Adult ADHD and Comorbid Disorders
Many adults with ADHD also suffer from comorbid conditions like major depression (Kessler and colleagues 2006). Many patients do not respond to medications for depression and may be able to benefit from psychotherapy.
Neurotransmitters are brain chemicals that regulate and increase levels of certain brain chemicals. Commonly prescribed stimulants include amphetamine, methylphenidate and similar medications.
Medications
Adults can take a variety of drugs that are prescribed for ADHD in children. The dosages and the side effects are different. The doctor will look at the medical history, the family health history, and other factors of the individual to determine which medication is best.
The most common type of ADHD medication is a stimulant. It is used to regulate levels of two chemical messengers in the brain, norepinephrine as well as dopamine. Stimulants such as methylphenidate (also called Adderall) and dextroamphetamine increase the availability of neurotransmitters, which help improve concentration, decrease impulsivity, and reduce aggression. They come in many forms like chewable tablets, liquids, and skin patches. These stimulants that last longer are popular due to their improved patient compliance, and an easier, longer-lasting improvement in symptoms.
It is important to remember that comorbid conditions such as anxiety and depression may influence the response to ADHD medications. Therefore, it is recommended that these psychiatric problems be treated first, with medication used as secondary measure. Some medications that can be used to treat comorbid mood disorders include atomoxetine and bupropion and tricyclic antidepressants (Wilens 2004).
treatment of add in adults may exhibit some patients may experience a "rebound" effect in which they experience an increase in ADHD symptoms after stopping stimulant medications. In these cases it is possible to try a short-term trial of a low-dose stimulant medication may be beneficial.
Certain people are not able to take stimulant medication or take it because of addiction to drugs or other health conditions such as severe hypertension or heart problems. In these cases, nonstimulant medication such as atomoxetine (Strattera) and viloxazine (Qelbree) can be effective. These medications do not cause the same high rates of side effects as stimulants and have less risk of abuse and diversion.
Psychotherapy
Psychotherapy (talk therapy) can be beneficial for those suffering from ADHD. It can help them develop strategies to improve their lives, deal with anxiety and stress, work out conflicts, and cope with issues in relationships or at work. It can also help those suffering from mental illness that are co-existing like depression and addiction disorders.
Talk therapies include cognitive-behavioral therapy (CBT), mindfulness-based cognitive therapy and schema therapy. These therapies are based upon the idea that thoughts and beliefs affect how a person feels, thinks and acts. CBT helps people identify negative behaviors and thinking patterns, and then develops and implements positive strategies to alter them. Meditation techniques are employed in mindfulness-based cognitive therapy to help a person focus on their emotions and thoughts as well as control their impulses and improve executive functioning. Schema therapy, as well as other forms psychotherapy that focus on a person’s family history can be beneficial for people with ADHD.
In addition to medications psychotherapy can assist in addressing the particular issues caused by ADHD problems, like having trouble staying on track with schoolwork, resolving conflicts, being able to get along with others, forgetting important dates or obligations as well as impulsive decisions that result in financial or legal troubles. It can also assist a person explore and overcome issues that could be contributing to their ADHD symptoms, such as anxiety or depression.
Psychotherapy can help someone develop healthy coping strategies and reduce stress through exercises, sleep improvement and healthy eating habits. It can also provide support to a partner or family member who is struggling with the effects of ADHD on their relationship. Therapy for families and marriage can assist families and couples improve communication and learn to manage ADHD symptoms together.
Cognitive Behavioral Therapy
CBT is a form of psychotherapy that is focused on changing the client's negative thinking patterns. It uses skill-based dialogue with an experienced mental health professional (a therapist) to address the root causes of their symptoms. This treatment is designed to help clients overcome difficulties and develop coping skills that are in line with their daily routines.
Therapists who specialize in adult ADHD-CBT teach clients to recognize unhelpful thoughts and help them develop skills to replace them with more productive thoughts. This process can be lengthy. It is a viable option for adults who do not get enough relief from their medication.
In an upcoming RCT that was conducted, it was discovered that cognitive behavior therapy in combination with medication is superior over cognitive behavior therapy by itself in terms of improving the core symptoms, emotional symptom self-esteem, and social functioning in adults suffering from ADHD (Clarke and colleagues. 2017). The combination of CBT and methylphenidate is especially effective for those with more severe underlying issues.
The therapists who provide this treatment usually collaborate with the client to devise an action plan specific to solving their issues. They usually teach clients the fundamentals of therapy that are easy to apply and have a high chance of success. A chart system is often used to track progress and reward positive behaviour. For children, this could include a daily report that lists all the in-school activities or positive behaviors they need to accomplish and a system for recording and earning rewards. Therapists can also talk about the possibility that negative consequences could be inflicted upon a child who fails to complete their tasks. This can be done in a very sensitive and calming manner.
Metacognitive Therapy
Metacognitive therapy is a newer kind of psychotherapy that helps people change their mindset. This type of treatment is effective for those suffering from a range of mental health issues which include depression. It's a type of cognitive behavioral therapy that assists people to identify their negative thoughts and how they impact their emotions. This type of therapy helps them develop psychological tools that are versatile and useful.
The purpose of this study was to examine the relationship between metacognitive capabilities and daily functioning in adults suffering from ADHD. In order to do this we compared self and informant-rated impairments in daily functioning with predictors of Metacognition (comprehensive knowledge, regulation of Cognition, Debugging Information Management and Evaluation). The results showed that complete knowledge was associated with daily functioning among adults with ADHD but not with regulation processes. The predictors of metacognition had moderate predictive power, which suggests they could explain a significant amount of variance in daily functioning of adults with ADHD.
Metacognitive therapy (MCT) is a form of psychotherapy that focuses on changing dysfunctional metacognitive beliefs that perpetuate states of anxiety and rumination, as well as attention fixation. MCT was created by Adrian Wells based on an information processing model created by Wells and Gerald Matthews. MCT has been proven to be effective in treating anxiety disorder. However, it's still an in-development treatment, and further research is required to determine if it could help with other disorders.
This study was created to determine the efficacy of MCT in the reduction of symptoms of illness anxiety disorder, however it can be applied to a range of disorders and may even be more effective than other treatments. It is not a cure, but can be utilized to treat ADHD.
Family Therapy
In family therapy, the therapist collaborates with all members of the family. All members can better understand the client's needs and learn how to help their loved ones. It also helps them find ways to manage their own frustrations and improve their communication skills. Additionally, this type of therapy is helpful in dealing with comorbid disorders such as depression or addiction to drugs, which are typically found in conjunction with ADHD.
The therapist will commence with an intake session to gain a clearer understanding of the family's view of the problem and assess their potential for intervention by the family. The session can be conducted in person or via the internet. The Therapist will discuss the nature of the therapy with each client and lay out the do's and don'ts of family therapy.
A therapist who has experience in treating adults suffering from ADHD will ask a variety of questions in order to gain understanding of the client's current symptoms, their history, and their impact on their work as well as their family and social life. They will also inquire whether any other mental health problems are present, for example depression or anxiety.
Cognitive behavioral therapy (CBT) is an organized form of counseling that helps you change your negative thinking patterns and manage your behavior. It is focused on improving your self esteem and self-control through teaching strategies to manage anger and stress. This is a temporary option that can be used in conjunction with other types psychotherapy.
Individual talk therapy can help people with ADHD overcome emotional trauma from a long-running pattern of underachievement, failure academic struggles, frequent job changes and conflict in relationships. It can help you learn stress reduction and coping techniques and also how to manage your financial and time management. It can be used to treat comorbid conditions, such as mood and anxiety disorders, which are often seen alongside ADHD.
