Transitioning from bedside nursing to public health nursing after graduating with my Master's in Public Health was a unique experience, particularly as I navigated the COVID-19 pandemic as a new public health and nursing leader. Personal reflection pieces can provide a meaningful review of the experiences of integrating clinical nursing practice and more broad public health practice. The purpose of this paper is to offer my reflection and synthesis of the lessons learned while leading a Federally Qualified Health Center (FQHC) through the COVID-19 pandemic. Additionally, I reflect on the future of FQHCs and the critical need to advance public health as a field and empower public health nurses.
Cardiac implantable electronic device (CIED)-related perforation is uncommon but potentially lethal. Management typically includes the use of computed tomography (CT) scanning and often involves cardiac surgery.

Patients presenting to a single referral centre with CIED-related cardiac perforation between 2013 and 2019 were identified. Demographics, diagnostic modalities, the method of lead revision, and 30-day complications were examined.

A total of 46 cases were identified; median time from implantation to diagnosis was 14 days (interquartile range=4-50). M344 Most were females (29/46, 63%), 9/46 (20%) had cancer, 18 patients (39%) used oral anticoagulants, and no patients had prior cardiac surgery. Active fixation was involved in 98% of cases; 9% involved an implantable cardioverter defibrillator lead. Thirty-seven leads perforated the right ventricle (apex 24) and 9 punctured the right atrium (lateral wall 5). Abnormal electrical parameters were noted in 95% of interrogated cases. Perforation was visualized in 41% and 6% of cases with chest X-ray (CXR) and transthoracic echocardiography, respectively. CXR revealed a perforation, gross lead displacement, or left-sided pleural effusion in 74% of cases. Pericardial effusion occurred in 26 patients (57%) of whom 11 (24%) developed tamponade, successfully drained percutaneously. Pre-extraction CT scan was performed in 19 patients but was essential in four cases. Transvenous lead revision (TLR) was successfully performed in all cases with original leads repositioned in six patients, without recourse to surgery. Thirty-day mortality and complications were low (0% and 26%, respectively).

The COVID-19 epidemic is affecting the entire world and hence provides an opportunity examine how people from different countries engage in hopeful thinking. The aim of this study was to examine the potentially facilitating role of perceived social support vis-à-vis hope as well as the mediating role of loneliness between perceived social support and hope. This mediating model was tested concurrently in the UK, the USA, and Israel.

In April 2020, as the first wave of the virus struck the three aforementioned countries, we assessed perceived social support, loneliness, and hope in 400 adults per country (N=1,200). Assessments in the UK/USA were conducted via the Prolific platform, whereas in Israel they were conducted via Facebook/WhatsApp.

In all three countries, perceived social support predicted elevated hope, although the effect was smallest in the UK. Loneliness mediated this effect in all three countries, although full mediation was attained only in the UK.

Perceived social support may facilitate hope in dire times, possibly through the reduction of loneliness.

Findings are consistent with respect to the potentially protective role of perceived social support vis-à-vis hope. Perceived social support may increase hope through decreasing loneliness. In the UK, the above-noted mediating effect of loneliness appears to be stronger than in Israel and the USA. Elevated levels of perceivedsocial support should serve as a desired outcome in individual and group psychotherapy, as well as in community based interventions.

We describe two cases of COVID-19-associated pneumonia, which presented skin manifestations in advanced stage of illness, when nasopharyngeal swabs became negative for SARS-CoV-2. The first case presented erythematous, maculopapular lesions; the second developed petechial, vesicular and blood-encrusted lesions on the limbs. Histopathology documented perivascular lymphocytic infiltrates, with prevalent CD4+ T-cells in both patients. The research of SARS-CoV-2 in tissues with real time RT-PCR was negative. Basal keratinocytes displayed C4d deposits in one case, who developed laboratory signs indicative of a procoagulative condition at the same time as the skin rash. Skin manifestations during SARS-CoV-2 infection seem to be clinically relevant and further studies are necessary to assess if they are linked to systemic complications, lack of viral clearance or cascades of immune responses induced by the virus, even in patients affected by mild pneumonia.Appreciating the wide array of applications of lactic acid esters in the flavoring industry as well as in cosmetic and pharmaceutical industries, respectively, their clean and green synthesis gain immense importance. A photochemical route has been adopted where visible light illumination was used as the major driving force to synthesize lactic acid esters. This process involves the application of sulfonically functionalized nitrogen/tungsten-doped nanotitania as photocatalysts. These functionalized catalysts efficiently and repeatedly catalyzed the esterification of lactic acid at ambient temperature. The photo-activity of these catalysts is attributed to the presence of a large number of active sulfonate sites on its surface. As per the proposed photochemical mechanism, illumination initiates the electron discharge from the conduction band, which migrates toward absorbed lactic acid through sulfonate sites forming lactic acid radical.
