Local anesthetic systemic toxicity (LAST) in children occurs at an estimated rate of 8 per 100 000 blocks (95% CI 0.3 to 1.6). Due to the potential lethality of this iatrogenic complication, a better clarity of risk factors, clinical presentation, and prognosis may be clinically useful for contemporary treatment and prevention.
An extensive literature search and detailed analysis of reported cases of LAST in patients less than 18 years of age published between April 2014 and August 2019 was performed. Clinical patterns were summarized.
Thirty-two cases of LAST were described in 17 published articles and 2 additional cases reported to lipidrescue.org, for a total of 34 cases. Three patients did not meet the inclusion criteria. Thirty-one cases were included in the analysis.
Infants represented a disproportionately large number of reports of LAST. The majority of local anesthetic doses were within currently published guidelines. The convoluted membranes in these regions give the impression of cadherin clustering by fluorescence microscopy, but the ratio of cadherin to membrane is constant. Nevertheless, these interlocking fingers of membrane are important for adhesion because perturbing their formation disrupts cell adhesion. In contrast, blocking myosin-dependent contractility does not disrupt either the interdigitated microspikes or lateral membrane adhesion. "Puncta" are zones of strong cell-cell adhesion not due to cadherin clustering but that occur because the interdigitated microspikes expand the surface area available for adhesive bond formation and increase the asperity of the cell surface to promote friction between cells.Achieving persistence in household behavior modification has been a central but elusive goal of environmental conservation attempts that rely on behavioral interventions. We implemented a habit change intervention, designed to achieve persistent change in household water conservation behavior in an affluent residential community in urban India. We found a 15 to 25% reduction in household water consumption in the absence of any volumetric pricing. Most importantly, the effects of our 5-wk intervention persisted for more than a year, after which marginal pricing was introduced. The treatment gap was not bridged even after a year under the marginal price regime.The meteoritic material falling on Earth is believed to derive from large break-up or cratering events in the asteroid belt. The flux of extraterrestrial material would then vary in accordance with the timing of such asteroid family-forming events. In order to validate this, we investigated marine sediments representing 15 time-windows in the Phanerozoic for content of micrometeoritic relict chrome-spinel grains (>32 μm). We compare these data with the timing of the 15 largest break-up events involving chrome-spinel-bearing asteroids (S- and V-types). Unexpectedly, our Phanerozoic time windows show a stable flux dominated by ordinary chondrites similar to today's flux. Only in the mid-Ordovician, in connection with the break-up of the L-chondrite parent body, do we observe an anomalous micrometeorite regime with a two to three orders-of-magnitude increase in the flux of L-chondritic chrome-spinel grains to Earth. This corresponds to a one order-of-magnitude excess in the number of impact craters in the mid-Ordovician following the L-chondrite break-up, the only resolvable peak in Phanerozoic cratering rates indicative of an asteroid shower.
