To describe past trends and future projections for the number of intravitreal injections being administered at a large tertiary hospital in London, United Kingdom.
Retrospective data from Moorfields Eye Hospital were collected using the electronic medical record system. Descriptive statistics were used to visualise overall trends. Time series forecasting was used to predict the number of injections that will be administered up to and including the year 2029.
The number of injections has increased nearly 11-fold from 2009 to 2019, with a total of 44,924 injections delivered in 2019. The majority of injections were given for the treatment of neovascular age-related macular degeneration. Aflibercept formed 87% of injections administered in 2019. The number of injections is predicted to continue to increase every year, with nearly 83,000 injections forecasted in the year 2029.
The demand for intravitreal injections has increased substantially over the last decade and is predicted to further increase. Healthcare systems will need to adapt to accommodate the high demand. Other solutions may include longer-acting therapies to reduce the treatment burden.

Oculoplastics is a predominantly visual specialty and many of the pathologies can be diagnosed based on external appearance. An image-based eyelid lesion management service was piloted to reduce the number of patients who would require outpatient clinic review. The aim of this study was to determine its accuracy and feasibility, both as a hospital-based and community optometrist-based service. If successful, the service was envisaged to significantly reduce the number of patients that require face-to-face (F2F) review, in accordance with current post-COVID-19 principles of social distancing.
Patients with lid lesions attending an oculoplastics clinic were assessed by consultant oculoplastic surgeons in an F2F consultation (Arm A). The lesions were photographed by a professional clinical photographer (Arm B) and by an optometrist with a handheld digital camera (Arm C). These images were reviewed by independent consultants masked to the outcome of the F2F clinical encounter. Data were collected prospectively including patient demographics, diagnosis, suspicion of malignancy and management. The image-based clinic results were compared to the F2F clinic results.
Ninety-five patients were included. Clinical diagnoses were compared for intra-observer variability and substantial agreement was demonstrated between gold-standard F2F clinic visit (Arm A) and Arm B (Ƙ = 0.708) and C (Ƙ = 0.776). There was no statistically significant difference in the rate of discharge and all cases of malignancy were either identified or flagged for F2F review in the image-based arms.
This pilot demonstrated substantial diagnostic agreement of image-based diagnoses with F2F consultation and image review alone did not miss any cases of malignancy.

We aimed to evaluate the accuracy, reliability, quality, and content of the most viewed YouTube videos related to Peyronie's disease treatment. The keywords of "penile curvature", "penile deformity", "bent penis" "curved penis", and "Peyronie's disease" were searched on YouTube. Among 700 YouTube videos, 267 videos were included in the study. They were categorized by two independent urologists with board certification as accurate information (n = 138, 51.7%) or inaccurate information (n = 129, 48.3%). Accurate videos contained information about the treatment of Peyronie's disease with proven scientific accuracy according to the current guidelines, whereas inaccurate videos contained scientifically unproven or incorrect information and recommendations not in the guidelines. A 5-point modified DISCERN scale and Global Qfit physicians. The past 50 years have seen a growing ageing population with an increasing prevalence of type 2 diabetes mellitus (T2DM); now, nearly half of all individuals with diabetes mellitus are older adults (aged ≥65 years). Older adults with T2DM present particularly difficult challenges. For example, the accentuated heterogeneity of these patients, the potential presence of multiple comorbidities, the increased susceptibility to hypoglycaemia, the increased dependence on care and the effect of frailty all add to the complexity of managing diabetes mellitus in this age group. In this Review, we offer an update on the key pathophysiological mechanisms associated with T2DM in older people. We then evaluate new evidence relating particularly to the effects of frailty and sarcopenia, the clinical difficulties of age-associated comorbidities, and the implications for existing guidelines and therapeutic options. Our conclusions will focus on the effect of T2DM on an ageing society.Low and high serum uric acid levels are associated with an increased risk of cardiovascular diseases. However, whether there is a similar association between serum uric acid levels and blood pressure abnormalities has not yet been elucidated. ML390 Data from 236,221 individuals (age, 56.0 ± 15.0 years; 107,146 men) who underwent routine health checkups were analyzed. Participants with a serum uric acid level ≤2.0 mg/dL were separated, and the remaining participants were divided into quartiles according to their serum uric acid levels. Among each group, the proportions of participants with high blood pressure (≥130/80 mmHg, and/or medication) and hypertension (≥140/90 mmHg, and/or medication), were calculated. The adjusted odds ratio of each group compared to the group with the lowest proportion of blood pressure abnormalities was calculated. The participants were divided into five groups according to their serum uric acid levels, ≤2.0, 2.1-4.0, 4.1-5.0, 5.1-6.0, and ≥6.1 mg/dL, and the prevalence of high blood pressure was 49.
