This review describes emerging innovations in offline and online MRI-guided RT, exciting opportunities they offer for advancing research and clinical care, hurdles to be overcome, and the need for multidisciplinary collaboration. Background Data are limited regarding comparison between nonspherical polyvinyl alcohol (PVA) particles and tris-acryl gelatin microspheres (TAGM) in uterine artery embolization (UAE). Purpose To compare pain after UAE with PVA versus TAGM for treatment of symptomatic fibroids. Materials and Methods In this randomized clinical trial, participants were assigned to be administered nonspherical PVA (355-550 μm) or TAGM (500-700 μm). Both groups were administered fentanyl-based intravenous patient-controlled analgesia during the first 24 hours after UAE and rescue analgesics. Neutrophil-to-lymphocyte ratio was measured to assess inflammatory response. Contrast-enhanced MRI 1 day after UAE was used to evaluate dominant fibroid necrosis and ischemia of normal myometrium. Symptom severity score and health-related quality-of-life score were assessed before and 3 months after UAE. Variables measured over time were analyzed by using the generalized estimating equation method. Results A total of 54 participants (mean agusion When used in uterine artery embolization, polyvinyl alcohol particles and tris-acryl gelatin microspheres resulted in similar pain scores and fentanyl dose. Polyvinyl alcohol resulted in a greater inflammatory response, higher rates of rescue analgesic use, and more frequent transient global uterine ischemia. © RSNA, 2020 See also the editorial by Spies and Frenk in this issue. Background Lesions of cerebral small vessel disease, such as white matter hyperintensities (WMHs) in individuals with cardiometabolic risk factors, interfere with the trajectories of the white matter and eventually contribute to cognitive decline. However, there is no consensus yet about the precise underlying topological mechanism. Purpose To examine whether WMH and cognitive function are associated and whether any such association is mediated or explained by structural connectivity measures in an adult population. In addition, to investigate underlying local abnormalities in white matter by assessing the tract-specific WMH volumes and their tract-specific association with cognitive function. Materials and Methods In the prospective type 2 diabetes-enriched population-based Maastricht Study, structural and diffusion-tensor MRI was performed (December 2013 to February 2017). Total and tract-specific WMH volumes; network measures; cognition scores; and demographic, cardiovascular, and lifestyle characteristicsMH load was larger in the white matter tracts important for information processing speed. Conclusion White matter hyperintensity volume, local network efficiency, and information processing speed scores are interrelated, and local network properties explain lower cognitive performance due to white matter network alterations. © RSNA, 2020 Online supplemental material is available for this article. Background Evaluation of structural lesions in the sacroiliac (SI) joints can improve the accuracy for diagnosis of spondyloarthritis. However, structural lesions, such as erosions, are difficult to assess on routine T1-weighted MRI scans. Purpose To determine the diagnostic performance of MRI-based synthetic CT (sCT) in the depiction of erosions, sclerosis, and ankylosis of the SI joints compared with T1-weighted MRI, with CT as the reference standard. Materials and Methods A prospective study (clinical trial registration no. B670201837885) was performed from February 2019 to November 2019. Adults were referred from a tertiary hospital rheumatology outpatient clinic with clinical suspicion of inflammatory sacroiliitis. MRI and CT of the SI joints were performed on the same day. SCT images were generated from MRI scans using a commercially available deep learning-based image synthesis method. Two readers independently recorded if structural lesions (erosions, sclerosis, and ankylosis) were present on T1-weighlesions in individuals suspected of having sacroiliitis compared with routine T1-weighted MRI. © RSNA, 2020 Online supplemental material is available for this article. Digital breast tomosynthesis (DBT) is used for screening, but data on DBT for surveillance in this high-risk population are limited. Purpose To determine whether DBT leads to improved screening performance metrics when compared with two-dimensional digital mammography among breast cancer survivors. Materials and Methods In this study, screening mammograms obtained in breast cancer survivors before and after DBT implementation were retrospectively reviewed (March 2008-February 2011 for the digital mammography group; January 2013-December 2017 for the DBT group). Mammograms were interpreted by breast imaging radiologists with the assistance of computer-aided detection. Performance metrics and tumor characteristics between the groups were compared using multivariable logistic regression models. Results The digital mammo Among breast cancer survivors, screening with digital breast tomosynthesis led to fewer false-positive results and higher specificity but did not affect cancer detection. © RSNA, 2020 Online supplemental material is available for this article. read more See also the editorial by Hooley and Butler in this issue.Moore, G.W.K., Paolo Cristofanelli, Paolo Bonasoni, Gian Pietro Verza, and J.L. Semple. Was an avalanche swarm responsible for the devastation at Mount Everest Base Camp during the April 2015 Nepal earthquake? High Alt Med Biol. 21352-359, 2020. Introduction An avalanche triggered by an earthquake on April 25, 2015, struck the Mount Everest Base Camp (EBC) resulting in 15 deaths and over 70 injuries. Despite the common occurrence of avalanches in this region, little is known about their intensity and the stability of the glaciers that ring the Mount Everest massif. Here we present unique observations from a nearby automatic weather station (AWS) in the minutes just after the earthquake. Methods Several (AWS) were deployed along the Khumbu Valley in Nepal. The site at Kala Patthar (elevation 5,613 m asl) 3.5 km from EBC and 4 km from the col along the ridge between Pumori and Lingtren was active from 2010 to 2015 and recorded temperature, relative humidity, pressure, solar radiation, and wind speed and direction.
