In this study, an ultrasensitive monoclonal antibody (mAb) was prepared and used to develop an indirect competitive enzyme-linked immunosorbent assay (ic-ELISA) and a colloidal gold-based immunochromatographic strip (CG-ICS) for the analysis of fentanyl in urine and serum. Under optimum conditions, the anti-fentanyl mAb belonging to the subtype of IgG2b showed a half-maximal inhibitory concentration (IC50) of 0.11 ng mL-1 and a linear range of detection of 0.020-0.50 ng mL-1. Fenanyl-spiked original urine and serum diluted eight times were used for the analysis of fentanyl by ic-ELISA and CG-ICS. IC50 from the standard curves was 0.46 ng mL-1 for urine and 2.6 ng mL-1 for serum in ic-ELISA and 1.6 ng mL-1 for urine and 6.27 ng mL-1 for serum in CG-ICS. The recovery test revealed that the ic-ELISA and CG-ICS, with a recovery rate of 87.0-108.4% and a coefficient of variation of 3.3-10.9%, were the same reliable tools as the liquid chromatography tandem mass spectrometry for fentanyl analysis in real samples.A density functional theory based study is presented with the aim of addressing the surface energy stabilization mechanisms of transition metal carbide and nitride surfaces from a crystal structure different from that of the most stable polymorph. To this end, we consider the MoC(001), MoN(001), WC(001), and WN(001) surface of rocksalt structures, which, for these compounds, is not the most stable one. The geometry optimization of suitable slab models shows that all these surfaces undergo a sensible reconstruction. The energy difference per formula unit between the rock salt and the most stable polymorph seems to be the driving force behind the observed reconstruction. A note of caution is given in that certain small periodic boundary conditions can artificially restrain such reconstructions, for which at least (2×2) supercells are needed. Also, it is shown that neglecting such a surface reconstruction can lead to artifacts in the prediction of the chemical activity and/or reactivity of these surfaces.The poor health outcomes of Renal Replacement Therapy (RRT) in the elderly has promoted Conservative Management (CM) as a therapeutic option in advanced chronic kidney disease. However, there is still a lack of evidence about prognosis of these patients; thus, the aim was to analyze the survival rate of elderly patients under CM and RRT and evaluate the variables related to the initiation of such treatments in clinical practice.
Prospective cohort study of RRT and CM patients >75años. Renal function parameters and geriatric assessments were carried out. This evaluation included analysis of comorbidity, functional, cognitive, frailty, nutritional and socio-family status.

Cohort of 37 RRT and 82 CM patients. CM patients were significantly older, with more frequency of history of vascular event, more comorbility (Charlson), worse functional situation (Barthel), higher risks of cognitive impairment (Pfeiffer) and malnutrition (MNA-SF), and higher frailty and socio-familiar impairment. Mortality rate was lower in RRT patients (8.72 vs. 3/1,000 patients/month; HR= 0.37, p=0.018), but survival advantage reduced drastically after adjustment for the different geriatric syndromes analyzed.

Charlson's comorbidity was found to be an independent mortality predictor in elderly patients with advanced chronic kidney disease. Dialysis did not improve survival with respect to conservative treatment in patients with Charlson higher than 8 points.
