We present a case of a 64-year-old man with neuroendocrine carcinoma. Incidental findings were demonstrated on Ga-FAPI-04 PET/CT in the inferior wall of left ventricle. A diagnosis of old myocardial infarction was made based on typical electrocardiogram change. Our case suggests that Ga-FAPI PET/CT, as a noninvasive method to reflect fibroblast activation, is potentially feasible for assessment of cardiac remodeling after myocardial infarction in a clinical setting.Reports from the Institute of Medicine regarding the number of errors in medicine leading to fatalities prompted a major review of medical practices and a move to make medical systems high reliability organizations. We present an extremely rare case of a 67-year-old man with cerebellar medulloblastoma with residual disease after surgery, chemotherapy, and radiation therapy, depicted with Tc-HYNIC-[D-Phe, Tyr-Octreotide] SPECT/CT. Somatostatin receptor scintigraphy was performed for staging and for the possibility of treatment with somatostatin analogs or peptide radionuclide therapy.
Extramedullary multiple myeloma (EMM) is a subset of multiple myeloma with a poor prognosis. We report a rare case with biopsy-proven concurrent liver and mesentery primary EMM at the time of initial staging after serologic diagnosis of multiple myeloma. 18F-FDG PET/CT is valuable in detection of EMM when the patient has no osseous lesions and a negative bone marrow biopsy.

A 13-year-old boy with mediastinal T-cell lymphoblastic lymphoma demonstrated an altered biodistribution with diffuse activity in subcutaneous white adipose tissue and decreased visceral activity on interim posttreatment FDG PET/CT. This altered biodistribution was attributed to administration of the chemotherapeutic enzyme l-asparaginase 3 hours preceding the PET/CT, altering adipocytes amino acid and glucose metabolism. Treatment response assessment was adversely affected by the altered biodistribution, emphasizing the importance of maximizing the time between chemotherapy and PET/CT during successive oncologic treatment cycles. A 70-year-old man with lung cancer complained about arthralgia after 7 courses of the pembrolizumab therapy. Tc-HMDP bone scintigraphy revealed symmetrically strong uptakes in the major distal joints of the upper and lower extremities, thereby clearly identifying them as the affected joints. The pattern of uptakes was not consistent with that of other pathophysiologies including bone metastases, hypertrophic osteoarthropathy, and rheumatoid arthritis. Tc-HMDP bone scintigraphy is more practical and cost-effective compared with PET to reveal the affected joints in pembrolizumab-induced inflammatory arthritis.
The purpose of the study was to compare a single 2D image processing system (IMAGE) to underwater weighing (UWW) for measuring body volume (BV) and subsequently estimating body fat percentage (%Fat), fat mass (FM), and fat-free mass (FFM) via a 3-compartment (3C) model.
A sample of participants aged 18-39 years was recruited for this study (n=67, 47.8% female). BV was measured with UWW and predicted via the IMAGE software. The BV estimates from UWW (3CUWW) and IMAGE (3CIMAGE) were separately combined with constant total body water and body mass values for 3C model calculation of %Fat, FM, and FFM.
BV obtained from the IMAGE was 67.76 ± 12.19 L and 67.72 ± 12.04 L from UWW, which was not significantly different (p = 0.578), and very largely correlated (r = 0.99, p < 0.001). When converted to %Fat (3CUWW = 21.01 ± 7.30%, 3CIMAGE = 21.08 ± 7.04%, p = 0.775), FM (3CUWW = 14.68 ± 5.15 kg, 3CIMAGE = 14.78 ± 5.08 kg, p = 0.578), and FFM (3CUWW = 57.00 ± 13.20 kg, 3CIMAGE = 56.90 ± 12.84 kg, p = 0.578) with the 3C model, no significant mean differences and very large correlations (r values ranged from 0.96 to 0.99) were observed. In addition, the standard error of estimate, total error, and 95% limits of agreement for all three metrics were small and considered acceptable.
