Further investigation showed a small bowel obstruction caused by adhesions. After two days expectation, surgery be-came necessary. During laparoscopy, the surgeon's finger was introduced intraabdominally and used to release the adhesion. We here present a possible approach to a situation encountered during laparoscopic adhesiolysis and encourage creativity and laparoscopy in selected patients with mechanical small bowel obstruction.Urticaria has a high socioeconomic burden worldwide. However, the global epidemiology of urticaria and its geographical and temporal trends are not well studied. Using the Global Burden of Disease dataset, the aim of this study was to analyse the age-standardized prevalence, incidence, years lived with disability, and mean duration of urticaria in 195 countries from 1990 to 2017. In addition, the relationship between socioeconomic development and urticaria was evaluat-ed. The global prevalence of urticaria in 2017 was 86 million people. Females and children aged 1-4 years were more commonly affected than males and adults, respectively these differences were outside the 95% uncertainty intervals. Regression analyses show-ed that a lower gross domestic product per capita was associated with a higher prevalence and incidence of urticaria (p  less then  0.001). The global prevalence of urticaria, incidence, and years lived with disability have remained stable from 1990 to 2017.
This study aimed to investigate the effectiveness of brief training in the guideline-oriented biopsychosocial management of low-back pain (LBP) in occupational health services using a cluster-randomized design. A small sample of physiotherapists and physicians from the intervention units (N=12) were given three- to seven-day training focusing on the biopsychosocial management of LBP, while professionals in the control units (N=15) received no such training.

Eligible patients with LBP, with or without radicular pain, aged 18-65, were invited to participate. A web-based questionnaire was sent to all recruited patients at baseline, three months and one year. Between-group differences were analyzed using linear and generalized linear mixed models adjusted for baseline-response delay as well as variables showing between-group imbalance at baseline.

The final study sample comprised 234 and 81 patients in the intervention and control groups, respectively at baseline, and 137 and 47 patients, respectively, at one year. At baseline, the mean duration of pain was longer in the intervention group (P=0.017), and pain-related fear concerning physical activity was lower (P=0.012). We observed no significant difference between the groups' primary outcome measure (adjusted one-year mean difference in the ODI 2.3; 95% confidence interval -1.0-5.7; P=0.175) or most secondary outcomes.

It implies that an intrinsic 'winter clock' synchronizes bloom with the progression of winter to spring. The carbohydrate-temperature (C-T) model established a mechanistic association between carbohydrate metabolism in dormant trees and hourly winter temperatures. Using historical winter temperature and bloom times of Prunus dulcis (almond), Malus domestica (apple), Pistachia vera (pistachio), and Juglans regia (walnut) in California and Washington states, we parametrized species-specific metabolic parameters to the C-T model. There was a sound fit between actual and projected bloom dates with a deviation (RMSE) of 4-7 days in all species. The parameterized model enabled us to study how the observed variability in soluble carbohydrate concentrations at senescence (SC0) could affect bloom time. The C-T model projected that low SC0 could advance, and high SC0 possibly delay the bloom of the early-blooming almond trees. In contrast, high SC0 would advance the bloom of apple, pistachio, and walnut trees. These novel projections suggest that, after experimental validation, SC0 could guide post-harvest farming applications that affect fall carbohydrate accumulation to mediate climate shifts' effects.Dormancy release and reactivation of temperate-zone trees involve the temperature-modulated expression of cell-cycle genes. However, information on the detailed regulatory mechanism is limited. Here, we compared the transcriptomes of the stems of active and dormant larch trees, emphasizing the expression patterns of cell-cycle genes and transcription factors, and assessed their relationships and responses to temperatures. Twelve cell-cycle genes and 31 transcription factors were strongly expressed in the active stage. Promoter analysis suggested that these 12 genes might be regulated by transcription factors from 10 families. Altogether, 73 cases of regulation between 16 transcription factors and 12 cell-cycle genes were predicted, while the regulatory interactions between LaMYB20 and LaCYCB1;1, and LaRAV1 and LaCDKB1;3 were confirmed by yeast one-hybrid and dual-luciferase assays. Last, we found that LaRAV1 and LaCDKB1;3 had almost the same expression patterns during dormancy release and reactivation induced naturally or artificially by temperature, indicating that the LaRAV1-LaCDKB1;3 module functions in the temperature-modulated dormancy release and reactivation of larch trees.
