The establishment of the Massive Open Online Course (MOOC), "Introduction to Histology," which delves into the human body's tissues, enabled the dissemination of digital resources to a broader audience, complementing the on-campus instruction for students. This article elucidates the pedagogical decisions that informed the design of the MOOC, its incorporation with in-person meetings, and the achievement of targeted learning objectives. It has been demonstrated that the pedagogical alignment of learning outcomes, as classified by cognitive levels, corresponds to online and in-person learning and assessment. Using both perception and performance data, the influence of this blended design on the academic program has been assessed. Among the findings were student satisfaction, engagement, and motivational cues. The educational strategy's implementation preserved performance levels, enabling the realization of the teachers' foreseen objectives. The advantages of incorporating a Massive Open Online Course (MOOC) into traditional classroom instruction were emphasized, along with potential obstacles to its effective implementation.
Despite its rarity, visceral involvement in cutaneous T-cell lymphoma (CTCL) remains a poorly investigated and complex complication. We investigated the clinical presentations, treatment approaches, and outcomes of vCTCL patients at our medical center.
The Winship Cancer Institute at Emory University performed a retrospective review of cases, specifically focusing on patients with vCTCL, a subset of patients with confirmed CTCL diagnoses. vCTCL was defined as a TNMB stage 4B diagnosis with extracutaneous metastasis (M1), which was either pathologically confirmed or highly likely suspected clinically through imaging, patient symptoms, and clinical assessment by the treating physician. A patient cohort was identified from our CTCL database, which spanned the period from 1990 to 2022 and included 656 cases. Clinical features were comprehensively described in an in-depth manner. The clinical outcomes of overall survival (OS) and progression-free survival (PFS) were quantified through Kaplan-Meier curve analysis and univariate Cox regression.
From the substantial group of 656 patients with vCTCL, a contingent of 26 individuals was determined. A striking 423% of the patient sample comprised Black individuals. Among the patients evaluated, twenty-two were diagnosed with mycosis fungoides/Sézary syndrome (MF/SS), and four others displayed alternative types of cutaneous T-cell lymphoma (CTCL), including primary cutaneous anaplastic large-cell lymphoma (pcALCL), gamma-delta lymphoma, and cytotoxic T-cell lymphoma. The progression-free survival (PFS) median was 73 months (range 38 to 111 months), while the overall survival (OS) median was 121 months (range 99 to 182 months). The average time taken for metastasis to develop, after the initial diagnosis, was 121 months. The most prevalent M1 locations were liver, exhibiting a frequency of 192%, and lung, with 423%. M1 sites located outside the liver and lungs were linked to a worse overall survival (HR 89, 95% CI 27-295, P<.001) and poorer progression-free survival (HR 43, 95% CI 144-127, P=.009). The implementation of any treatment or baseline factor did not lead to enhanced survival.
Past cases of vCTCL, analyzed retrospectively, showcase a persistent pattern of resistance to therapy and undesirable patient outcomes.
The retrospective study demonstrates a disheartening pattern of treatment resistance and poor patient outcomes in vCTCL cases.
