These results have implications for support services, as they highlight the relationship between mental health (especially depression) and quality of life. Given that sleep challenges (e.g. insomnia) are related to mental health, an intervention addressing both insomnia and mental health may be most useful in helping autistic individuals improve their quality of life.Cage subsidence in transforaminal lumbar interbody fusion (TLIF) is one of the concerns. The objective was to numerically assess the resulting segmental lumbar lordosis (SLL) and stresses at the bone-cage interface as functions of cage height (8- vs. 10-mm) and cage placement (oblique asymmetric, vs. anterior symmetric) for normal and osteoporotic bone quality. A L4-L5 detailed finite element model of TLIF was subjected to the functional loadings of 10 Nm in the physiological planes after the application of a 400 N follower-load. The SLL was increased by 0.9° (11%) and 1.0° (13%), respectively in oblique asymmetric and anterior symmetric cage placement with 8-mm height; they were 1.4° (18%) and 1.7° (21%) for the 10-mm cage. The maximum stresses at the cage-bone interface, in normal bone model, were increased up to 16% and 41% with the 10-mm cage and asymmetric oblique placement, respectively, and they increased up to 16% and 43% in osteoporotic bone model. The greater cage resulted to a higher simulated SLL. Oblique asymmetric placement and the use of a greater cage may increase the risk of cage subsidence. Due to the lower mechanical strength of osteoporotic bone, the risk of cage subsidence should be higher.Adolescent HIV testing rates remain low with many unaware of their status. click here We explored factors associated with HIV testing uptake among adolescents aged 15-19 years using data from the Zambian Demographic Health Survey 2013-2014. The sample consisted of 7030 adolescents of which 42% reported ever testing for HIV. We found that as the age of a respondent increased so did their odds of testing (aOR = 1.26; 1.21-1.32); females had higher odds of testing than males (aOR = 1.719; 1.53-1.92); those with secondary or higher education (aOR = 3.64; 2.23-5.96) and those with primary education (aOR=1.97; 1.21-3.19) had higher odds of testing than those with no education; those who were formerly married or living with a partner (aOR =  4.99; 2.32-10.75) and those who were currently married or living with a partner (aOR = 4.76; 3.65-6.21) had higher odds of testing than those who were never married or lived with a partner; as the age at first sexual intercourse increased so did the odds of testing (aOR = 1.07; 1.06-1.08); and as HIV knowledge increased so did the odds of testing (aOR = 1.13; 1.06-1.19). The data points to population level social determinants that may be targeted to increase testing among adolescents. Background Persons of African ancestry are thought to carry a higher risk for extrapyramidal syndromes (EPS) in schizophrenia.Aim We investigated the phenomenon of spontaneous and treatment-emergent EPS in a sample comprising Xhosa (South Africa) and Yoruba (Nigeria) Africans with first-episode schizophrenia and first exposure to antipsychotics. Methods The Extrapyramidal Symptom Rating Scale (ESRS) and a variety of validated tools were used for the assessment of participants before, and two-weekly after treatment with low dose flupenthixol decanoate. Participants were followed up for 12 months. Association of EPS with clinical characteristics was investigated using Pearson's correlation and linear regression analyses. Results Of 88 participants at baseline, 16 (18.1%) had at least one definite EPS prior to antipsychotic exposure and 34 (38.6%) had treatment-emergent EPS. While spontaneous Parkinsonism was associated with negative symptoms (r = 0.2, p = 0.043; β = 0.6, p = 0.043), treatment-emergent EPS demonstrated non-significant correlations with clinical characteristics. Apart from dyskinesia, the frequency of treatment-emergent EPS decreased over 12 months observation. Conclusion These findings support the hypothesis suggesting that spontaneously occurring Parkinsonism in schizophrenia may be the motor spectrum of negative symptomatology. Future studies of this relationship may lead to early identification of patients who may be more sensitive to EPS.This paper proposes a new variant of spiking neural P systems (in short, SNP systems), nonlinear spiking neural P systems (in short, NSNP systems). In NSNP systems, the state of each neuron is denoted by a real number, and a real configuration vector is used to characterize the state of the whole system. A new type of spiking rules, nonlinear spiking rules, is introduced to handle the neuron's firing, where the consumed and generated amounts of spikes are often expressed by the nonlinear functions of the state of the neuron. NSNP systems are a class of distributed parallel and nondeterministic computing systems. The computational power of NSNP systems is discussed. Specifically, it is proved that NSNP systems as number-generating/accepting devices are Turing-universal. Moreover, we establish two small universal NSNP systems for function computing and number generator, containing 117 neurons and 164 neurons, respectively.BACKGROUND Compared to walking on level ground, ascending stairs requires a large range of motion not only of the hip and knee joint, but also of the ankle joint. The prosthesis often worn by persons with partial foot amputation largely prevents the ankle motion needed during stair ascent. OBJECTIVES Aim of this study was to assess subjects with a Chopart amputation utilizing a clamshell device during stair ascent to identify potential biomechanical deficits. STUDY DESIGN Cross-sectional study with reference group. METHODS Six subjects with unilateral Chopart amputation and 17 unimpaired subjects underwent three-dimensional motion analysis while ascending stairs in a step-over-step manner. RESULTS During weight acceptance, the involved side showed increased external hip-flexing and reduced knee-flexing moments and the sound side a higher ankle power than in the control group. The sound side showed higher external knee-flexing, dorsi-flexing, and hip-adducting moments than the controls during weight acceptance.
