Recent results from the HARVEST study confirm the existence of an association between ISHY and endurance training. In that study the prevalence of ISHY was directly related to physical activity habits being 8.8% in the sedentary subjects and 25.7% in the athletes. Eligibility to competitive athletics of athletes with ISHY should be based on the results of 24-hour ambulatory BP monitoring and careful clinical assessment of all cardiovascular risk factors and target organ involvement in order to establish the global level of risk.
Pneumomediastinum (PNM) can develop as a severe complication of severe COVID 19 and may be correlated with greater morbidity and mortality. PNM is a rarely reported complication in COVID-19 patients and usually associated with endotracheal intubation.

Our aim is to describe the characteristics of patients with PNM in twenty-one patients with COVID-19 related pneumonia and acute respiratory failure in a retrospective case series.

Twenty-one patients were diagnosed, two were treated with high flow nasal cannula, five with non invasive ventilation and four with invasive mechanical ventilation. In four cases PNM was massive PNM was often associated to subcuatneous emphysema; more rarely associated also with pneumothorax. Conservative management was the most used therapeutic strategy.

Aim of the present multimodal neuroimaging study was to examine the associations of the TSPO polymorphism with I) conditioned pain modulation, II) expectancy-modulated pain processing assessed during functional magnetic resonance imaging, and III) the concentration and balance of glutamate and γ-aminobutyric acid (GABA) in the rostral anterior cingulate cortex (rACC) and thalamus using proton magnetic resonance spectroscopy in FMS (n = 83) and healthy controls (HC, n = 43). The influence of TSPO on endogenous pain modulation presented in the form of TSPO HABs, as opposed to MLABs, displaying less efficient descending pain inhibition and expectancy-induced reduction of pain. TSPO HABs in both groups (FM and HC) were found to have higher thalamic glutamate concentrations and exhibit a pattern of positive correlations between glutamate and GABA in the rACC, not seen in MLABs. Altogether, our findings point to TSPO-related mechanisms being HAB-dependent, brain region-specific, and non-FM-specific, although in FMS the disadvantage of an aberrant pain regulation combined with a HAB genetic set-up might hamper pain modulation more strongly. Our results provide evidence for an important role of TSPO in pain regulation and brain metabolism, thereby supporting the ongoing drug development targeting TSPO-associated mechanisms for pain relief.
Healthcare systems have been experiencing profound changes in recent years driven by technology, regulations, demographic and societal trends. This volatile and stressful environment has had its consequences for healthcare organizations and people who work for them. The resulting phenomena of burnout or lowered well-being may affect key aspects of health care delivery on individual, organizational and financial level.

The objective of this paper was to investigate level of burnout and well-being in a group of Polish physicians, its impact on errors and associations with other factors.

The cross-sectional, self-administered questionnaire survey with 65 questions and validated tools such as Maslach Burnout Inventory and WHO-5 Well-being Index. Correlations between variables were investigated using Spearman's rank correlation analysis. Univariable and multivariable logistic regression models were used to evaluate predictors of burnout and errors. The study included residents and physicians of various specialties.

The total number of 125 residents and physicians, mostly internists and cardiologists, participated in the study out of which 67% were found to be burned-out. The average level of WHO-5 score was 40 points. Two thirds of all physicians admitted to making an error in the last three months which was strongly associated with burnout.

The level of burnout among respondents was high and had numerous negative consequences that may affect the entire healthcare system. The results suggest an urgent need to address the problem of burnout among physicians.

We report a case of a primary epigastric hernia containing a falciform ligament repaired using nonabsorbable barbed suture under laparoscopic surgery. A 74-year-old woman presented with discomfort in the upper abdomen. Physical examination revealed a 3-cm non-tender mass, and abdominal computed tomography revealed epigastric hernia with an incarcerated falciform ligament. By observation with a laparoscope, the fascial defect was single and 1.0 cm in diameter, thus sutured repair was selected. At 4 months follow-up, she had no complications or recurrence. The laparoscopic approach is useful for diagnosis and deciding a repair method of an epigastric hernia. The barbed suture closure system is convenient and effective for intracorporeal hernial defect closing. Both primary and incisional falciform ligament herniation have been reported in the past, and we reviewed six cases, including our case.
