In low-resource, malaria-endemic settings, accurate diagnosis of febrile illness in children is challenging. The World Health Organization (WHO) currently recommends laboratory-confirmed diagnosis of malaria prior to starting treatment in stable children. Factors guiding management of children with undifferentiated febrile illness outside of malaria are not well understood.
This study examined clinical presentation and management of a cohort of febrile Kenyan children at five hospital/clinic sites from January 2014 to December 2017. Chi-squared and multivariate regression analyses were used to compare frequencies and correlate demographic, environmental, and clinical factors with patient diagnosis and prescription of antibiotics.
Of 5735 total participants, 68% were prescribed antibiotic treatment (n = 3902), despite only 28% given a diagnosis of bacterial illness (n = 1589). Factors associated with prescription of antibiotic therapy included negative malaria testing, reporting head, ears, eyes, nose an of febrile children in these settings is difficult given the lack of diagnostic testing. Providers may benefit from improved clinical education and implementation of enhanced guidelines in this era of malaria testing, as their management strategies must rely primarily on critical thinking and decision-making skills.The early outbreak of coronavirus disease-2019 (COVID)-19 became associated with various 'hot spots' in the USA, particularly in large cities. However, despite the widespread nature of the outbreak, much of what is known about the virus' impact and clusters is understood either for individuals, or at the state level. This paper assesses the predictors of outbreaks at the neighborhood level. Using data from the Louisiana Department of Health, we use spatial regression models to analyze the case count through 3 May 2020 and its relationship to individual and geographic neighborhood characteristics at the census tract level. We find a particularly strong and large correlation between race and COVID-19 cases, robust to model specification and spatial autocorrelation. In addition, neighborhoods with lower rates of poverty and those with fewer residents over 70 have fewer cases. Policy makers should adjust testing strategies to better service the hardest hit populations, particularly minorities and the elderly. In addition, the results are greater evidence of the impact of systemic issues on health, which require a long-term strategy for redress.
The federal Right-to-Try (RTT) Act created an alternate regulatory pathway for preapproval access to investigational drugs. A few studies have examined the experiences of physicians with Food and Drug Administration's Expanded Access Programs (EAP), but no study has yet to examine their attitudes and experiences toward RTT.
This study explored the views of 21 oncologists at a major cancer center with 3 main sites across the U.S. using semi-structured interviews and qualitative analysis. Participants were selected to have experience with EAP.
Most oncologists had limited familiarity with RTT and several reported confusion about the legislation, including whether patients have a right to investigational drugs and an obligation for companies to provide them. While oncologists were interested in decreased regulatory burdens, three areas of concern were articulated lack of safety and oversight; unclear structure and no provision for data collection; and potential heightening of patient expectations. Only fou the need for medical centers to inform oncologists about RTT and other preapproval pathways so that they are able to address questions from patients interested in non-trial investigational drugs.
We explored the portrayal of older adults and the public response to topics concerning older adults during the COVID-19 pandemic in Chinese social media (Weibo topics, equivalent to hashtags on Twitter). We also explored the temporal trends of dominant themes to identify changes over time.
Topics related to older adults were searched in the Weibo topic search engine between January 20 and April 28, 2020. Overall, 241 topics and their view frequency and comment frequency were collected. Inductive thematic analysis was conducted to classify the topics into themes. The popularity of each theme was also analyzed. In addition, the frequency with which each theme appeared during the three major stages of the pandemic (outbreak, turnover, post-peak) was reported.
Six main themes were identified. SKL2001 "Older adults contributing to the community" was the most prominent theme with the highest average comment frequency per topic. It was also the most dominant theme in the first stage of the pandemic. "Older patients in hospitals" was the second most prominent theme, and the most dominant theme in the second and third stages of the pandemic. The percentage of topics with the themes "Care recipients" and "Older adults caring for the young" increased over time.

Scientists have long argued that sexual conflict is a key driver of speciation. This hypothesis, however, has been highly debated in light of empirical evidence. Recent advances in the study of macroevolution make it possible to test this hypothesis with more data and increased accuracy. In the present study, we use phylogenomics combined with four different diversification rate analytical approaches to test whether sexual conflict is a driver of speciation in brush-footed butterflies of the tribe Acraeini. The presence of sphragis, an external mating plug found in most species among Acraeini, was used as a proxy for sexual conflict. Diversification analyses statistically reject the hypothesis that sexual conflict is associated with shifts in diversification rates in Acraeini. This result contrasts with earlier studies and suggests that the underlying mechanisms driving diversification are more complex than previously considered.
