Severe vehicle crashes have resulted in a large-scale social and economic loss. As a result, the reduction of those crashes has become one of the key objectives of policy makers. Although traffic barriers have been utilized to reduce the run-of-road crash severity, still those crashes account for a high number of severe crashes. Previous studies of traffic barrier crashes often either ignore the heterogeneity across different traffic barrier types, or just focus on specific types of traffic barriers. Thus, this study developed a Bayesian Hierarchical model (BHM) to identify the contributory factors impacting the severity of traffic barrier crashes while accounting for that heterogeneity. The assessment of model fit, inter-class correlation (ICC) coefficient, and deviance information criterion (DIC) all favoured the use of the BHM. Besides accounting for the heterogeneity between barrier types, the interaction across variables shoulder width and traffic barrier height were incorporated into the analysis. Due to the differences in traffic barrier design and vehicle performance across different roadway classifications, only Wyoming interstate traffic barrier were considered. Results indicated that there is an important interaction term between traffic barrier height and shoulder width so that the impact of these two predictors should not be separated. In addition, having a citation record, negotiating a curve, being a female driver, non-speed compliance, alcohol involvement, and showing emotional signs at the time of crash were factors increasing the severity of traffic barrier crashes. On the other hand, having a turn before hitting a traffic barrier, being a younger driver, and driving in adverse weather conditions were factors that significantly decrease the severity of traffic barrier crashes.Anesthetic management for anterior mediastinal mass resection is often challenging. The main concern being that the tumor might, on reduction in muscle tone, cause circulatory and/or airway collapse. In the setting of pregnancy, the expected physiologic changes (eg, increased oxygen demand, decreased functional residual capacity, and aortocaval compression) may further increase the risks. The objective of this report is to present a challenging case of a pregnant woman undergoing an anterior mediastinal mass resection with the additional rare requirement for one-lung anesthesia, and to describe the perioperative considerations and the plan executed to ensure a successful outcome. A 30-year-old pregnant (23 weeks) patient with a large anterior mediastinal mass and evidence of significant cardiovascular and tracheobronchial compression presented for thymectomy requiring one-lung ventilation. Anesthesia consisted of preoperative preparation, thoughtful selection of vascular access sites, preservation of spontaneous ventilation until sternotomy was accomplished, use of bronchial blocker and readily reversible pharmacologic agents, availability of backup airway and oxygenation plans, standby high-frequency ventilation, and anticipation of postoperative respiratory difficulties. Surgical considerations included the possibility of extracorporeal membrane oxygenation and the need for lifting the thymoma to relieve the compression of the mediastinum. A methodical and multidisciplinary plan is described to mitigate the risk of cardiorespiratory collapse in the setting of anterior mediastinal mass resection. Backup measures in case of catastrophe, as well as careful consideration of the physiologic changes of pregnancy, must be taken into account.Staphylococcus aureus is a leading cause of infections world-wide. To this end, we established two in vitro infection models based on primary human umbilical vein endothelial cells (HUVEC), which mimic conditions of the endothelium when infection occurs. For comparison, the laboratory strain S. aureus HG001 was used. As shown by flow cytometry and fluorescence- or electron microscopy, differentiation of HUVEC into a cell barrier with cell-cell junctions sets limits to the rates of bacterial internalization, the numbers of internalized bacteria, the percentage of infected cells, and long-term intracellular bacterial survival. Clear strain-specific differences were observed with the HG001 strain infecting the highest numbers of HUVEC and displaying the longest intracellular persistence, whereas the MRSA strains reproduced faster intracellularly. Nonetheless, all internalized bacteria remained confined in membrane-enclosed LAMP-1-positive lysosomal or vacuolar compartments. Once internalized, the bacteria had a higher propensity to persist within the differentiated endothelial cell barrier, probably because internalization of lower numbers of bacteria was less toxic. Altogether, our findings imply that intact endothelial barriers are more likely to sustain persistent intracellular infection.Purpose Using a theory-led action research process test applicability of humanizing care theory to better understand what matters to people and assess how the process can improve human dimensions of health care services. Consideration of the value of this process to guide enhancements in humanly sensitive care and investigate transferable benefits of the participatory strategy for improving human dimensions of health care services. Methods Action research with service users, practitioners and academics, with participatory processes led through the application of theory via a novel Humanizing Care Framework in two diverse clinical settings. Results Participants engaged in a theory-led participatory process, understood and valued the framework seeing how it relates to own experiences. Comparative analysis of settings identified transferable processes with potential to enhance human dimensions of care more generally. We offer transferable strategy with contextualized practical details of humanizing processes and outcomes that can contribute to portable pathways to enhance dignity in care through application of humanizing care theory in practice.
