Ketamine is commonly used in emergency department procedural sedation. Mild to moderate transient increases in blood pressure, heart rate, and cardiac output are common due to ketamine causing an increase in sympathetic activity. There is a concern that these physiological changes could result in an increased myocardial oxygen demand that may exacerbateunderlying cardiac disease.

Convenience sample of patients older than 50 years receiving ketamine for procedural sedation in the emergency department was used (n = 31).Patients were selected to receive ketamine based on provider discretion.Primary outcome was incidence of new myocardial ischemia apparent on an electrocardiogram (ECG). ECGs were obtained prior to sedation and during the sedation approximately one minute after administration of ketamine.ECGs were reviewed by a board-certified emergency medicine physician and a board-certified cardiologist.

New onset ischemia was found in 9.7% (3/31) of ECGs. Of these, one was in a patient who had previously of ketamine to obtain adequate sedation in order to hopefully lessen the occurrence of ECG changes suggestive of myocardial ischemia. Based on this small sample, single-site study, no evidence of statistically or clinically significant ischemia was seen with the use of ketamine for procedural sedation. Estimating the incidence of infants with congenital heart defects (CHD) whose mothers have diabetes in Saudi Arabia at a tertiary hospital in the National Guard Health Affairs (NGHA) system.

This study was a retrospective cohort. We performed femoral and sciatic nerve blocks for a 55-year-old male patient who underwent BKA and subsequent formalization surgeries as the main surgical analgesia while providing MAC in the operating room. In both cases, the patient could not tolerate surgical incisions in the posteromedial aspect of the lower leg, despite reporting no pain in other areas of the lower leg with surgical stimulation. There may exist a small population of patients in which PFCN makes significant contribution to the sensory innervation of the posterior lower leg. For these patients, the combination of femoral and sciatic nerve blocks may not be adequate in providing surgical analgesia for BKA and related procedures.Aim To describe the clinical characteristics and outcome of hospitalized COVID-19 patients with diabetic ketoacidosis (DKA). Methods We report eight cases of diabetic ketoacidosis in COVID-19 who presented to our institution in New Jersey, USA. COVID-19 was diagnosed by nasopharyngeal swab reverse transcription polymerase chain reaction (RT-PCR). The patients' electronic medical records were reviewed. Data on patients' age, sex, ethnicity, laboratory values, glycosylated hemoglobin level, oral antihyperglycemic agents (OHAs), insulin, and clinical outcomes were collected. Results The median age of the patient was 42.5 years, and seven were males and one was female. Out of eight patients, five had type 2 diabetes mellitus (DM), two had undiagnosed DM, and one had type 1 DM. Median value of initial glucose on presentation was 454 mg/dL. Median value of HbA1c on presentation was 11.4% and of anion gap was 26.5 mEq/L. Four patients had large ketonemia, one patient had moderate ketonemia, and three patients had small ketonemia.
