We aim to assess the long-term oncological outcomes of children with bladder/prostate rhabdomyosarcoma (B/P RMS) treated with multiagent chemotherapy as a monotherapy. We hypothesize that a highly select patient subset can be treated with multiagent chemotherapy as a monotherapy and spared the morbidity of local treatment with similar oncological outcomes.
Patients (≤21-year-old) treated for non-metastatic non-alveolar B/P RMS at a tertiary center and followed for>one year, were retrospectively reviewed. After pathological confirmation, patients received 12 weeks of induction VAC chemotherapy (IC) followed by second-look biopsies. Between 1996 and 2006 (group A), patients with>50% tumor size reduction and negative second-look biopsies following IC were spared local treatment and followed-up closely. Between 2007 and 2020 (group B), local treatment was routinely given at 12 weeks according to the COG protocols, irrespective of IC response. For all patients, consolidation chemotherapy was administered treatment of B/P RMS is associated with significant patient morbidity. In this study, CR to IC predicted better OS and EFS. Patients who achieved CR (radiological and pathological) to IC remained alive free of relapse irrespective of local treatment.
Our results suggest that patients with non-metastatic non-alveolar B/P RMS who achieve CR to IC can be treated with combination chemotherapy as a monotherapy and spared the morbidity of local treatment with durable survival outcomes. Perhaps the most overt output of the circadian clock is the sleep-wake cycle, the integrity of which is critical for health and homeostasis of the organism. In this review, we summarize our current understanding of the circadian regulation of sleep. We discuss the neural circuitry and molecular mechanisms underlying daily sleep timing, and the trajectory of circadian regulation of sleep across development. Total work completed during 1 h of treadmill walking at a fixed heart rate of 130 b∙min
was assessed in 19 young adult males across a variety of warm and hot climate types, characterised by wet-bulb globe temperatures (WBGT) ranging from 12 to 40 °C. For data presentation and obtaining initial parameter estimates for modelling, participants were grouped into low (n = 6, 74 trials), moderate (n = 8, 76 trials), and high (n = 5, 29 trials) fitness, with group mean V̇O
42, 52, and 64 mL∙kg
∙min
respectively. For the heated conditions (WBGT 18 to 40 °C), we calculated PWC% by expressing total energy expenditure (kJ above resting) in each trial relative to that achieved in a cool reference condition (WBGT = 12 °C = 100% PWC).
The relative reduction in energy expenditure (PWC%) caused by heat was significantly smaller by up to 16% for the fit participants compared to those with lower aerobic capacity. V̇O
also modulated the relationship between sweat rate and body temperature changes to increasing WBGT. Including individual V̇O
data in the PWC prediction model increased the predicting power by 4%.

improved the predictive power of the heat stress index WBGT for Physical Work Capacity in the heat. The largest impact of V̇O

LEC of the larynx/hypopharynx is extremely rare, accounting for only 0.2% of malignant tumors of the larynx. This study describes a case of locally advanced LEC of the larynx/hypopharynx with effective response to chemoradiation (an option that spares the patient from the risks of surgical resection). A 60-year-old sedentary, hypertensive male patient with a previous history of smoking and alcohol abuse received a histopathological diagnosis of locally advanced LEC of the larynx/hypopharynx in May 2018. He underwent treatment with chemoradiation and exhibited a complete clinical response. At about 3 years of follow-up, the patient is currently alive, free from disease and has not presented any tumor recurrences.
