Background Fracture-dislocations of all four ulnar (second to fifth) carpometacarpal (CMC) joints are rare hand injuries and frequently overlooked or missed. These injuries can be treated conservatively when closed reduction is successfully achieved, though they are sometimes irreducible and unstable. Case Description We report the case of a 17-year-old boy involved in a vehicular accident. Clinical images showed dorsal dislocation of all four ulnar CMC joints of the left hand associated with a fracture of the base of the fourth metacarpal. Although closed reduction was attempted immediately, the affected joints remained unstable and easily redislocated. Therefore, we performed open reduction and percutaneous fixation of all ulnar CMCs. He showed excellent recovery after 1 year postoperatively, reported no pain, and demonstrated complete grip strength and range of motion of the affected wrist and fingers. Literature Review Accurate clinical diagnosis of this lesion is difficult because of polytrauma, severe swelling masking the dislocated CMC joint deformity, and overlapping of adjacent metacarpals and carpal bones on radiographic examination. As for the treatment strategy, it has yet to obtain a consensus. Some reports value open reduction to guarantee anatomical reduction, and it is definitely needed in the patients with interposed tissues to be removed or with subacute and chronic injuries. Clinical Relevance Delayed diagnosis or treatment could lead to poor outcomes. Therefore, surgeons must be aware that precise preoperative assessment is critical, and anatomical open reduction of interposed bony fragments, like our case, may be required even in an acute phase.Background Giant cell tumor (GCT) of bone is a benign, aggressive tumor of bone and occurs commonly around the knee and distal radius. GCT of bone rarely occurs in hand. In hand, metacarpals are the most common site and there have been only a few case reports of GCT of the carpal bone. In hand, these tumors are very aggressive and these have a high tendency to recur after intralesional curettage. No long-term study is available for choosing an appropriate method of treatment for these tumors in hand. Case Description Herein, we report a case of GCT of the bone involving triquetrum which was managed by excision of triquetrum and scaphoid bone. The reconstruction of the wrist was done through limited carpal fusion, by doing three corner fusion of the wrist. At 2 years after surgery, the patient was asymptomatic and radiographs revealed fusion of lunocapitate and lunohamate joints with no evidence of recurrence. Literature review Current literature regarding GCT of triquetrum includes case reports that are reviewed in this report. Clinical Relevance This case illustrates the successful treatment of GCT of triquetrum, which is a rare tumor. Three-corner fusion is a reliable and reproducible procedure that has been used for other arthritic conditions. The use of three corner fusion procedure can be extended to GCT of carpal bones. It has been a motion-preserving alternative to proximal row carpectomy.Background Corrective osteotomies of the proximal phalanx are typically stabilized with plate and screws. Although intramedullary headless screws form an alternative fixation method in the treatment of acute phalangeal fractures, reports about fixation of opening wedge corrective osteotomies with these implants are lacking. Objective The goal of the present study was to biomechanically compare the failure force of both fixation methods for this specific indication. Methods Twenty-four cadaver phalanges were equally distributed between apex volar and apex lateral opening wedge osteotomy groups. In each group, half of the osteotomies were fixed with a 1.3-mm dorsal locking plate, the other half with a 2.4-mm intramedullary headless screw. A three-point bending test was performed. Results The mean maximal failure force after apex lateral osteotomy was 178.4 N for the plate-screw construct and 144.0 N after intramedullary headless screw fixation. After apex volar osteotomy, mean maximal force was 237.6 N in the plate-screw group and 160.9 N in the intramedullary headless screw group. Mean stiffness after apex lateral osteotomy was 63.3 N/mm in the plate-screw group, and 55.9 N/mm in the intramedullary headless screw group. Mean stiffness after apex volar osteotomy was 197.5 N/mm and 60.0 N/mm for the plate-screw and intramedullary headless screw group, respectively. click here Conclusion For apex volar osteotomies, dorsally applied angular stable plate and screws provide significantly stronger fixation than intramedullary headless screws. For apex lateral osteotomies, fixation force is comparable. Clinical relevance These data are useful when considering fixation of opening wedge osteotomies with intramedullary screws.Background Following surgery, the standard regimen for fractures of the distal forearm includes radiographs taken 2-weeks postoperatively. However, it is unclear whether these radiographs have any therapeutic risks or benefits for patients. Objective The purpose of this study is to determine the importance of radiographs taken 2-weeks after surgery on distal forearm fractures, especially if it leads to further operations, and to establish whether this practice should be continued. Materials and Methods This is a retrospective cohort study of patients with a distal forearm fracture treated surgically with a volar locking plate at two university hospitals in Denmark. Standard aftercare at both departments is 2 weeks in a cast. Patients attend a 2-week follow-up, at which the cast is replaced with a removable orthosis and radiographs are taken. It was recorded whether these radiographs had resulted in any change of treatment in terms of further operations, prolonged immobilization, additional clinical follow-up, or additional diagnostic imaging. Results A total of 613 patients were included in the study. The radiographs led to a change of standard treatment for 3.1% of the patients. A second operation was required by 1.0%; 0.5% were treated with prolonged immobilization, and 1.6% had additional outpatient follow-up due to the findings on the radiographs. Additional diagnostic imaging was performed on 1.9% of the patients. Conclusion The radiographs taken at the 2-weeks follow-up resulted in a change of treatment in 3.1% of the cases. Given the low cost and minimal risk of radiographs of an extremity, we concluded that the benefits outweigh the costs of routine radiographs taken 2 weeks after surgical treatment of distal forearm fractures.
