In summary, TMTME is inevitable because of the special complementary approach between miRNA and its target. It means that miRNA therapeutics would trigger a series of unknown and unpreventable consequences, which makes it a considerable alternative for application.
To propose a new filtering technique in vitrectomized eyes with glaucoma and report its clinical results and safety.
The medical records of 13 eyes that developed glaucoma following pars plana vitrectomy and underwent pars planectomy, from 2011 to 2018, at Songklanagarind hospital, Hatyai, Songkhla, Thailand were retrospectively reviewed. The main outcome measures were visual acuity (VA), intraocular pressure (IOP), number of glaucoma medications, and postoperative complications. Surgical success was defined as IOP value at the last visit of 6-21 mmHg, regardless of anti-glaucoma medication usage, and without further glaucoma surgery.
The mean follow-up duration was 47.7 ± 32.1 months (range, 0.3-101.1 months). Preoperative BCVA increased from LogMAR 1.01 ± 0.85 to 1.2 ± 0.91 at the last visit (p = 0.233). The mean preoperative IOP was 28.15 ± 9.17 mmHg with an average of 3.46 ± 0.52 anti-glaucoma medications. At the final visit, the mean IOP was 14.08 ± 4.89 mmHg (p = 0.006) and the mean number of anti-glaucoma medications decreased to 1.31 ± 1.38 (p = 0.000). The probability of surgical success was 58.3%, 50%, and 37.5% at 1, 2, and 6 years after pars planectomy, respectively. Postoperative complications included vitreous hemorrhage in 1 eye (7.7%). No retina and pars plicata associated complications were found.
Pars planectomy is efficient and safe as well as requires a short learning curve. It should be considered as an alternative filtering surgery in glaucoma after vitrectomy, especially with an extensive limbal scar that might be a limitation in trabeculectomy and GDDs techniques and outcomes.

Community photoscreening for amblyopia had successfully been adopted by many communities, however many clinics curtailed screening as a result of the COVID-19 pandemic. We modified three conventional devices and tested them for outdoor, drive-by socially distanced photoscreening and refraction.
External frames that provide luminance control and focus distance were fashioned for plusoptiX S12 (Nuremberg, Germany), Adaptica 2WIN in Kaleidos case (Padova, Italy) and the Rebion blinq (Boston, USA). Children were screened by each device and then Retinomax (Righton, Japan) before AAPOS guideline validation.
Eighty-eight children average age 8±7 years had precise refraction and alignment from which 69% AAPOS 2003 risk factors were determined. The sensitivity/specificity/inconclusive rate for plusoptiX was 85%/96%/16%, for 2WIN 79%/89%/5% and for blinq 43%/74%/8%. Blinq improved to 54%/70% when screening for amblyopia ± strabismus. Bland Altman analysis of spherical equivalent showed plusoptiX and 2WIN with less over-minus than Retinomax and J0 and J45 vectors highly reliable for astigmatism determination.
The infrared photorefractors in modified cases reliably screened amblyopia risk factors and refraction. The birefringent scanner provided drive-by results but less reliably with wire-frame opaque case than without the case in a dimly lit room. Modified drive-by photoscreeners could help reduce amblyopia and provide socially distanced refraction during an extended pandemic.

We previously reported a novel technique using an elastic silicone bowl, which was handmade from a phaco test chamber, to salvage dislocated lenses and perform phacoemulsification and aspiration (PEA) on the bowl. However, creating the bowl was challenging.
A factory-made elastic silicone sheet (ESS), a ready-to-use alternative for the posterior capsule of the lens, was prepared. We used this sheet to salvage and support the dislocated/subluxated lenses in 11 eyes of nine patients.
We could use a sheet immediately after we opened the package. In all the cases, we could insert a sheet beneath the lens and PEA performed at the center of the operative field. There was only one incident where the lens could not be placed onto the disc at the first attempt, but the lens was eventually salvaged. There was no risk of rupture during PEA, and the ESS could be removed completely without any complications.
The factory-made ESS described here can be used for salvaging dislocated/subluxated lenses and safely performing PEA.

To compare the short-term therapeutic effects of topical corticosteroids in patients with refractory dry eye disease (DED) according to the tear matrix metalloproteinase 9 (MMP-9) point-of-care positivity.
This study was conducted on 137 patients who were referred from other eye clinics, complaining of irresponsive DED or acute DED flares despite routine treatment with topical cyclosporin or diquafosol. The patients received treatment with topical corticosteroids for 1 month. DED was evaluated by SANDE (Symptom Assessment in Dry Eye) questionnaire, tear film breakup time, ocular surface staining score, and meibomian gland dysfunction stage. The InflammaDry MMP-9 immunoassay was conducted in more symptomatic eyes of all patients. check details The changes in the subjective symptoms were additionally surveyed as symptom improvement score.
The mean age of the patients was 57.8±13.4 years, and the tear MMP-9 positivity was 73.0%. Topical corticosteroids treatment showed significant improvement of symptoms and signs in the patients with refractory DED irrespective of the positivity of MMP-9 (each
