Effective and safe behavioral health interventions in primary care are critical during pandemic and other disaster situations. California shelter-in-place orders necessitated rapid transition of an effective mindfulness-based medical group visit (MGV) program from in-person to videoconferenced sessions (VCSs). Aim to Describe procedures, acceptability, and feasibility of converting from in-person to VCS.
Methods qualitative. Dataset primary care. Intervention a six-session 2-h MGV program with educational and mindfulness components was converted. Four in-person sessions and two VCSs were held. General Anxiety Disorder and Patient Health Questionnaire-9 were administered at first and last sessions. A semi-structured focus group was conducted after session six. Population studied six primary care patients (42 ± 11 years) with stress, anxiety, or depression participated.
Procedural changes included remote material distribution, scheduling, hosting, and facilitation functions using the Zoom platform. The focceptable during COVID-19. Future iterations of this program are proposed which incorporate procedural changes and facilitate relationship building between patients in VCS.
We aim to characterize the incidence and relative risk of rheumatic and systemic immune-related adverse effects (irAEs) among immune checkpoint inhibitor (ICI) therapy compared with those after placebo treatment.
Randomized clinical trial studies with placebo control with the following keywords were searched from Embase, PubMed, Cochrane databases immune checkpoint inhibitors, neoplasms, randomized controlled trials, and adverse effects.
Among the 5444 published and 316 registration records, nine placebo-controlled randomized clinical trials met our selection criteria, and included data from 5560 patients. Compared with placebo use, using ICIs increases the risk of overall-rheumatic irAEs. The incidence and relative risk of all-grade rheumatic irAEs were 18.40% [95% confidence interval (CI) 12.16-25.59%,
< 0.01] and 2.30 (95% CI 1.32-4.02), respectively, while musculoskeletal irAEs were 11.30% (95% CI 9.76-12.85%) and 1.01 (95% CI 0.84-1.22). The incidence and relative risk of severe rheumatic irA irAE, while colitis and pneumonitis were commonly observed severe irAEs. Rare rheumatic irAEs like hepatitis, hypophysitis, thyroiditis, and myositis warrant special attention.
ICI therapy increased the incidence and relative risk of all-grade and severe rheumatic irAEs. Arthralgia was the most commonly observed non-severe irAE, while colitis and pneumonitis were commonly observed severe irAEs. Rare rheumatic irAEs like hepatitis, hypophysitis, thyroiditis, and myositis warrant special attention. This study was performed to determine whether Autologous Matrix-Induced Chondrogenesis (AMIC) is an effective and safe treatment option for patients with symptomatic Osteochondral defects of the Talus (OCTs) and to identify factors that influence the clinical outcome. A systematic review of the literature was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Three reviewers independently conducted the literature search using the MEDLINE/PubMed database and the Cochrane Database of Systematic Reviews. The databases were queried using the terms "autologous" AND "matrix" AND "induced" AND "chondrogenesis." Thirteen studies were eligible for review. All studies that compared the preoperative and postoperative mean values of different clinical/functional scores showed significant clinical improvement. The final postoperative mean Magnetic Resonance Observation of Cartilage Repair Tissue (MOCART) score ranged from 50.9 to 74.5. The included studies indicated that age and body mass index may have a detrimental impact on the postoperative outcome. A higher re-intervention rate is expected with the open technique, mainly because of hardware removal after malleolar osteotomy. This data analysis demonstrated that both arthroscopic and open AMIC procedures are effective and safe for the treatment of OCTs. Level IV, systematic review of therapeutic studies.Vohwinkel Syndrome, also known as Keratoderma Hereditarium Mutilans, is an extremely rare dominant autosomal keratosis. It typically presents with "starfish" keratoses on the knuckles, palmoplantar keratoderma (PPK), hearing impairment and mutilating digital constriction bands (pseudoainhum) that cause strangulation, often leading to autoamputation of the affected digit. Both medical and surgical treatment haven't shown to date consistent results, in the treatment of pseudoainhum. In this study we present the case of a woman with Vohwinkel syndrome who showed constriction bands causing ischemic changes of the 5th digit of the right hand for which she was treated with surgery. We also present a review of the literature for the management of this disease. The main source of energy in the world is fossil fuels, but these can irreparably harm the environment. My research focuses on making a catalyst that is economical, efficient, and environmentally friendly. The oxygen reduction reaction is an important reaction for energy conversion systems, such as fuel cells. Onion-like carbon nanoparticles are used as a catalytic support for fuel cell applications due to its high conductivity and surface-to-volume ratio. The combination of carbon compounds with conductive polymers results in new materials and devices with possible practical applications. check details In my research we obtain onion-like carbon from nanodiamonds through a pyrolysis process. These particles are then polymerized using the aniline monomer to dope the OLCs with nitrogen. These particles are characterized by different characterization techniques such as Raman and Fourier transform infrared spectroscopy and scanning electron microscopy. After characterizing them, electrochemical experiments were carried out.As a metal (gold) surface at a given, but variable potential slides past a dielectric (mica) surface at a fixed charge, across aqueous salt solutions, two distinct dissipation regimes may be identified. In regime I, when the gold potential is such that counterions are expelled from between the surfaces, which then come to adhesive contact, the frictional dissipation is high, with coefficient of friction μ ≈ 0.8-0.9. In regime II, when hydrated counterions are trapped between the compressed surfaces, hydration lubrication is active and friction is much lower, μ = 0.05 ± 0.03. Moreover, the dissipation regime as the surfaces contact is largely retained even when the metal potential changes to the other regime, attributed to the slow kinetics of counterion expulsion from or penetration into the subnanometer intersurface gap. Our results indicate how frictional dissipation between such a conducting/nonconducting couple may be modulated by the potential applied to the metal.
