We tested the hypothesis that patient-centered, streamlined human immunodeficiency virus (HIV) care would achieve lower mortality than the standard treatment model for persons with HIV and CD4 ≤ 350/uL in the setting of population-wide HIV testing.

In the SEARCH (Sustainable East Africa Research in Community Health) Study (NCT01864603), 32 communities in rural Uganda and Kenya were randomized to country-guided antiretroviral therapy (ART) versus streamlined ART care that included rapid ART start, visit spacing, flexible clinic hours, and welcoming environment. We assessed persons with HIV and CD4 ≤ 350/uL, ART eligible in both arms, and estimated the effect of streamlined care on ART initiation and mortality at 3 years. Comparisons between study arms used a cluster-level analysis with survival estimates from Kaplan-Meier; estimates of ART start among ART-naive persons treated death as a competing risk.

The loss of response to infliximab is a challenge for clinicians in the management of inflammatory bowel disease (IBD). Mounting evidence suggests that therapeutic drug monitoring at induction may predict remission during maintenance. The aim of the study was to improve predictive models of remission by exploring new peak and intermediate infliximab measurements during induction.

This was a prospective multicenter study evaluating the pharmacokinetics of infliximab during induction in a pioneer cohort of 63 patients with IBD. Pharmacokinetics data including peak, intermediate, and trough levels were combined with clinical and biological parameters and were subsequently fed into tailored logistic regression and tree-based techniques to predict remission at week 30.

Infliximab peak levels at week 2, intermediate levels at week 3, and trough levels at week 6 were correlated with remission at week 30. Predictive models exhibited an increased accuracy over the successive timepoints of the induction with key inputs such as albumin, C-reactive protein, eosinophils, neutrophils, lymphocytes, intermediate level at week 3, trough level at week 6, and age at diagnosis. Our predictive model of remission at week 30 was obtained with an area under the receiver operating characteristic curve of 0.9 ± 0.12, a sensitivity of 89%, and a specificity of 75%.

This study showed the clinical relevance of measuring new infliximab levels to predict remission in patients with IBD. These findings lay the foundation for a personalized medicine in which biotherapies could be monitored at an early stage, thereby improving patients' clinical management.

When supplied in a beneficial range of 1-5 mM, Cl - increases leaf cell size, improves leaf osmoregulation and reduces water consumption without impairing photosynthetic efficiency, resulting in overall higher WUE. Thus, adequate management of Cl - nutrition arises as a potential strategy to increase the ability of plants to withstand drought stress. To study the relationship between Cl - nutrition and drought resistance, tobacco plants treated with 0.5-5 mM Cl - salts were subjected to sustained water deficit (WD; 60% field capacity) and water deprivation/rehydration treatments, in comparison with plants treated with equivalent concentrations of nitrate, sulphate and phosphate salts. Results showed that Cl - application reduced stress symptoms and improved plant growth during WD. Drought resistance promoted by Cl - nutrition resulted from the simultaneous occurrence of WD avoidance and tolerance mechanisms, which improved leaf turgor, water balance, photosynthesis performance and WUE. Thus, it is proposed that beneficial Cl - levels increase the ability of crops to withstand drought stress, promoting a more sustainable and resilient agriculture.Improving drought tolerance in wheat is crucial for maintaining productivity and food security. Deep roots, optimal root length density and xylem diameter, and increased root surface area are root traits contributing to the drought tolerance. Given the diverse environments where wheat is grown, the balance among root and shoot traits, as well as individual and population performance are discussed. The known functions of key genes provide information for the genetic dissection of root development of wheat in multiple conditions, and will be beneficial for molecular marker development, marker-assisted selection and genetic improvement in drought tolerance breeding.
