The purpose of this study was to explore what motivates family members to visit a relative with dementia who has been transferred to a nursing home in Taiwan.

Data were collected for this qualitative descriptive study using audiotaped, semi-structured, in-depth, face-to-face interviews. A total of 20 family members of elderly nursing home residents participated in the study. Nursing home residents were from four nursing homes in Taiwan and had been diagnosed with probable or possible dementia by a psychiatrist or neurologist. Transcribed audiotaped interviews were analyzed using thematic analysis.

Most family members were the children of the residents (n =17, 85%). The theme describing the core motivation for family members' visits to nursing home residents was "to maintain the unforgotten family affection." This motivation comprised four relevant categories hoping to slow degeneration, providing a congruous environment, honoring filial and karmic duty, and ensuring the quality of care.

The proposed "pause-then-cancel" model suggests that the subthalamic nucleus provides a rapid stimulus-unspecific "pause" signal, followed by a stop-cue-specific "cancel" signal from striatum-projecting arkypallidal neurons. To determine more precisely the relative contribution of the different basal ganglia nuclei in stopping, we simulated a stop-signal task with a spiking neuron model of the basal ganglia, considering recently discovered connections from the arkypallidal neurons, and cortex-projecting GPe neurons. For the arkypallidal and prototypical GPe neurons, we obtained neuron model parameters by fitting their neuronal responses to published experimental data. Our model replicates findings of stop-signal tasks at neuronal and behavioral levels. We provide evidence for the existence of a stop-related cortical input to the arkypallidal and cortex-projecting GPe neurons such that the stop responses of the subthalamic nucleus, the arkypallidal neurons, and the cortex-projecting GPe neurons complement each other to achieve functional stopping behavior. Particularly, the cortex-projecting GPe neurons may complement the stopping within the basal ganglia caused by the arkypallidal and STN neurons by diminishing cortical go-related processes. Furthermore, we predict effects of lesions on stopping performance and propose that arkypallidal neurons mainly participate in stopping by inhibiting striatal neurons of the indirect rather than the direct pathway.Since 2013, H5N6 highly pathogenic avian influenza viruses have caused considerable economic losses in the poultry industry and have caused 24 laboratory-confirmed human cases. In this study, we isolated nine (B1-B9) H5N6 viruses from healthy ducks in Guangdong Province, Southern China from December 2018 to April 2019. Phylogenetic analysis revealed that B1, B2, B3, B4, B5, B7, B8, and B9 clustered into the G1.1 genotype and shared high sequence similarity with human H5N6 isolates from Southern China in 2017 and 2018. Meanwhile, B6 clustered into the G1.1.9 genotype. The hemagglutinin (HA), neuraminidase (NA) and nonstructural protein (NS) gene segments of B6 were closely related to the human H5N6 isolates, while the other genomic segments were closely related to H5N6 viruses isolated from waterfowl in Southern China. Compared to B7, B6 had higher pathogenicity and induced stronger inflammatory responses in mice. B6 carried a full-length PB1-F2 protein (90 aa), while the rest carried an 11-amino acid C-terminal-truncated PB1-F2. The PB1-F2 protein may increase the virulence of B6 compared to that of B7. Fixed-effects model analysis was used to obtain the pooled relative risks (RRs) and 95% confidence intervals (CIs) of lifestyle intervention for diabetes in women with GDM.
