CONCLUSION Urolift® implants improved significantly the lower urinary tract symptoms in our population, with a good tolerance profile. More than 60% of the patients did not need an additional treatment after 5years of follow-up. LEVEL OF EVIDENCE 3. AIMS To study the incidence of type 1 diabetes (T1D) in children less then 14 years in the island of Gran Canaria (Canary Islands, Spain) during the 2006-2018 period and to evaluate its temporal trend, seasonality, age and sex distribution. SUBJECTS AND METHODS We studied children less then 14 years of age living in Gran Canaria. We calculated the annual and overall incidence using recorded data from the Pediatric Endocrinology Department as the primary source and the local Diabetes Association and the hospital's pharmacy as secondary sources. The primary source is the only paediatric endocrine unit in the island. RESULTS 453 new T1D cases were observed during the 13-year period. The overall incidence of T1D between 2006 and 2018 was 30.48/100,000 (95% CI 27.74-33.42). Distribution among age groups was 24.8%, 38.2% and 36.9% for children between 0-4, 5-9 and 10-13.9 years old respectively. No significant temporal trend, seasonality or sex differences were found. CONCLUSIONS Our study shows that the Island of Gran Canaria has one of the highest childhood incidences of T1D reported worldwide among the highest rates in Europe, and higher than the rates published for the neighbouring African countries. BACKGROUND AND OBJECTIVES Mortality of patients with pulmonary tuberculosis (TB) admitted to emergency departments is high. This study was aimed at analysing the risk factors associated with early mortality and designing a risk score based on simple parameters. METHODS This prospective case-control study enrolled patients admitted to the emergency department of a referral TB hospital. Clinical, radiological, biochemical and microbiological risk factors associated with death were compared among patients dying within one week from admission (cases) and those surviving (controls). RESULTS Forty-nine of 250 patients (19.6%) experienced early mortality. Multiple logistic regression analysis showed that oxygen saturation (SaO2) ≤90%, severe malnutrition, tachypnoea, tachycardia, hypotension, advanced disease at chest radiography, severe anaemia, hyponatremia, hypoproteinemia and hypercapnia were independently and significantly associated with early mortality. A clinical scoring system was further designed to stratify the risk of death by selecting five simple parameters (SpO2 ≤ 90%, tachypnoea, hypotension, advanced disease at chest radiography and tachycardia). This model predicted early mortality with a positive predictive value of 94.88% and a negative predictive value of 19.90%. CONCLUSIONS The scoring system based on simple parameters may help to refer severely ill patients early to a higher level to reduce mortality, improve success rates, minimise the need for pulmonary rehabilitation and prevent post-treatment sequelae. BACKGROUND Several factors besides appointment availability can influence access to care. Among these factors are the diverse challenges that patients may experience in navigating the healthcare system. However, the relationship between these challenges or "hassles" and delaying or forgoing care has not been assessed. METHODS We examined the relationship between healthcare system hassles and delaying or forgoing needed care. We used data from a 2016 Veterans Affairs (VA) survey of women veterans (N = 821) who were active users of primary care (3+ primary care visits in the past year) at any of 12 VA medical centers. The main independent variable was a measure of 16 healthcare system hassles, encompassing a wide range of clinically-relevant aspects of patient experience, such as uncertainty about when/how to take a medication or difficulty getting questions answered between appointments. The outcome was a self-reported measure of delaying or forgoing needed care. We used logistic regression to estimate this outcome as a function of hassles, adjusting for age, comorbidities, and health care utilization. Survey weights accounted for within-site clustering, nonproportional sampling, and nonresponse. RESULTS Overall, 26% of participants reported 0 hassles, and 39% reported 4 or more. Reporting 4 or more hassles (vs. 0) was associated with a roughly 5-fold increase in the predicted probability of delaying or forgoing care. CONCLUSION Addressing healthcare system hassles could yield unexpected benefits to realized access. Published by Elsevier Inc.PURPOSE Condylar head fractures (CHFs) are increasingly treated by open reduction and internal fixation (ORIF). However, there are no reports on the three-dimensional postoperative volumetric transformation of the condyle, especially with regard to fragmented cases. Protruding hardware can lead to severe complications, so the goal of this study was to examine the amount of condylar bony resorption occurring after ORIF. METHODS Included were surgically treated CHFs with eligible cone beam computer tomography (CBCT) datasets immediately after ORIF (T1) and after implant removal (T2), plus fractures of the condylar neck and base as a reference. 2D vertical and 3D volume changes of the condylar head after ORIF of CHFs were evaluated by CBCT datasets transformed into 3D models for 3D volumetric assessment using Slicer freeware. AZD9291 clinical trial RESULTS Among a total of 50 fractures (38 patients), including 41 CHFs (ORIF with titanium positional screws, including 15 minor and 12 major fragmented cases) plus nine extracapsular fractures (eight upper neck and one base fracture), postoperative condylar volume decreased by a mean of 0.27 cm³ (median 0.25 cm³; SD 0.23 cm³) or 16% (median 14%; SD 11%). Major fragmented CHFs showed significantly higher resorption rates (p less then 0.001, range 8-42%). Age correlated with a decrease in condylar volume (mean 2.4% per 10 years of age, (p = 0.011). No significant correlation could be established between loss of condylar volume and fracture localization. A protrusion of metallic implants was seen in 20% of the assessed cases. CONCLUSION Due to substantial volume changes of the condylar head occurring after ORIF, protrusion of implants (both metallic and resorbable) needs to be taken into consideration. An early removal of metallic ostheosyntesis material (around 4 months after ORIF) is strongly recommended to avoid adverse short- and long-term effects.
