recommendations may put the benefits of antineoplastic therapy at risk.Although single-lung transplant on the side with better lung function is challenging in patients with significantly asymmetrical lung function between the right and left sides, it sometimes can be a realistic option because of the recipient's condition and from the viewpoint of organ sharing. We report our experience with a successful case of single-lung transplant on the side with a pulmonary perfusion ratio of 89%. The transplant was performed with the patient under central venoarterial extracorporeal membrane oxygenation through a clamshell incision, and the patient had an acceptable short- and long-term outcome with a remarkable improvement of lung function.Primary membranous nephropathy is an autoimmune kidney disease and the most common cause of nephrotic syndrome in adults. About 70%-80% of cases are caused by anti-PLA2R antibodies. Its association with anti-THSD7A antibodies and other autoantibodies has also been described. Recent pilot studies and clinical trials have shown that several biological agents targeting autoantibody-producing cells are effective in controlling the disease with an acceptable safety profile. In this narrative review, we update key concepts about the pathogenesis, autoantibody-based diagnosis, and kidney biopsy findings in primary membranous nephropathy. In addition, we propose a diagnostic and therapeutic algorithm, including guidance on monitoring the response to therapy. We compare the efficacy and safety of currently available treatments, including rituximab and new biological agents, and identify unmet clinical needs.Influenza infection has been identified as a triggering or exacerbating factor for cardiovascular events. To analyse the effect of influenza vaccination on cardiovascular disease, a systematic search of studies published between 2009-2019 was conducted. All the studies that evaluated the effect of vaccination against influenza on cardiovascular events and their outcome were considered. find more Finally, 30 of the 1147 identified studies were included. These studies show a protective effect of the influenza vaccine on the development of cardiovascular events (myocardial infarction, cerebrovascular accidents, atrial fibrillation, and heart failure) and on the worsening of these conditions. Furthermore, the data showed that vaccinated patients have a lower risk of death from cardiovascular pathologies. Influenza vaccination is therefore an effective measure in the prevention of cardiovascular diseases both in patients with established cardiovascular disease and in the population without previous coronary pathology.
How material loss from sleeves and drills is affected when different guide sleeve materials and different sizes of implant drills are used for different regions of surgical guides is unclear.
The purpose of this invitro study was to compare the amount of material loss from different guide sleeves (zirconia and cobalt-chromium) and drills of different diameters during osteotomy preparation in different regions.
Three tooth-supported surgical guides with sleeve holes positioned in the first premolar and second molar sites were prepared. Guide sleeves (Ø 2.20 mm, 3.40 mm, and 4.05 mm) were milled from zirconia (n=60) and cobalt-chromium (n=60) blocks. A total of 12 titanium nitride-coated stainless steel twisted drills (n=6 per sleeve material) of different diameters (Ø 2.00, 3.20, 3.85 mm) were used with corresponding sleeves during the drilling. The weight loss from the drills and the volume loss from the guide sleeves after drilling were analyzed by using multiple linear mixed effect models (α=.05).
Amaterial loss from drills and sleeves. The sleeve material and the region did not affect the material loss from drills.
Scannable healing abutments are a convenient option to facilitate impression making for implant-supported restorations. However, studies evaluating the accuracy of the impression technique with scannable healing abutments are lacking.
The purpose of this invitro study was to evaluate the accuracy of implant impression techniques with scannable healing abutments.
A partially edentulous mandibular dentiform model was fabricated with an epoxy resin, and implants were placed in the mandibular right second premolar and first molar areas. A maxillary dentiform model was then fabricated, and both models were mounted on an articulator. Scan data were obtained from the mounted models and set as the reference scans. The experimental models were divided into 4 groups (n=10). The conventional pick-up impression technique and definitive casts were used in group CI. The scan data from the definitive casts were obtained with a 3D model scanner. An intraoral scanner with a digital body scan was used in group DS. Group r interarch deviations (P>.05).
The accuracy of intraoral scanning with scannable healing abutments was comparable with that of conventional pick-up impression techniques and digital scans with scan bodies. However, model scanning with scannable healing abutments may not be clinically acceptable for implant impressions.
The accuracy of intraoral scanning with scannable healing abutments was comparable with that of conventional pick-up impression techniques and digital scans with scan bodies. However, model scanning with scannable healing abutments may not be clinically acceptable for implant impressions.
The intraoral scanning of the edentulous arch might be challenging for an inexperienced operator because of the large mucosal area and the use of scan bodies.
The purpose of this exvivo study was to compare the trueness of 5 intraoral scanners in replicating implant scan bodies and soft tissues in an edentulous maxilla and to investigate the effects of operator experience.
The maxilla was resected from a fresh cadaver, 5 implants placed, and a reference scan made. Eight scans were made by experienced operators and 8 by an inexperienced operator with each scanner (iTero Element 2, Medit i500, Primescan, TRIOS 3, TRIOS 4). The implant platform deviation was measured after complete surface alignment and after scan body alignment. Deviation data were analyzed with a generalized linear mixed model (α=.05).
After complete surface alignment, the mean ±standard deviation implant platform deviation was higher for the inexperienced operator (421 ±25 μm) than for experienced ones (191 ±12 μm, P<.001) for all scanners.
