The implications of these findings are discussed.Atrial fibrillation (AF), the most common arrhythmia in the adult population worldwide, represents a significant burden in terms of cardiovascular mortality and morbidity and has repercussions on health economics. Oral anticoagulation (OAC) is key to stroke prevention in AF and, in recent years, results from landmark clinical trials of non-vitamin K oral anticoagulants (NOAC) have triggered a paradigm shift in thrombocardiology. Despite these advances, there is still a significant residual vascular risk associated with silent AF, bleeding, premature sudden death and heart failure. The authors review AF epidemiologic data, the importance of new tools for early AF detection, the current role of catheter ablation for rhythm control in AF, the state-of-the-art in periprocedural OAC, the optimal management of major bleeding, the causes of residual premature death and future strategies for improvements in AF prognosis.
To describe the training injury incidence in amateur women's rugby union in New Zealand over two consecutive seasons.
A prospective cohort observational study METHODS A total of 69 amateur women's rugby 15s team playerswere observed. Training exposure and training injury incidence were calculated.
The 38 training injuries resulted in a total injury incidence of 11.4 (8.3-15.6) per 1,000 training-hours. There were 12 injuries that resulted in a time-loss injury incidence of 3.6 (95% CI 2.0-6.3) per 1,000 training-hours. Forwards recorded more total (RR 1.8 [95% CI 0.9-3.5]; p=0.0516) and time-loss (RR 2.0 [95% CI 0.6-6.6]; p=0.2482) injuries than Backs. The tackle was the most common injury cause for total (3.0 [95% CI 1.6-5.6] per 1,000 training-hours.) injuries, but collisions (1.5 [95% CI 0.6-3.6] per 1,000 training-hours.) with the ground or another person were the most common cause for time-loss injuries.The training injuries occurred most often to the lower limb and during the latter part of training sessions. These injuries were mostly minor in nature resulting in minimal time-loss away from training.
The time-loss injury incidence (3.6 per 1,000 training-hours.) for the amateur women's rugby 15s team players was higher than that reported for National (1.2 per 1,000 training-hours.) and Rugby World Cup for women (0.2 to 3.0 per 1,000 training-hours.) competitions.

Animal Assisted Interventions (AAI), which are generally defined as 'pet therapy', focus on the improvement of human health and wellbeing through a multidisciplinary approach and the involvement of domestic animals. An Italian survey conducted in 2017 revealed that donkeys were the third most involved species in AAI. Nevertheless, data on the main features of the providers of Donkey Assisted Interventions (DAI), especially in Italy, and their organisational models are scarce.
This questionnaire-based pilot study aimed to collect information about the features of DAI providers in the Veneto Region to have a general framework about the field.
In this descriptive mixed-method pilot study, we interviewed DAI providers through a computer assisted telephone interview. Respondents were selected by integrating data from the Veneto Region Livestock Database and from the Italian official register of AAI professionals. The questionnaire was designed following the advice from Equator Network.
Our outcome measures included the prevalent organisational system adopted by our respondents, main type of AAI programs, and users. Furthermore, we collected qualitative data about economic and income and respondents' personal opinions about DAI (strengths and weaknesses).
Data analysis revealed a heterogeneous picture of the DAI field with a prevalence of non-profit organisations providing mainly Donkey Assisted Activities to a large range of user categories. Despite the poor economic framework and low income from DAI, DAI providers were strongly motivated and spent more time and efforts in delivering this service.

Medication safety is an important hot spot of patient safety. This fact prompted the Swiss Patient Safety Foundation to conduct its annual CIRRNET (Critical Incident Reporting & Reacting NETwork) reporting month on the topic of "Units of measurement of medicines and calculation errors in prescription/dispensing (mg, g, mmol etc.)".
During the CIRRNET reporting month 2018, the CIRS (Critical Incident Reporting System) managers of the health institutions participating in CIRRNET forwarded all incident reports on the topic to the CIRRNET database. The archive was also searched for usable reports on the same topic. The reports were analysed according to the type of medication involved (type of application and medication class according to ATC codes), the process stage affected (prescription, preparation, administration and documentation) and the error category.
Of the 110 reports analysed, 27 reports originated directly from the reporting month, 83 reports were additionally extracted from the archive. 6m. Therefore, good solution strategies need to be implemented in order to sustainably improve medication safety and thus patient safety.
