The highly infectious and pathogenic novel coronavirus (CoV), severe acute respiratory syndrome (SARS)-CoV-2, has emerged causing a global pandemic. Although COVID-19 predominantly affects the respiratory system, evidence indicates a multisystem disease which is frequently severe and often results in death. Long-term sequelae of COVID-19 are unknown, but evidence from previous CoV outbreaks demonstrates impaired pulmonary and physical function, reduced quality of life and emotional distress. Many COVID-19 survivors who require critical care may develop psychological, physical and cognitive impairments. There is a clear need for guidance on the rehabilitation of COVID-19 survivors. This consensus statement was developed by an expert panel in the fields of rehabilitation, sport and exercise medicine (SEM), rheumatology, psychiatry, general practice, psychology and specialist pain, working at the Defence Medical Rehabilitation Centre, Stanford Hall, UK. Seven teams appraised evidence for the following domains relating to COVID-19 rehabilitation requirements pulmonary, cardiac, SEM, psychological, musculoskeletal, neurorehabilitation and general medical. A chair combined recommendations generated within teams. A writing committee prepared the consensus statement in accordance with the appraisal of guidelines research and evaluation criteria, grading all recommendations with levels of evidence. Authors scored their level of agreement with each recommendation on a scale of 0-10. Substantial agreement (range 7.5-10) was reached for 36 recommendations following a chaired agreement meeting that was attended by all authors. This consensus statement provides an overarching framework assimilating evidence and likely requirements of multidisciplinary rehabilitation post COVID-19 illness, for a target population of active individuals, including military personnel and athletes.Activation of the immune costimulatory molecule cluster of differentiation 40 (CD40) in Müller glia has been implicated in the initiation of diabetes-induced retinal inflammation. Results from previous studies support that CD40 protein expression is elevated in Müller glia of diabetic mice; yet, the mechanisms responsible for this increase have not been explored. Here, we evaluated the hypothesis that diabetes augments translation of the Cd40 mRNA. Mice receiving thiamet G (TMG), an inhibitor of the O-GlcNAc hydrolase O-GlcNAcase, exhibited enhanced retinal protein O-GlcNAcylation and increased Cd40 mRNA translation. TMG administration also promoted Cd40 mRNA association with Müller-cell specific ribosomes isolated from the retina of RiboTag mice. Similar effects on O-GlcNAcylation and Cd40 mRNA translation were also observed in the retina of a mouse model of type 1 diabetes. In cultured cells, TMG promoted sequestration of the cap-binding protein eukaryotic translation in initiation factor 4E (eIF4E) by eIF4E-binding protein 1 (4E-BP1) and enhanced cap-independent Cd40 mRNA translation as assessed by a bicistronic reporter that contained the 5'-UTR of the Cd40 mRNA. Ablation of 4E-BP1/2 prevented the increase in Cd40 mRNA translation in TMG-exposed cells, and expression of a 4E-BP1 variant that constitutively sequesters eIF4E promoted reporter activity. Extending on the cell culture results, we found that in contrast to wild-type mice, diabetic 4E-BP1/2-deficient mice did not exhibit enhanced retinal Cd40 mRNA translation and failed to upregulate expression of the inflammatory marker nitric oxide synthase 2. These findings support a model wherein diabetes-induced O-GlcNAcylation of 4E-BP1 promotes Cd40 mRNA translation in Müller glia.A large amount of tsunami debris from the Great East Japan Earthquake in 2011 was sunk on the seafloor and threatened the marine ecosystem and local communities' economy, especially in fisheries. However, few studies estimated spatial accumulations of tsunami benthic debris, comparing to their flows on the ocean surface. click here Here, a spatially varying coefficient model was used to estimate tsunami debris accumulation considering the spatial structure of the data off the Tohoku region. Our model revealed the number of vessels nearest the coast at the tsunami event had the highest positive impact, whereas the distance from the coast and kinetic energy influenced negatively. However, the effect of the proximity to the coast wasn't detected in the Sendai bay, indicating spatial dependency of these effects. Our model estimation provides the fundamental information of tsunami debris accumulation on the seafloor, supporting early reconstruction and risk reduction in marine ecosystems and local communities. We measured spatial and temporal patterns of ambient noise in dynamic, relatively pristine Arctic marine habitats and evaluate the contributions of environmental and human noise sources. Long-term acoustic recorders were deployed around St. Lawrence Island and the Bering Strait region within key feeding and migratory corridors for protected species that are inherently important to Native Alaskan cultures. Over 3000 h of data from 14 recorders at nine sites were obtained from October 2014 to June 2017. Spatial and temporal ambient noise patterns were quantified with percentile statistics in 1/3rd-octave bands (0.02-8 kHz). Ice presence strongly influenced ambient noise by influencing the physical environment and presence of marine mammals. High variability in noise was observed within and between sites, largely as a function of ice presence and associated factors. Acute contributions of biological and anthropogenic sources to local ambient noise are compared to monthly averages, demonstrating how they influence Arctic soundscapes. The pressing need to solve marine plastics pollution as a multi-source and multi-stakeholder problem is an ongoing global issue. This presents a challenge to policy makers tasked with understanding and accommodating different stakeholder perceptions and weighting their alternative propositions for solutions. In the case of Hong Kong, pollution of the marine environment by discarded plastics, polystyrenes and other items is a cumulative and accelerating problem that has yet to be resolved. We demonstrate how a geostatistical mapping approach can achieve joint stakeholder prioritization at any such regional scale. Joint prioritized area mapping is a methodology that links perspectives of otherwise disparate stakeholders to ecosystem-based-management, thereby balancing ecological, socio-economic and governance principles across temporal and spatial scales. It can serve as a generic scoping tool to help assess any marine spatial planning problem.
