=0.11) were trending towards significance.

Lung cancer disproportionately affects older adults. Surgical treatment may result in decreased functional status and health-related quality of life (HRQOL). Population-based data about patient-reported health outcomes for patients with early-stage lung cancer who do not undergo surgery is lacking.
We identified lung cancer patients with localized disease from the SEER-Medicare Health Outcomes Survey (MHOS) linked database. Patients with baseline HRQOL survey prior to cancer diagnosis and follow-up survey ≥1year after diagnosis were selected. Generalized estimating equation (GEE) model was used to compare patients who underwent surgery versus those who did not by demographics and HRQOL measures.
Overall, 108 patients were evaluated, of whom 75 (69%) underwent surgery. Surgical patients were younger (mean 73 versus 80years, p<0.001); otherwise, patient groups were similar. Surgery was not performed based on surgeon recommendation (n=21, 64%), contraindication(s), or patients' decision. Both groups expithout surgery so that patients and surgeons can make informed decisions balancing survival benefit and QOL interests.Older cancer survivors may experience ageing at an accelerated rate when compared to their similar-aged, cancer-free counterparts. Ageing is undoubtedly a significant risk factor for cancer. There is evidence to suggest, however, that this relationship may in fact be bidirectional, with cancer and its treatments contributing to the ageing process. In this review, we outline the current literature linking cancer and anti-cancer therapy to adverse ageing outcomes and explore what additional research is needed in order to conclusively define cancer and its treatment as an accelerator of ageing.
Inflammation and endothelial dysfunction are associated with morbid obesity (MO) and atherosclerosis.
To evaluate inflammation and endothelial function as the initial mechanisms underlying subclinical atherosclerosis in patients with MO, with and without atheromas, and their evolution after bariatric surgery (BS).

Plasma samples from 66 patients with MO were obtained before BS and 6 and 12 months after BS. Patients were divided into 2 groups based on the presence of atheromatous plaques (detected by ultrasound imaging).
Inflammation was increased as demonstrated by changes in the levels of fibroblast growth factor 21, adiponectin, leptin, interleukin 6, tumor growth factor α, nonesterified free fatty acids, lipoprotein(a) and C-reactive protein (CRP). Endothelial dysfunction was characterized by impaired angiogenesis (measured through angiopoietin 1 and 2 and brain-derived neurotrophic factor), vascular function (changes in endothelin 1 and thrombomodulin levels), and diapedesis (changes in intercellular and vascular cell adhesion molecules, and E- and P-selectins).
