The primary objective of this study was to achieve the lowest effective analgesic concentration (MEC50 and MEC90) of ropivacaine for saphenous nerve block in arthroscopic meniscectomy. The secondary objective was to determine whether the block causes muscle weakness in the postoperative period.

The study was randomized, comparative, and double-blind. Fifty-one patients between 18 and 65 years old of both sexes, ASA I or II, who underwent knee arthroscopic meniscectomy at São Domingos Hospital were included. Patients underwent saphenous nerve block with 10 mL of ropivacaine administered by using the up-and-down method. The ropivacaine concentration was determined based on the previous patient's response (a biased-coin up-down design sequential method). If a patient had a negative response, the concentration of ropivacaine was increased by 0.05% in the next patient; if the response was positive, the next patient was randomized to be administered the same concentration of ropivacaine or a 0.05% lower concentration. Successful block was defined as pain <4 during 6 h. Patients underwent general anesthesia with 30 µg/kg alfentanil and propofol and maintenance with propofol, and, if necessary, remifentanil was administered. Postoperative analgesia was complemented with dipyrone, and if necessary, tramadol (100 mg) could be used. The following parameters were assessed the success of the block; pain intensity after 2, 4, and 6 h; the consumption of remifentanil; time to the first analgesic supplementation; percent of patients who needed analgesics during 6h; and muscle strength.

The MEC50 of ropivacaine was 0.36%, and the MEC90 was 0.477%. The block was successful in 45 patients.

Trigeminal neuropathic pain is very common clinically, but effective treatments are lacking. Chemokines and their receptors have been implicated in the pathogenesis of chronic pain. This study explored the role of the chemokine CXCL10 and its receptor, CXCR3, in trigeminal neuropathic pain in mice.

Trigeminal neuropathic pain was established by partial infraorbital nerve ligation (pIONL) in wild-type and

mice. Facial mechanical allodynia was evaluated by behavioral testing. CDDO-Im ic50 A lentivirus containing

shRNA) was microinjected into the trigeminal ganglion (TG) to knock down
expression. Quantitative polymerase chain reaction assays and immunofluorescence staining were used to examine
mRNA expression and protein distribution. Western blotting was performed to examine activation of extracellular signal-regulated kinase (ERK) and AKT in the TG. Intra-TG injection of an AKT inhibitor was performed to examine the role of AKT in trigeminal neuropathic pain.

pIONL induced persistent trigeminal neuropathic pain, which was alleviated in

mice. Intra-TG injection of LV-
shRNA attenuated pIONL-induced mechanical allodynia. Furthermore, pIONL increased the expression of CXCR3 and its major ligand, CXCL10, in TG neurons. Intra-TG injection of CXCL10 induced pain hypersensitivity in wild-type mice but not in

mice. CXCL10 also induced activation of ERK and AKT in the TG of wild-type mice. Finally, pIONL-induced activation of ERK and AKT was reduced in

mice. Intra-TG injection of the AKT inhibitor alleviated pIONL-induced mechanical allodynia in WT mice but not in

mice.

This study aimed to obtain a translation and validation of the Fear of Pain Questionnaire 9 Items (FOP-9) into simplified Chinese.

The questionnaire was translated following the forward-backward method. The final version was filled out by (n = 300) patients. Cronbach's coefficient was calculated to test the internal consistency of simplified Chinese version of FOP-9 (sc-FOP-9), and 50 painless patients completed the sc-FOP-9 questionnaire within a 2-weeks interval to evaluate test-retest reliability. To verify the construct validity, exploratory factor analysis was used to explore the factor structure, and confirmatory factor analysis was conducted to evaluate the goodness fit of models.

Satisfactory psychometric qualities were obtained (Cronbach's α of the total score was 0.873 and intraclass correlation coefficient was 0.975). Three first-order models were tested and all show a good model fit and the 3-factor structure may be better due to its higher factor loading.

The sc-FOP-9 is a reliable and valid instrument to evaluate the fear of pain among Chinese patients with or without pain. Fear of pain may have an important effect on perioperative pain and chronic pain, and this tool is a good complement to the measurement in mainland China.
