A concerning number of students enter kindergarten facing an intractable variation of mathematics difficulties (MD). Protein Tyrosine Kinase inhibitor This study investigated the impact of an explicit, core kindergarten mathematics program on the mathematical outcomes of kindergartners who demonstrated risk for severe MD at kindergarten entry and examined whether these students improved from a category of high MD risk (i.e., less then 10th percentile) to a lower risk of MD (i.e., norm-referenced performance at or above the 10th, 20th, 30th, and 40th percentiles) between the fall and spring of kindergarten. Differential response to the program based on the classroom-level proportion of students with severe MD was also explored. A total of 795 kindergarteners with severe MD from 122 classrooms were included in the analyses. Results suggested students with severe MD in treatment classrooms improved from fall to spring at a greater rate than their control peers. Treatment students also demonstrated higher rates of improvement from below the 10th percentile to a performance at or above the 20th, 30th, and 40th percentiles across the school year. No evidence of differential efficacy of the program by the classroom-level proportion of students with severe MD was found. Implications for using explicit mathematics programs to thwart the onset of severe MD among academically vulnerable students are discussed. Background Recently, inflammatory cell ratios have gained importance as useful indicators in the categorization of asthma. Objective We compared the concentration of white blood cells in peripheral blood, as well as their respective inflammatory cell ratios, between patients with asthma and a healthy control group. Methods We performed cross-sectional analyses of the data obtained from 53 adult patients with asthma and 109 adult controls. In our study, we estimated and compared the following inflammatory cell ratios Neutrophil-Lymphocyte Ratio (NLR), Eosinophil-Lymphocyte Ratio (ELR), Eosinophil-Neutrophil Ratio (ENR), Eosinophil-Monocyte Ratio (EMR), and Platelet-Lymphocyte Ratio (PLR). The magnitude of association was quantified with the odds ratio. Results In both groups, the average age was 33 years. In asthmatic patients, we obtained the following results eosinophils ≥ 400 cells/µl, accounted for 37.7%; basophils ≥ 110 cells/µl, comprised 37.7%; and monocytes less then 320 cells/µl, reached 11.3%. In the control group, the results were as follows 4.6%, 9.2% and 0.9%, respectively. When compared to the control group, asthmatic patients had higher odds of eosinophils ≥ 400 cells/µl (OR = 12.61, p less then 0.0001); higher odds of basophils ≥ 110 cells/µl (OR = 6.00, p less then 0.0001); and increased odds of monocytes less then 320 cells/µl (OR = 13.79, p = 0.017). NLR did not differ between our two groups; however, ELR, ENR, EMR and PLR were significantly higher in the asthma group. Conclusions Overall, patients with asthma have a higher concentration of eosinophils and basophils, fewer monocytes in their blood, and higher ratios of increased chronic inflammation.
To compare the pregnancy outcome of women who underwent cesarean section in the second stage of labor, with or without a vacuum extraction attempt.
A retrospective cohort study of women who underwent a cesarean section during the second stage of labor in a single tertiary university-affiliated medical center (2012-2019). Pregnancy outcome was compared for women who underwent cesarean section following a failed vacuum extraction to women who had cesarean section during the second stage of labor with no vacuum extraction attempt. Neonatal outcomes included umbilical artery pH less than 7.1, Apgar at 5 min < 7, hypoxemic ischemic encephalopathy and NICU admission. Maternal outcomes included duration of hospitalization, need for blood transfusion and need for re-surgery in 45 days.
Overall, 88,375 women delivered during the study period. Of them, 120 women had a cesarean section following a failed vacuum (study group). Another 551 women underwent a cesarean section in the second stage of labor without a VE attempt (control group). The groups were similar with regard to obstetrical and demographic characteristics. The rates of umbilical artery pH < 7.1 (17.50% vs 6.53%,
< .001), NICU admission (13.33% vs 2.90%,
< .001), hypoxemic ischemic encephalopathy (5.83% vs 0.18%,
< .001) and epicranial sub-aponeurotic hemorrhage (16.67% vs 2.18%,
< .001) were significantly higher in the study group. No significant differences were found in maternal outcomes. In a sub-analysis including only labor with reassuring fetal heart tracing, failed vacuum attempt was associated with higher rate of NICU admission and epicranial hemorrhage (16.67% vs 3.13%,
= .009, 27.78% vs. 3.41,
= .001, respectively).
Failed vacuum attempt is associated with a significant increased neonatal morbidity, but not increased maternal morbidity.
Failed vacuum attempt is associated with a significant increased neonatal morbidity, but not increased maternal morbidity.Introduction Antimicrobial resistance is a worldwide problem accounting for the reduction or in some cases absence of drugs effectiveness normally used in infections treatment. In the light of the even more spread ability of microbials to develop resistance, there is an urgent necessity to find novel and alternative routes to fight infections. Natural compounds or extracts can be a valid alternative either as monotherapy or as adjuvant in order to improve the effectiveness of the failing drugs. Areas covered This review provides a comprehensive update (2018-2020) on the development state of innovative antimicrobial agents based on natural compounds and extracts, also describing their compositions, methods of production and use, mechanism of action, along with anti-microbial data when available. Expert opinion Owing to the pivotal role that natural compounds often cover in the finding of novel drugs, their in-depth analysis could pave the way to the discovery of new antimicrobial agents. Most of the alternative approaches reported in this short review were validated through in vitro and in vivo (animal as well as human) models.
