Background Older adults living with HIV (OALWH) are a growing population facing unique challenges to successful antiretroviral therapy.Objective To assess efficacy and safety profiles of antiretroviral regimens, including those containing dolutegravir, in OALWH.Methods Combined data from 6 phase III/IIIb trials in treatment-naive (ARIA, FLAMINGO, SINGLE, SPRING-2; N = 2634) and treatment-experienced (DAWNING, SAILING; N = 1339) participants receiving dolutegravir- or non-dolutegravir-based regimens were analyzed by age ( less then 50, ≥50 to less then 65, and ≥65 years). Baseline data included comorbidities and numbers of concomitant medications. Week 48 efficacy outcomes included virologic response (HIV-1 RNA less then 50 copies/mL) and CD4+ cell count change from baseline. Safety outcomes included incidence of adverse events (AEs), serious AEs, and AE-related withdrawals.Results Use of ≥5 concomitant medications was more frequently reported among treatment-naive and treatment-experienced participants aged ≥50 to less then 65 (30% [90/296] and 25% [57/227], respectively) and ≥65 years (43% [10/23] and 29% [4/14]) than among those aged less then 50 years (13% [310/2315] and 11% [118/1098]). Comorbidities were more prevalent in the older age groups. There were no major differences in safety outcomes in each age group.Conclusions In these analyses of combined phase III/IIIb trial data, efficacy and safety of dolutegravir-based regimens were generally similar across age groups in treatment-naive or treatment-experienced participants. Polypharmacy and comorbidities were more common among OALWH than those aged less then 50 years.Beginning with the observation that a good integration of soma and psyche is often missing in our work with severely disturbed analysands, two concepts are discussed that focus constellations of bodily encoded unconscious material, which needs a specific form of working-through. The concept of encapsulated body engrams referring to defence structures revolving around the inhibition or disorganization of an affective bodily impulse is outlined. The imbalance arising this way is perceived as a foreign body within the body-self. These engrams are repetitive and are not susceptible to symbolic transformation. Parallels between the encapsulated body engram, the autistic objects (Tustin, F. 1980. "Autistic Objects." International Review of Psycho-Analysis 27 27-39) and Freud's theory of inhibition (Freud, S. 1926. "Inhibitions, Symptoms and Anxiety." SE 20 77-175) are discussed. To unblock encapsulated engrams, body feelings emerging in the chain of associations are understood as attempts to communicate. In a process called somatic narration these perceptions are contextualized with other bodily sensations. The working-through of this material in transference and countertransference is described in the light of Lombardi's concepts (Lombardi, R. 2017. Body- Mind Dissociation in Psychoanalysis - Development after Bion. New York Routledge). Two case reports precede the theoretical discussion.A total of 120 internet bought stainless-steel utensil samples intended for food serving were analysed for their release of Cr, Ni, Al, Fe, Mn, Cu, Zn, and Mo. Samples were extracted using simulants under continuous and discontinuous test conditions, extracts were analysed by ICP-MS, and metallic compositions of utensils were indicated by XRF analysis. r2 values for all analytes were determined between 0.9982 and 0.9997. RSD and WRm values ranged between 4.5-8.5% and 80-107% for all analytes, respectively, and LOD was in the range of 0.11-25.8 µg/l. Migration results show that 35% of total sample types show non-compliance with the SMLs established by the Nordic guidelines. XRF data show good correlation between the metals' migration levels and their respective compositions in the utensil, and toxicological exposure estimates no significant adverse health effects from food served with articles exceeding the SMLs for Fe, Al, and Ni.In two experiments, horizontal and vertical orientated sounds moved in parabolas. Participants had to touch a screen to indicate where and when a virtual moving ball would cross a visible line. We predicted that due to the sensitivity of the auditory system to temporal information, manipulations of pitch should affect temporal errors more than spatial errors. Stimuli were sound sources at five different pitches moving along a parabola produced through loudspeakers mounted around a touch screen. Results showed pitch effects on spatial constant and spatial variable errors when the parabola was horizontally oriented (Exp. 1), and on temporal constant errors in vertically oriented parabolas (Exp. 2). We conclude that temporal and spatial precision in interception tasks were affected differently by pitch manipulations and require consideration in future studies when assessing the impact of auditory information on catching virtually moving balls.In order to understand the 'late' Bion, i.e. his production from Learning from Experience onwards, the most difficult and controversial, it is worth re-reading not only the essays in Second Thoughts, but also Experiences in Groups. The hypothesis of this work is that Bion spent his life as a scholar - probably without even being fully aware of it, since he never talks about it - to transpose his theory of groups into his theory of individual psycho-analysis. To illustrate this thesis, the author systematically compares some of the principles of both. Going back to Experiences in Groups, then, is also useful for another reason. Bion revolutionises theory but still works as a Kleinian. Despite brilliant suggestions, he does not fully develop a new technique. To have a tool box that anyone can use, we have to wait the analytic field theory, which is an original development of his thought. In this model the analyst sees in the analytical pair not two isolated subjects that interact, but a group. There is no 'fact' of analysis that cannot be heard as unconsciously co-created.
