Mechanical ventilation (MV) is the key to the successful treatment of acute respiratory failure (ARF) in the intensive care unit (ICU). The study aims to formalize the concept of individualized MV strategy with finite mixture modeling (FMM) and dynamic treatment regime (DTR).

ARF patients requiring MV for over 48h from 2008 to 2019 were included. FMM was conducted to identify classes of ARF. Static and dynamic mechanical power (MP_static and MP_dynamic) and relevant clinical variables were calculated/collected from hours 0 to 48 at an interval of 8h.

A total of 8768 patients were included for analysis with a mortality rate of 27%. FFM identified three classes of ARF, namely, the class 1 (baseline), class 2 (critical) and class 3 (refractory respiratory failure). The effect size of MP_static on mortality is the smallest in class 1 (HR for every 5 Joules/min increase 1.29; 95% CI 1.15 to 1.45;
< 0.001) and the largest in class 3 (HR for every 5 Joules/min increase 1.83; 95% CI 1.52 to 2.20;
< 0.001).

MP has differing therapeutic effects for subtypes of ARF. Optimal MP estimated by DTR model may help to improve survival outcome.

Preoperative anemia is an important pillar of perioperative patient blood management. However, there was no literature comprehensively described the current situation of preoperative anemia in China.

We conducted a national retrospective cross-sectional study to assess the prevalence and intervention of preoperative anemia in Chinese adults. Data were from the National Preoperative Anemia Database based on hospital administration data from January 1, 2013 to December 31, 2018.

A total of 797,002 patients were included for analysis. Overall, 27.57% (95% CI 27.47-27.67) of patients had preoperative anemia, which varied by gender, age, regions, and type of operation. Patients who were female, age over 60 years old, from South China, from provinces with lower per capita GDP, underwent operations on the lymphatic and hematopoietic system, with laboratory abnormalities were more likely to have a high risk of preoperative anemia. Among patients with preoperative anemia, 5.16% (95% CI 5.07-5.26) received red blwill be required to optimize the treatment of preoperative anemia.

Daily oral emtricitabine (FTC, F)/tenofovir disoproxil fumarate (TDF) combination is approved for HIV pre-exposure prophylaxis (PrEP) in men and women. Tenofovir alafenamide fumarate (TAF) is a newer, more potent prodrug of tenofovir (TFV), and in combination with FTC, has recently been approved for prevention of HIV through rectal transmission.

This Phase I, prospective, interventional, randomized study was conducted in three clinical sites PROFAMILIA, Santo Domingo, Dominican Republic; University of Pittsburgh and Eastern Virginia Medical School. We assessed the multi-compartmental pharmacokinetics (primary outcome) and safety (secondary outcome) among HIV uninfected women randomized to F/TDF (200mg/300mg) or F/TAF (200mg/25mg; F/TAF25) (n=24) in a single dose phase (SDP) and F/TDF, F/TAF (200mg/10mg; F/TAF10), or F/TAF25 (n=75) in a multiple dose (14 daily doses) phase (MDP). We described PK parameters in plasma, peripheral blood mononuclear cells (PBMCs), and cervicovaginal (CV) and rectal fluids and FTC and FTC-TP concentrations in all compartments support the potential of F/TAF as a new PrEP combination for women.
F/TAF was safe and well-tolerated. TFV-DP concentrations were higher in PBMCs and similar or higher (4h post dose) in female genital tract tissues for F/TAF versus F/TDF. High FTC and FTC-TP concentrations in all compartments support the potential of F/TAF as a new PrEP combination for women.
West African studies have suggested that national immunisation campaigns with oral polio vaccine (C-OPV) may non-specifically reduce all-cause child mortality rate by 15-25%. We investigated whether C-OPVs had similar non-specific effects in rural Bangladesh from 2004 to 2019.

Chakaria, is a health and demographic surveillance system (HDSS) in Southern Bangladesh. From 2004-2011 the HDSS covered a random sample of households; from 2012 to 2019 it covered a random sample of villages. Using Cox proportional hazards models, we calculated hazard ratios (HR) comparing mortality for children under 3 years of age after C-OPV versus before C-OPV to assess the effect of receiving a C-OPV. We allowed for different baseline hazard function in the two periods (2004-2011, 2012-2019), with separate models for each period.

There were 768 deaths (2.1%) amongst 36,176 children. Rapamycin The HR after C-OPV was 0.69 (95% confidence interval 0.52-0.90). National campaigns providing vitamin A or measles vaccine did not have similar effects.
