Monosodium Glutamate (MSG) is the most commonly utilized food additive in the world. However, data on possible biochemical reasons underlying the neurotoxic effects of dietary MSG is limited. Therefore, this study investigated the effects of dietary supplementation of MSG on redox status and neurochemical indices in lobster cockroach nymph. These were evaluated via assessment of enzymatic and nonenzymatic antioxidants, acetylcholinesterase and monoamine oxidase activities, and dopamine content in the cockroach nymph head homogenate. MSG supplemented diet caused dose-dependent significant (p less then .05) reduction in % survival, thiol, GSH, dopamine contents, and GST activity, increased ROS, NO, Fe2+ , MDA contents, and MAO activity but no significant (p less then .05) difference was obtained in GSH and TBARS contents, and AChE activity. Increased oxidative, cholinergic, and monoaminergic activities coupled with decreased dopamine level might be the plausible biochemical explanation for the neurotoxic effects observed during sub-chronic consumption of large amounts of MSG in diet. PRACTICAL APPLICATIONS This study suggests that consumption of monosodium glutamate should be reduced to the barest minimum due to its capability to induce oxidative stress and nervous toxicological effects at high dosage.

The aims of this study were to predict pharmacokinetic/pharmacodynamic target attainment rates with a commonly used vancomycin regimen and to design a new dosing scheme incorporating therapeutic drug monitoring (TDM) to maximize target attainment in patients receiving vancomycin and hemodialysis with high- or low-flux hemodialyzers. Vancomycin pharmacokinetic- and dialysis-specific parameters were incorporated into Monte Carlo simulations (MCS). A commonly used vancomycin regimen was modeled to determine its likelihood of attaining AUC24h /MIC targets for 1 week of thrice-weekly hemodialysis treatments. MCS was then used to develop optimal initial vancomycin dosing for patients receiving intradialytic or postdialytic vancomycin administration with either high- or low-flux hemodialyzers. Finally, a new MCS model incorporating TDM was built to further optimize the probability of pharmacokinetic/pharmacodynamic target attainment. Traditional vancomycin dosing methods are unlikely to meet AUC24h /MIC targets. Depending on dialyzer type and whether vancomycin is administered during or after hemodialysis, loading doses of 25 to 35 mg/kg followed by maintenance doses of 7.5 to 15 mg/kg are necessary to reach minimum AUC24h /MIC targets in 90% of virtual patients. For a 3-day interdialytic period, a 30% higher maintenance dose is required to maintain target attainment. Dosing based on a single vancomycin serum concentration obtained prior to the second dialysis session greatly enhances the probability of target attainment.
The management of ovarian cancer requires complex surgical and medical interventions. Specialized care is associated with superior outcomes in early and advanced stages. This study aimed to estimate the effect of hospital characteristics on the overall survival of women with epithelial ovarian cancer.

We established a cohort with data recorded by the Fundação Oncocentro de São Paulo cancer registry. We included 6111 women treated for ovarian cancer in the state of Sao Paulo from January 2000 to December 2018. From 76 hospitals analyzed, 7 were high volume (20 or more cases a year) and 69 low volume. Twenty-nine were teaching and 47 community hospitals. A 10-year survival was analyzed using the Kaplan-Meyer estimator and the Cox model.

Fifty-two percent of the epithelial ovarian cancer patients were treated in high-volume hospitals. High-volume - (HR, 0.86; 95% CI, 0.8-0.92; P < .001) and teaching - (HR, 0.91; 95% CI, 0.85-0.99; P = .019) were hospital characteristics associated with low risk of death in 10 years.

High-volume and teaching hospitals are associated with better overall survival in ovarian cancer. Our data suggest that both hospital characteristics are important indicators of good quality of care in ovarian cancer treatment.
